Doses Where Tirzepatide and Retatrutide Start Working for Weight Loss and Stacking Experiences

48 replies20 peopleApr 22, 2026☆ Follow
Summary

At what doses of tirz or reta do people first notice real weight loss, and how does stacking the two compare to using either alone?

Responses vary widely: some report appetite changes and steady loss starting at 2 mg reta or 2.5 mg tirz, while others see little until 8–10 mg tirz or higher reta doses. Stacking is described as less predictable than a simple combined GLP dose, with some users keeping both for pain relief or food-noise control and others preferring to switch entirely. Slow loss of roughly one pound per week is viewed by several as acceptable and potentially better for skin and muscle preservation.

What dose of tirz or reta made you first notice real weight loss and body changes? Did it happen right away or only after you reached a certain level? For those stacking both, do you treat them as two separate things or just add the weekly totals together? I began with 1 mg tirz and 0.5 mg reta and after two weeks was already using both. Now in week nine I am at 3.5 mg tirz and will move to 2 mg reta soon, for a weekly total of 5.5 mg. Does that still count as low? Since the start of the month I have only dropped another 2.6 lb despite the increases, bringing my total to 12.6 lb. I know results differ, but the bigger numbers others report make me feel behind. I still have about 40 lb to go from my original 51 lb goal. My diet is solid, protein is adequate, and I try to stay hydrated.

PlainCompass54archiveApr 22

I needed to reach 10 mg of tirz before it really started working well for me.

PlainAlder11archiveApr 22

I noticed effects right away at 2 mg reta and they are still going strong at 6 mg.

For me 2.5 mg shut down appetite within a couple of hours. I later went up to 5 mg but now think that step was unnecessary. My sister needed a few weeks at 2.5 mg before she felt much appetite suppression. A friend of mine saw nothing until she reached 5 mg. There are clearly a lot of individual differences.

I have had to titrate very slowly because of gastroparesis risk. After six months I am around 3 mg tirz and have lost about 30 lb, roughly a pound a week on average with some plateaus. Your pace seems reasonable to me. Slower loss can actually help with skin and loose-skin concerns. Besides the usual individual variation, I would check total calories and food balance, daily movement beyond workouts, water retention after dose changes, and how well the current doses are tolerated. Stacking tirz and reta does not behave like a simple combined dose.

30 lb is great. I am also a slow titrator and so far have had no sides. I had no appetite before starting and could not lose even at 900–1100 calories, so my metabolism was clearly not cooperating. Reta actually brought my appetite back, which I hope is a good sign. I still have good control around food and sugar cravings are mostly gone. I suspect under-eating is part of my problem now, so I am aiming for 1200 calories with a protein shake plus collagen and flax as an evening option. We cook mostly whole foods at home. Exercise is inconsistent because of pain, but tirz has helped that a lot. When I stopped tirz the pain returned, so I am keeping both.

That is good to hear and still a modest dose. I had no appetite before starting either and could not lose even with phentermine and other attempts. It is discouraging to see people drop 10–12 lb a month when it has taken me more than two months for just over 2 lb recently. Still, it is better than before, so I am trying to stay grateful.

IronLantern24archiveApr 23

Reta has worked for me from the first day. This is the first week since January 5 that I have not lost anything.

WryHarbour70archiveApr 23

Everyone is very different. Anything over 2 mg wrecks my sleep and leaves me able to drink only shakes, so 1–1.5 mg is my usual dose. Some people feel almost nothing until twice that amount.

PatientAlder89archiveApr 23
↳ replying to @IronLantern24

What dose are you at now? I am sure the scale will move again soon.

KeenPebble33archiveApr 23
↳ replying to @PatientAlder89 (opening post)

You do not have to guess. Published studies show expected weight-loss ranges at different doses. For tirz the best return seems to be around 10 mg per week; 15 mg adds only a few extra percentage points over a year. For reta the stronger effect appears near 8 mg. Below those levels you are in a lower-response range that may be enough for small body-fat tweaks but not for losing 15–20 percent of body weight.

PatientAlder89archiveApr 23

You sound like a strong responder, similar to my husband who is still on 1 mg. I am going to try 2 mg this week and hope it moves the scale without hurting sleep. Responding well at such a low dose is fortunate.

IronLantern24archiveApr 23
↳ replying to @PatientAlder89

I just started 5 mg this week. The stall might be from creatine I took last week; I forgot it causes water retention.

PatientAlder89archiveApr 23
↳ replying to @KeenPebble33

I am definitely not at a healthy weight. I started with more than 50 lb to lose and still have about 40 lb left. 8 mg sounds high, but I hope I can increase without sides. Everyone reacts differently.

PatientAlder89archiveApr 23
↳ replying to @IronLantern24

Creatine can definitely cause retention. Hormonal shifts and other factors can too. Next week should make up for it.

