Final Goal Achieved!!! Now what?

Summary

how to find a maintenance dose of tirzepatide after hitting goal weight

Users who reached their target weight on tirzepatide (mostly at or below 5 mg) are now testing ways to hold steady without further loss or regain. The main approaches discussed are lowering the weekly dose while keeping the schedule, or keeping the dose and stretching the interval between injections, but not both at once. Several people report planning to stay on the medication indefinitely for appetite and inflammation benefits and are experimenting slowly over months to find their personal minimum effective dose. Anecdotes vary from no changes needed beyond relaxed eating habits to gradual titration down to 3 mg weekly.

Morning everyone, reached my final target this morning and haven't seen that number since I was 12. Never went over 5 mg of Tirz the whole time. Now I need to sort out the right amount and how often to take it so I neither gain nor lose. Any ideas on finding a maintenance dose? Maybe drop the dose and stretch shots out to every 10 days? Or split it every 5 days? New territory for me, so all suggestions welcome.

I see these meds as temporary support until you can manage on your own. By this point you probably already have good habits and a solid calorie tracker.

Yeah I have those habits, but I'm not planning to stop Tirz. Even with tracking and diet the food noise has been a lifelong issue for me and no amount of willpower fixes it. The anti-inflammatory effects are a bonus too. Tirz quiets my brain and after 40 years of that internal fight I'm not going back. My focus now is just dialing in the right maintenance level.

One-Factor-at-a-Time is the way to test this properly. Change either the dose or the spacing, but not both, and hold it steady for about a month until levels stabilize. Then you'll see if the average exposure or the peaks matter more, or if it makes no difference. It could take three to six months to map it out. Studies show most people regain once they quit entirely.

Nice work. I'm still about five pounds out and plan to drop to 2 or 4 mg of Reta, maybe 6 mg, from my current 9 mg. We'll see how it goes.

I was thinking of lowering the dose while also stretching the time between shots and just watching when the food noise gets too strong again. Not sure if that's the best approach but that's what experimenting is for. Hoping others who've already hit goal can share what actually worked. Main thing is not regaining while I figure it out. I'll stay on Tirz for good; I have enough stocked for years.

Seeing those goal numbers on the scale feels great, congrats to you too. I'm in the trial phase now and really don't want to drop any more, but I'm also worried about creeping back up. Can't afford to gain; I just bought new clothes and got rid of the old ones.

That's exactly why I mentioned the single-variable rule. If you tweak two things at once without the right tools to separate the effects, you're just guessing. You could try it for a month but leftover medication in your system would make it hard to interpret the results clearly.

My approach stays pretty similar, especially that improvising element. Halting at this stage would work out okay, yet I'm targeting those remaining five pounds that look to have leveled off. My spouse stays on the increased amounts. Those older outfits from my job period two years back now suit me well, possibly even a touch loose. All the extra pounds showed up after I left work.

Got it. I'll start by keeping the dose the same and just lengthening the gaps. Thanks for laying it out.

Congrats! I'm currently stalled and even gained a couple pounds, though I'm not sure if it's fat or water since I'm still exercising regularly. Good to see others hitting their numbers. No personal experience yet but I've seen advice to titrate down and hold for a while to gauge how you feel before adjusting further.

LevelThistle44archiveJun 22

First thing is to set your boundaries: you won't stay at 110 lb, so decide at what point you'll step in, maybe a couple-pound window either direction. If you drop below, eat more; if you go over, eat less. Appetite control here mostly comes from dose size. What's your recent trend at the current dose? That tells you how big an adjustment to make. Personally I'd rather keep the same day each week than track shifting intervals. Smaller doses or moving from twice weekly to once weekly might be simpler for me.

Congrats, that's a big payoff for the effort. A lot depends on how you arrived there. At such a low dose there could be more room to adjust. I kept a strict routine until I was four pounds under goal, then relaxed my own rules while leaving dose and timing unchanged. That single change has held for over a year. I also started focusing on body composition and muscle instead of just the scale. At 5 mg you're still at a low level, so loosening up on treats might be enough to balance things without other changes. Either way, well done.

Thanks. So basically if you want something you just eat it now? Not everything, but you get the idea. That might work well for me too. Would be nice to keep the same schedule I've used all along.

BrightKettle28archiveJun 22

This is the part where only personal trial and error will show what works for you, especially if you're not seeing a specialist who could just tell you what to try. Your own ideas are worth testing; pick one and track it, then update us.

AmberTimber37archiveJun 22

My endocrinologist knows I'm using Retatrutide and suggested for maintenance either spreading the same dose over 6 to 10 days or lowering the dose while keeping the same frequency. Both approaches can work; some people just like one better. I'm at goal now and have moved down to 3 mg weekly from my prior 3+2 schedule.

CopperPebble48archiveJun 22

No advice from me yet since I'm still ten pounds out, but I wanted to say congrats anyway. Hopefully I'll be asking the same questions soon.

PatientTimber12archiveJun 22

Congrats @SeaGypsy !

That's great, congrats. Since you never went above 5 mg you could test whether that amount is already your maintenance level. Or drop to 4 mg for a month and check results, then 3 mg for another few weeks, and so on. If food noise returns, bump it back up by a milligram. The idea is to find the lowest effective amount, which will take several weeks of testing, but at least the hard part of reaching goal is done.

SharpHarbour92archiveJun 22

Big congrats. Like others have said, lower the dose gradually and don't change the interval at the same time. Go slow.

ClearWillow59archiveJun 22

Congrats to Sea Gypsy and everyone else who has hit their goals here. Solid work.

SteadyTimber49archiveJun 22

Amazing! I'm about five pounds away and wondering the same thing. Sounds like the answer is just to experiment and find what fits.
