I've been on Tirz since October 2025. At first the food noise disappeared and I felt nauseous at times. I began low and have now been at 5 mg for five weeks with Saturday injections. Today the noise is back and feels stronger than at any point since I started. I realize I'm not following the Eli schedule but I would rather stay on the lower side as long as it works.

A few quick checks: did the vial just get opened or did the source change? Are you hitting roughly a gram of protein per pound of goal weight? How is sleep, water, and electrolytes? Tirz worked well for me at first. When I tried RETA the noise took weeks to settle even after reaching 3 mg, so I added a small daily amount of Cagri. If the basics are covered that might be worth considering.

Staying lower than usual is fine when it works, but it does not appear to be working here. Moving the dose up makes more sense than layering on extra compounds right away.

I agree with that view. After six months only at 5 mg there is no rush or risk of reaching a high amount soon. A modest increase of 1-2 mg might resolve the noise without needing anything new.

The noise came back for me around the middle of the week on 5 mg. I now inject 5 mg on Fridays and add a 500 mcg dose on Tuesdays. That small extra keeps levels steadier and controls the noise. If needed I can raise the Tuesday amount to 1 mg later.

Raising the tirz amount is the direct answer. If significant weight remains, holding at low doses works against progress. A 1-2 mg top-up on a hungry day can test whether more is needed without committing to a full higher weekly dose. Side effects are unlikely when hunger is present. At 58 and having already lost 24 lb the medication is probably helpful overall, so staying under-dosed while still hungry offers little benefit.

That last dose came from the end of the vial so potency might have varied. Protein intake has been low, though a protein drink sometimes reduces hunger when it spikes. Electrolytes were started but not kept up. Water is usually fine on weekdays but slips on weekends. Sleep is still short even with the cpap. The questions helped me see I need to fix those habits before assuming the dose itself is the only issue.

I am open to going higher but want to move slowly. Once the top dose is reached there would be nowhere left to go if weight loss stalls. That may sound short-sighted but I would rather not max out early.

Given how slowly you have increased so far it would likely take another year to reach 15 mg. What is your goal weight and height?

You must be near your goal already, so reaching a maximum is probably not an immediate concern. Cagrilintide could still be an option later since it works through a different pathway. Trying a higher tirz dose first avoids needing a new compound. Adding daily walks of at least 10,000 steps can also offset extra calories.

I have not used Cagri but sema has always controlled appetite well for me and is inexpensive. I recently added 0.75 mg weekly after skipping the lowest starter weeks. After two and a half years on GLP-1s I find the combination effective at that low sema amount.

Are you still dropping weight despite the noise? If loss continues there may be no need to increase yet. Once the noise stops progress I would go up, but only by 0.5 mg each time to keep changes gradual.

I still recommend keeping everything else tight and raising the dose by 1-2 mg per week if faster loss is wanted. That adjustment should carry progress for months.