KeenPebble33archiveApr 23
↳ replying to @PatientAlder89

That is what the data indicate. There is little difference between 8 mg and 12 mg for reta. On tirz I had no sides at all and actually felt better because food noise disappeared. On reta I developed skin sensitivity at 8 mg, so I switched back to tirz and am very happy with it. I have lost 60 lb in a year on 10 mg.

PlainMeadow55archiveApr 23

I felt it within 30 minutes of the first injection at my starting dose of 2.2 mg tirz. Food noise vanished and appetite dropped sharply. I have titrated to 4.4 mg and plan to stay there as long as it continues working. I track calories in and out the way I did before starting. It is almost working too well; some days I struggle to reach even a minimum calorie target. The weight is coming off steadily.

PatientAlder89archiveApr 23
↳ replying to @KeenPebble33

Congratulations on 60 lb. I have not reached 3 mg yet and will keep increasing only while results improve. I do not want to lose sleep or run into other issues. I am actually sleeping better than before starting both. Tirz has also helped noticeably with pain, though not completely.

PatientAlder89archiveApr 23
↳ replying to @PlainMeadow55

Glad the weight is dropping for you. I hope you reach your goal soon. I already had little appetite before starting and still do not think about eating much, but I am working on getting enough calories. Long-term very low intake probably was not helping me.

NorthSignal21archiveApr 23

I have been stacking reta with cjc/ipa and another peptide. Recovery from injuries has improved and I can lift more in the gym, which has been great. Weight loss and skin changes from the reta have not appeared after six weeks, so the other compounds are probably driving the strength gains.

PatientAlder89archiveApr 23
↳ replying to @NorthSignal21

Maybe the heavier lifting is adding muscle that offsets the scale. Have you been including carbs? Some people find they need them to lose on reta.

WarmSparrow81archiveApr 23

At 0.25 mg tirz, appetite suppression started within hours of the injection and some circulatory or nerve symptoms improved within a day or two.

KeenSignal54archiveApr 23
↳ replying to @PatientAlder89

At your current rate you will pass my 30 lb mark well before six months. We seem to have a lot in common, including exercise habits. Years of 900–1000 calories without loss usually means the body has adapted and is conserving energy. The fact that reta restored appetite may be a positive signal rather than something to override. It is still hard to tell whether reta is clearly stronger for you. I ended up staying with tirz and will probably save reta for later, possibly only at maintenance.

PatientAlder89archiveApr 24
↳ replying to @KeenSignal54

I am looking forward to seeing bigger numbers too and will just keep going. The 900–1100 calorie range was my norm for years with no loss. Now I aim for about 1200, which seems to be the recommended minimum for women. I agree the appetite return on reta could mean something was reactivated. I have been stacking since week two. I had planned to drop tirz but pain started returning after a week, so I kept both.

PatientAlder89archiveApr 28

This week I moved tirz to 4 mg and will keep reta at 2 mg. Last week the scale did not move at all. I know 5 mg tirz is often where results improve, but I hoped the 2 mg reta would help enough together. Hunger is low, eating is on track, and I am doing some exercise, but I really want to see the scale drop.

ClearHarbour45archiveApr 28

At 10 mg reta plus 1.5 mg sema I hit a stall. Adding 12.5 mg tirz with 6 mg reta broke the stall.

PatientAlder89archiveApr 28
↳ replying to @ClearHarbour45

I guess I will have to keep increasing if this week at 4 mg tirz and 2 mg reta does not produce movement. I hope that is not necessary since I am only two and a half months in.

QuietLedger67archiveApr 29

I used tirz and had steady loss from the starting 2.5 mg dose. I never needed more than 7.5 mg to reach goal and now maintain on 1.25 mg every one to two weeks. My budget appreciates the lower dose.

PatientAlder89archiveMay 12
↳ replying to @QuietLedger67

How long did it take you to reach 7.5 mg?

PlainAlder11archiveMay 12

It actually started at 2 mg; I am now on 8 mg and have lost 40 lb.

AmberPebble13archiveMay 12

I am new here and did not realize stacking tirz with reta was an option. What is the benefit of using both?

GreenBeacon77archiveMay 13

There is conflicting information. Many people are trying it, but the general view seems to be that it is not ideal. I recently started reta after seven months on tirz and will reduce tirz while increasing reta, overlapping only briefly. Some use tirz for better food-noise control because reta does not suppress it as strongly. Using both means doubling the GLP component, which is why I plan to drop tirz once reta reaches an effective dose. I am mainly interested in maintenance, so more data would be helpful, but that may not come soon since reta is not yet released.

PatientAlder89archiveMay 13
↳ replying to @AmberPebble13

I started on tirz, then decided to switch to reta. After stopping tirz I realized it had been helping with pain that I had not noticed until it returned, so I added it back.

AmberPebble13archiveMay 13
↳ replying to @GreenBeacon77

I wondered whether there is an advantage to having less glucagon activity and more of the other two receptor effects.

LevelThistle44archiveMay 13

My loss has been steady across doses and as weight has come down; there has not been one clear tipping point where efficacy suddenly changed.

