Does your food noise come back when it’s time to up your dose?

24 replies22 peopleMay 7, 2026☆ Follow
Summary

Does food noise or cravings return before it's time to increase the dose on these medications?

Many users report that hunger and food noise tend to reappear toward the end of their dosing interval, often around day 4-6, which they interpret as a signal to raise the dose or shorten the interval. Experiences vary widely, with some maintaining steady control on low weekly amounts for weeks while others adjust frequency to every 4-6 days or split doses to reduce peaks and valleys. Weight loss itself can increase baseline hunger, requiring the medication to work harder over time. Individual responses differ enough that what works for one person may not translate directly.

What this discussion establishesWhere people disagree

Whether returning hunger signals the need for a higher dose, a dosing schedule change, or simply reflects normal adaptation to weight loss.

I've noticed cravings and food noise returning around the point when I should raise my dose, and I'm wondering whether that's typical or if it means something isn't right. It feels milder than it used to be. Right now I'm at 8mg of triz.

This pattern shows up often with single weekly injections but shows up less when people split the dose. It might point to needing a higher amount. You left out how long you've been on it and which compound.

NorthMarble58archiveMay 11

The same thing happened to me on Triz. I could actually sense when the next dose was due.

PlainAnchor17archiveMay 11

Splitting 2mg twice a week for a 4mg total has worked well. I notice I can eat a little more right before the next pin, so I time the shots for Sunday and Wednesday nights to handle weekends better when others are watching. I went through a full kit that way last year.

SteadyCinder74archiveMay 12

I began with weekly shots just over four weeks ago. From the start, evening snacks sounded unappealing. Then two days ago I suddenly wanted something salty and crunchy at night. I raised the dose the next day and the urges disappeared again. So yes.

PatientBeacon95archiveMay 12

I don't get hungrier overall. I aim for every Sunday but sometimes forget, so I take it when I remember and shift the day going forward if several days have passed.

BrightPebble45archiveMay 12

It sure does. I like that it shows the medication is working and then confirms the effect has worn off for the week.

WarmHarbour28archiveMay 12

Whether I'm at 10mg or 15mg of tirz, a full week feels too long. I always have a bit extra so I add another pin on Thursday or Friday. I don't see how splitting would work without under-dosing and wasting some.

GreyBramble56archiveMay 12

I'm on reta at about 1.2mg every five days. To your question, yes, around the fifth day I start noticing cravings returning. I might go up later, but five days is working so far.

BlueKettle52archiveMay 13

Reading all this makes me think I'm unusual. I've used 1mg of reta weekly for four weeks. Only in the first week did I have brief mild cravings for a couple hours right after the first shot. Since then I've had no hunger or noise for three straight weeks at that same low weekly amount. People just respond differently.

LevelCinder33archiveMay 15

I've been on reta for three months at 2mg. Only in the last two weeks has food started returning. I pin on Saturday and by Thursday the noise begins creeping back in.

QuietFenwick11archiveMay 15

Not a problem for me. I stay with once-weekly dosing. Some days I'm hungrier than others but it hasn't changed the scale. If I want dessert I have it and feel satisfied afterward.

This compound builds up in the system over time even with weekly use, and faster if dosed every few days. Comparing plots shows the peaks and valleys change. I went by the scale rather than the calendar or how I felt, staying at a dose as long as loss continued. Some hunger is fine if you make good choices then, like soup for volume with protein.

WarmQuill25archiveMay 16

Of course it does. From my experience this happens when the dose is still too low. Below 3mg I felt hunger returning one or two days early and had trouble staying in a deficit. At 3mg it improved and at 4mg even more so. You're on 8mg of tirz though, so that's surprising. How long have you been using it? Maybe the batch isn't as strong as labeled.

SharpLedger37archiveJun 1

I began at 0.5mg weekly, went to 1mg after three weeks, then added 1mg every four weeks up to 4mg. By 2mg I saw strong cravings on day six so I switched to every six days, which is working. I've now reached 5mg and feel okay there.

ClearBeacon15archiveJun 2

I'm at 10mg of tirz and try stretching to two weeks. Lately the noise returns by the end of the first week. I've been on the 10s for years now. Not sure what's changed. Might switch compounds next time.

Yes, by afternoon of day five on reta the noise starts returning. I changed to dosing every four days and that fixed it.

I don't think solid evidence shows tolerance building to the appetite or weight-loss effects, but as weight comes off hunger naturally rises so the current dose has to overcome that extra drive. It would help to know how much weight was lost between dose changes or whether efficacy truly felt lower.

SteadyKettle33archiveJun 2

Dosing my tirz every six days removed that issue completely, though I'm only at 7.5. When I first stepped up I felt it wasn't working as well so I moved to the current level. Sometimes I still do the full seven days just to feel the hunger again as a reset. I shorten the interval rather than stay at a dose that lets noise and cravings return. If controlling your own amounts, a small increase after enough time at the current level can help.

I have another idea about hunger noise that has worked for me. I'm 58, 1.80m, started at 223 and reached 170 in eight months on a low dose that stayed at 5mg then dropped for maintenance. I never had real noise because I kept protein and fiber high and avoided carbs and fats. Sugar cravings faded on their own. I think noise returns when the body is undernourished, and raising the dose at lower weight only makes sense if you can't meet needs otherwise. Occasional thoughts of food are just signals, not something to fear.

AmberBeacon78archiveJun 2

On tirz I followed the usual start at 2.5 and added 2.5 every four weeks. At 7.5 I tried staying longer since loss was steady, and it still is, but now hunger clearly returns around day four or five. I'm treating it as practice for tracking and eating through the noise. I'll probably move to 10mg this week, but see the returning noise as training for eventually stopping.

PatientHarbour23archiveJun 2

When the noise gets a little louder that's usually my cue to increase, but I only go up by a unit or two at a time.

What you're doing clearly works for you so it's the right approach. Losing that much on a low dose is better than average and keeping it off on even less is notable. Food choices do help control hunger, and the way you eat lines up with what tends to work well.

WarmHarbour28archiveJun 3

More interesting are the strong responders who keep the effect even in maintenance. I know two people, one a doctor, who reached goal on under 10mg of tirz then stopped entirely, saying they'd take it only if needed. They haven't needed it. Others feel like much higher amounts do nothing. I'll keep going until late August and then stop at whatever weight I hit.

Thanks for the reply. Early on my doctor checked blood every four weeks including pancreas values and everything stayed fine. Later my muscles softened and training was the only advice given. I added other compounds on my own and noticed a difference after the first week. Some hunger appeared but I could ignore it by pushing breakfast later. Cravings show up mid-morning but fade after an hour if I don't act on them. By midday the signal is gone and a small meal ends it.
