Anyone on Reta experience panic attacks?

Summary

Do retatrutide users get panic attacks or racing heart later in treatment, and what helps?

Several users report sudden panic-like episodes with racing heart and anxiety that begin after the first month, often 1-3 days after an injection when levels peak. These are linked to the drug's effect on heart rate via glucagon and sympathetic activation, sometimes worsened by dehydration or blood sugar shifts. Lowering the dose, switching to tirzepatide, using beta-blockers, electrolytes, breathing techniques, or waiting for tolerance are the main approaches mentioned. Many who switched away from retatrutide saw the symptoms ease.

What this discussion establishesWhere people disagree

Whether true hypoglycemia, pseudohypoglycemia, or pure sympathetic activation is the main driver; also whether the episodes are rare new side effects or under-reported.

First month went smoothly. Then in the most recent couple of weeks I had two sudden episodes a few days after dosing that felt like panic attacks: pounding heart, wave of anxiety, and that intense sense something was wrong. The second one was bad enough I ended up at the VA getting Ativan and fluids. I had only experienced anything similar twice before and both times there were obvious stressors, which weren't present here. Electrolytes and blood sugar were already handled and didn't help. Labs looked normal and even the staff seemed surprised. For people further along: did panic-type feelings ever appear later rather than early, did they hit one to three days after the shot, and did anything besides dropping the dose or just waiting make a difference? Not seeking advice, just comparing notes.

The compound can push heart rate higher and cause palpitations in some users, which then gets read as panic symptoms.

RustHarbour10archiveFeb 17

What dose were you at when the episodes began? I haven't run into that myself on this drug, though I had panic attacks years ago. Things that triggered mine back then were poor sleep, too much caffeine, and certain over-the-counter meds like decongestants. Might not apply but worth mentioning.

Some people see these racing-heart and anxiety episodes show up or get stronger after the first month or so once levels accumulate. The glucagon action raises heart rate and that can spiral into anxiety for certain users. Common things others have tried include dropping the dose, getting a prescription for something to slow the heart, using breathing or grounding methods, cutting stimulants, and timing the shot earlier in the day. A number of users say it settles with time or after switching compounds. Switching to the dual agonist without the glucagon piece often reduces the heart and anxiety issues while keeping weight loss benefits.

This drug activates the sympathetic system more than the other GLP options, so heart and anxiety sides are more common. The one-to-three-day window lines up with peak levels when sides tend to be strongest. Going lower or switching and titrating slowly is probably safer. Getting treatment for the panic feelings makes sense too because they can quickly lead to avoiding situations.

Low blood sugar can produce the same cluster of symptoms. GLP-1 drugs can trigger rapid sugar drops that release adrenaline, leading to fast heartbeat, shaking, sweating, and strong anxiety. Even without actual lows, fast changes in glucose or worry about sides can set it off.

That lines up with what I've seen. Significant fluid loss on this drug can also feed into heart flutters and anxious feelings. I add an electrolyte packet to water every day. Tirzepatide is worth considering if the panic episodes are a concern.

Last night my heart rate suddenly climbed while I was just lying down. I felt a bit anxious, checked my watch and saw resting rate already at one hundred with no activity. Breathing didn't help much and it kept rising past one-fifty up to one-eighty-nine. Left hand went numb, dizziness hit, and I had that sense of doom like a heart attack. Ended up in the ER with rate stuck around one-twenty. All tests came back clear. Still sitting near one hundred the next day. This happened at nine point five milligrams after slow increases.

Yeah it feels awful. Now the mental health staff at the VA keep calling because they think something psychological happened since nothing showed on tests.

You said the first four weeks were fine. What starting dose and how did you step up to five milligrams?

SlowHarbour27archiveFeb 22

Never had full panic attacks but resting heart rate climbed noticeably and I got random palpitations. Two weeks after stopping everything settled. Low blood sugar episodes were rough at times so I carried snacks. Sleep was also broken. Overall it was worth running but I'm glad to be off for now and plan to try again later for a cut.

With these drugs a relative drop in blood sugar is more likely than a true low. For heart support I take taurine and CoQ10 along with electrolytes.

Started at one milligram and increased by one milligram each week.

I already do all of that exactly. The doctors still can't pin it down and now want me to get my ears checked. This is frustrating.

CopperBeacon14archiveFeb 23

Heart rate spikes are rough, especially at an office job where I sometimes have to step away. I went to the ER once when resting rate hit one-forty to one-sixty. Knowing it's the compound and not something else in my head helps a bit, but the spike itself still sets off panic.

Saw someone mention a similar panic attack on another forum recently. Could be the same person posting or it might be a side that is just starting to get noticed.

PlainCompass18archiveFeb 23

I had something similar on the dual agonist, not full attacks but the feeling that one was building. Often I would trigger it myself by worrying it might happen during a meeting. Most times I couldn't calm down and had to step outside. Just started this new compound so hoping for better.

Not tried it myself but have read that some people with anxiety and panic issues get good results from a certain research peptide. Might be worth looking into if the episodes continue.

Dealing with panic attacks, left-arm tingling like a heart attack, and bad shortness of breath. Doctor knew about the drug, checked everything and said it should pass in a few days. Still went to ER where tests were normal. Felt better in the cool hospital but symptoms returned at home. Heart racing again after sleep and arm still tingling. Stopping the drug after this.

SlowHarbour21archiveMar 22

I only get panic attacks when shipments of research compounds get held up.

Half, then three-quarters, then one milligram in the first week. No problems the first two weeks.

BlueKettle13archiveMar 22

Reached three milligrams twice weekly after three months. Suddenly heart rate in the one-fifties, couldn't breathe well, limbs went numb, plus vomiting, sweating, and diarrhea. Ambulance ride and fluids followed. Doctor blamed the GLP plus dehydration plus anxiety. Never had issues like this before and I'm done with the drug.

CopperSparrow86archiveMar 22

Anxiety and sleep both get worse on this. Eating more carbs sometimes helps but stopped working recently. The heart rate increase is real and low blood sugar can also drive anxiety symptoms even when I'm eating carbs. Planning to drop the dose.