WarmSparrow60archiveMay 13

Tomorrow will be shot 52, one full year. All tirz plus some other peptides starting at the six-month point, at 15 mg. I am down 97 lb, a 34.6 percent total loss. Most of the weight came off by month nine on tirz alone. I never used reta but bought a couple of kits and put them in the freezer, mostly out of curiosity. Stacking now seems unlikely. Next year the focus is adding muscle and improving body composition, especially legs and glutes. The lower abdomen still has about an inch of subcutaneous fat left. I have never been this consistent with self-care.

PatientAlder89archiveMay 13
↳ replying to @LevelThistle44

That is impressive. I wish my results were moving the same way. The past few weeks the scale has been stuck even though I added exercise and am eating less because appetite is lower. I started a new cartridge of tirz last week and may have taken 5 mg by mistake because I used the wrong reconstitution volume. I had planned to reach 5 mg anyway, but still no loss. You are getting great results. Did you stall and increase because of that?

LevelThistle44archiveMay 13
↳ replying to @PatientAlder89

I have not stalled. I have increased based on how well symptoms are tolerated and on measurable markers. The main temporary plateaus have come from water retention caused by TRT or, more recently, creatine; the creatine retention does not resolve unless the creatine is stopped.

PatientAlder89archiveMay 13
↳ replying to @LevelThistle44

I was wondering whether each increase happened because the previous dose stopped working. That may be what I need to do, just keep going up.

LevelThistle44archiveMay 13
↳ replying to @PatientAlder89

You do not see meaningful plateaus in my data. With a large amount of weight still to lose, staying low and slow can waste time. The trials titrated on a fixed schedule and produced predictable average results. I am above those trial doses but expect to lower the dose later once I reach a maintenance equilibrium.

PatientAlder89archiveMay 13
↳ replying to @LevelThistle44

Not at all. What tells you it is time to move up? I still have 40 lb to lose, which feels substantial. I am usually sensitive to medications, so I have increased slowly to stay functional. I am now at 5 mg tirz and will split 3 mg reta this week. Any side effects for you?

AmberPebble13archiveMay 13
↳ replying to @LevelThistle44

That chart is impressive. How has your energy been over that long period? Congratulations on the progress.

LevelThistle44archiveMay 13
↳ replying to @PatientAlder89

I increase when loss stays under about 1.1 percent per week and sides plus other markers stay acceptable. I have missed no work days from sides. Energy is fine aside from occasional tiredness linked to sleep. RHR is up roughly 8 bpm. HRV can dip with increases but has not been a real concern. No upper GI issues at all. Sleep can be affected but my tracked sleep debt has not exceeded five hours. I also have about 40 lb left. I was going to the gym daily until March and need to restart that habit.

BlueAnchor63archiveMay 13

I am a 205 lb guy using 2.5 mg tirz and 2 mg reta. Things really started moving two weeks ago. I am fairly active and after a three-hour bike ride finally broke a plateau. It took more than two months without the medications to drop from 210 to 202 on diet alone. Now I am losing about 2 lb a week without much effort. A big training day or a true fast day might help others break through too. I also realized how much my liquid calories, especially beer, were affecting results.

PatientAlder89archiveMay 13
↳ replying to @LevelThistle44

You are not losing too fast at all and have already dropped more than half of what you wanted. My sides have been minimal so far. I do not know my resting heart rate from before, but it is now in the 70s. I have also noticed fewer GI issues than before starting. Sleep was poor previously and is a bit better now. We both have 40 lb left. I am sure you will finish well ahead of me at your pace. My issue was never how much I ate, probably too little, but something internal that no doctor has identified. Some people just respond differently. I am tired too.

LevelThistle44archiveMay 14
↳ replying to @PatientAlder89

I judge increases by keeping sustained weekly loss around or below 1.1 percent. With a large total to lose I expect to need a higher dose during the cut. Maintenance will likely be a different equilibrium point between dose and my body's set point. My resting heart rate sits in the high 60s to low 70s, which does not worry me. Good luck managing your situation.

KeenPebble10archiveMay 14

I started reta at 2 mg and responded well. After about five weeks I went to 3 mg, then at eleven weeks to 4 mg where I am now. Response remains good and I plan to stay at 4 mg for another four or five weeks before reassessing.

PatientAlder89archiveMay 14
↳ replying to @LevelThistle44

I would like to speed things up from the current stall, so I probably need to keep increasing. I just do not want to feel terrible or run into problems. Thanks, and I hope the nap was good. I can rarely allow myself one unless I am sick. Hope you slept well afterward too.

ClearBramble68archiveMay 14
↳ replying to @GreenBeacon77

Is that really the general consensus? On safety or efficacy grounds, if the concern is total GLP-1 exposure from both compounds, consider that 7.2 mg of semaglutide is an approved dose with full clinical safety data. The roughly equivalent tirzepatide dose would be in the 50-plus mg range. Trials are already testing tirzepatide at 20 mg, 25 mg, and possibly 30 mg without apparent safety halts. So combining the two and worrying about GLP-1 load may not be the issue some assume.
