What to add?

Summary

What to add to tirzepatide to restart weight loss after gastric bypass and a stall?

The original poster describes a long history of obesity, stroke, bypass surgery, and partial success on tirzepatide now stalled at 7.5 mg, and asks what peptide to add next such as reta, cagri, or GHK-cu. Replies focus on blood-pressure control given the stroke history, mention of 5-Amino-1MQ during prior stalls, and caution about adding reta versus simply raising the current dose or addressing labs and body recomposition. Several users share their own stacking experiments or side-effect management.

Hi, the more I learn about each peptide the more I feel I want them all, but that would mean twenty at once which clearly can't happen. I'm trying to decide where to begin adding something to Tirz. Quick background: woman in my sixties, obese my entire life. Stroke three years ago at 340 pounds. Gastric bypass a little over two years ago, dropped roughly 70 pounds then progress stopped. Tried Mounjaro for about eleven months and lost another 44 pounds but stalled the last three or four months. Now at 7.5 mg and switched to gray Tirz a month ago. Planning to go up to 10 next month to see if loss starts again. What extra peptide or trick helped others keep losing? Still need to drop at least fifty more pounds. Considering Reta or Cagri. Also thinking GHK-cu because skin is getting loose, and maybe mitochondria repair too. Too many ideas at once. Any thoughts?

Congrats on the progress so far! No ideas from me. I'm looking into stacking with Tz myself so I'll be watching this thread. Hope you get useful replies. Are you also on Peppy's or similar places?

After reading a lot I'm getting ready to add low-dose Reta once a week on a different day from my 7 mg Tirz.

How are the lab numbers looking? Blood pressure okay? Raising Tirz seems simpler than bringing in Reta and avoids the heart-rate issue. Cagri feels overrated to me, which is why Reta and Tirz come up more. When the scale won't move I often look at labs or try to add muscle instead. I've been sitting at 200 for months but my clothes are smaller so that's a win.

Bloodwork two weeks ago came back great. A1c at 5.3, never diabetic but it had hit 6.0 before. Stopped thyroid meds last June and levels are fine now. Maybe skip Reta for now and try Glow/Klow for the loose skin. Does it really sting that much? That part concerns me. Blood pressure is still high at around 140/90 even on 10 mg Lercanidipin.

CopperSignal27archiveMar 2

Given the stroke history, is there a reason blood pressure isn't being pushed lower toward 120/80? 140/90 still raises the chance of another stroke or heart issues. Do you check it at home with multiple readings? Have you talked to the doctor about it? Leaving it undertreated is pretty common. Tirzepatide might lower it eventually with more weight loss, but after a stroke it seems safer to address sooner.

I tried SLU-PP-332 and 5-Amino-1MQ during my two worst stalls. The 5-Amino-1MQ worked better for me with no other changes. This is just my experience and I used the oral version then, though a shot version exists now and might absorb better. Nice job by the way. I started near 600 pounds and needed both surgery and a GLP-1 to reach 179. You've got this.

Congratulations. Sometimes both surgery and a GLP-1 are needed to get there.

Lipids also looked good? Do they test vitamin D where you are? They rarely do here unless asked. Deficiency often links to stiffer arteries and higher pressure, and regular panels miss it. I'd get serious about that number. Tried the DASH diet? There are several medical eating plans for pressure, including starch-based ones. Intermittent fasting has also appealed to me because it's less limiting overall. I still eat out a few times a week.

Lipids were fine. HDL good, LDL slightly up from occasional keto but not worried. Had a CAC scan after the stroke and arteries were clear so I'm not taking statins. Vitamin D came back a little low so I'm supplementing now. I'll track blood pressure more often. It was 135/80 an hour ago, which feels acceptable.

Your view still sounds pretty old-school unless the doctor thinks the same way. Some supplements can move blood pressure numbers more than expected.

SharpCinder28archiveMar 2

Systolic Blood Pressure Intervention Trial results showed that aiming below 120 gives clear heart benefits in higher-risk patients compared with the usual below 140 target.

BrightSignal76archiveMar 2

Similar situation here. Stalled on Reta after reaching 12 mg because sleep got wrecked. Dropped back to 8 mg weekly on Sundays and sleep improved a bit, scores now 60-75 instead of 40-50. Still not ideal but night sweats and hourly waking are mostly gone. Thinking about adding Cagri since hunger returns hard toward the end of the week. Tried Tesamorelin for visceral fat but both hands developed bad carpal tunnel after a month. Low-dose Cagri with current Reta seem like the next move? Don't want to undereat while on test and aiming for 200 g protein daily.

Have you tried splitting the Reta dose across two days? That sometimes reduces side effects. The main issue with Cagri for me is the wait to dial it in and then reports that it stops working as well after a while. At best a temporary stall breaker. Still some FOMO though. Raising the GLP with split dosing feels simpler and safer than adding Cagri. I've been cutting coffee creamer, doing more intermittent fasting, and using fewer fats to manage higher-dose side effects.

Split dosing didn't help much and actually made sleep worse. Took the 8 mg yesterday and it's hard not to head downstairs and eat, yet I still need the protein. Daytime fasting means missing the protein target. Might need hypnosis. I'll probably just order more salads and push through the cravings.

What made you choose that instead of just increasing the Tirz dose?

SharpCinder28archiveMar 3

For a sense of how well Cagri actually performs: the trial details are on the maker's site.

WryHarbour76archiveMar 3
↳ replying to @NorthBramble29

Probably just the appeal of something new. I've got years of Tirz on hand at the current dose but know I'll need to raise it soon anyway. Reta looks like the next big thing so adding a bit might capture some of its different effects.

CopperSignal27archiveMar 3

Statins cut stroke and vascular dementia risk quite a bit, especially after one has already happened. Pairing them with tighter blood-pressure control could roughly halve the chance of another stroke. That difference might be worth another look.

I've been on Lipitor for almost twenty-five years, now the generic. It was my first prescription and I felt odd about it at first, but seeing the long-term benefits and that doctors take it themselves made me glad. At my age it's probably been one of the best things, and I've taken CoQ10 with it the whole time.

Just joined Peppy's. It's a lot to take in at once. Do we know the exact differences from this group? Seems more research-focused and private. Still figuring it out.
