Speed of titration to maximum dose

Summary

How fast can cagrilintide be titrated to the max dose without worsening side effects?

One analysis found no meaningful difference in gastrointestinal issues when reaching 2.4 mg in four weeks versus sixteen weeks. Users describe very different tolerances, with some hit hard by low doses and others experimenting higher when stacking compounds. Flexible, symptom-guided increases are preferred over fixed schedules because side effects can appear suddenly even after smooth steps up.

A study comparison indicated that reaching the top dose of 2.4 mg in four weeks instead of sixteen weeks did not produce a clear increase in stomach-related side effects. Individual reactions still differ, so moving slowly seems safer. At just 0.25 mg the effect on appetite was already noticeable, so further increases may not be needed.

So the highest weekly amount studied is 2.4 mg?

Just because something is possible does not mean it is advisable.

Phase 2 trials tested weekly doses of 0.3 mg up to 4.5 mg, with the highest amount showing greater weight loss. For later trials the chosen maintenance dose settled at 2.4 mg once a week, especially when paired with semaglutide. That means 4.5 mg has been used by itself, while 2.4 mg is the figure when the two are combined.

Nausea and vomiting rates were already high with this compound, and fatigue shows up even at small amounts. Jumping doses quickly therefore looks unattractive. Gradual steps do not guarantee zero side effects, yet they often give early signals like mild nausea that can guide whether to stay put or move up. Rigid trial schedules rarely allow waiting or adjusting based on how someone actually feels.

Even half a milligram felt overwhelming, and a third of a milligram is still rough. Going that high or that quickly would be unbearable.

I have used 1.5 mg together with 4-5 mg of another compound and felt pretty rough.

Two point four milligrams is the upper limit, yet caution is still warranted. Unless the current dose feels ineffective there is no reason to increase. Moving from 0.5 mg to 0.8 mg already felt quite strong.
