Suggestions for adjusting tirzepatide dose due to side effects

Hello everyone, I've been taking Tirze for 11 weeks and reached 7.5mg with my last two shots. I need to inject again today but have run into noticeable nausea and constipation over the past couple of weeks that didn't appear at 5mg. I have some reta available but wanted to hold off until Tirze lost effectiveness. It is still helping me lose weight. My question is whether to keep going at 7.5mg or drop back to 5mg and add 1mg of reta. Any thoughts?

You could test 5.5mg or 6mg or 6.5mg instead. Another option is injecting 5mg every five days or splitting into two smaller shots of 3mg or 3.5mg per week. Plenty of ways to adjust.

I would keep using Tirz as long as it continues working. It is a strong medication and I would stay on it except that higher doses make me feel down. If the side effects feel too strong, lower the amount, perhaps to 6mg.

No requirement exists to increase the dose every four weeks. Stay at the smallest amount that still delivers results. Since Tirz is still effective I would not combine it with reta yet and would only raise the dose later if needed.

Thanks for the reply. The side effects are tolerable but I was concerned about staying at 7.5mg because the average level in my system will keep rising over the next two injections. If those effects worsen with the higher concentration it could become an issue. That is why I am unsure what to do.

Docusate sodium can help with constipation when needed. I would stay at the current level for a few weeks before making changes. In my experience early nausea often came from still eating too much.

What if I lowered the dose to 5mg and then added a small extra amount the following Sunday if appetite or food noise returned after three and a half days?

I wondered whether the nausea came more from the kinds of food I ate in the last two weeks, especially the increase in vegetables and legumes, rather than from the total amount. Still not sure that explains it.

Yes. You can divide the weekly amount into two injections spaced three and a half days apart. That reduces the highs and lows. When I had bad GERD after moving up from 5mg I switched to split dosing and later returned to a single weekly shot. My body needed four to six weeks to settle and I had to experiment a bit with the schedule.

Another vote to stay on Tirz. Just because reta is sitting there does not mean it has to be used right away. Most people who switch or combine do so when their current medication stops working, but here the opposite is true. With these medications easy to obtain it is tempting to add more than needed. 7.5mg is already a big jump from 5mg so lowering it a little could reduce side effects and make the supply last longer.

Okay everyone, I just did the injection and chose to go down to 6mg. I will see how the next few days go and might add a small amount on Sunday if needed. Appreciate all the input.

Update: while I was on 6mg the side effects stayed about the same as at 7.5mg but felt slightly milder, so I moved down to 5mg. Three days after that injection the effects are now much lighter. I will continue at 5mg as long as it works. Last week I lost 1kg.

This is the right approach. When you decide to increase again you might try 0.5mg steps. 5.5mg worked well for me for nearly three months.
