Dose up to 4mg or 3?

13 replies11 peopleFeb 27, 2026☆ Follow
Summary

Should I jump from 2 mg to 4 mg of retatrutide after one stalled week or stick with the planned 3 mg step while adding tesamorelin?

Most participants say a single week without scale movement is normal fluctuation and not a reason to accelerate dosing. They recommend staying at 3 mg unless side effects are absent, then moving to 4 mg only if results remain unsatisfactory. Several note that faster titration shortens the useful life of the drug and increases side effects. The original poster ultimately decided to follow the original 3 mg plan.

Whether a one-week stall justifies moving straight to 4 mg or requires waiting another week at 3 mg

Nothing here is advice.

On week eight with this compound, first four weeks at the lowest dose then four at double that. Scale had been moving about a kilo each week until now. I had planned to step to the next level for the coming month but the lack of movement has me wondering about going straight to the higher amount instead. I also intended to begin the other peptide at the middle dose during that step-up.

From what I tried myself, moving from the 2.5 level straight to 4 produced a clearer drop in weight than the smaller steps before it. My own sequence went 1 then 2 then 2.5 then 4 then 5 then 6.

Either choice ends up about the same.

For my own research supply I would try the middle amount for one week, check whether the scale moved and whether it felt tolerable, then go to the next level the week after if the results still felt too slow.

You can raise the dose faster than the usual schedule since you have been cautious so far. I wondered about the other peptide schedule though. The usual approach I have seen is the lower amount five days on and two off for roughly six months so the receptors can reset. The higher amount does not appear to add extra visceral-fat loss in the studies that were done.

Going from 2 mg straight to 4 mg gave me the same weekly loss rate I had seen earlier. I am on the fourth week at that level now and will decide Monday whether to stay there or move higher depending on the numbers. Stomach issues that showed up early have mostly disappeared after eight weeks total. I also started a lower-carb way of eating around the same time and usually have a couple of beers each day. Started near 215 and am now at 195 aiming for 175.

It comes down to how you feel at the current level. Any side effects and you should move up by one step only; none and you could try the bigger jump. Either way you might still run into new effects later.

I agree it probably will not change much either way, but one quiet week is pretty normal when calories are reduced. Two or more weeks would be worth looking at more closely. What also stands out is how little anyone mentions actual food intake or total calories in these threads.

If you are still dropping a kilo a week then the present amount is working as intended. One week of no change is just normal variation, not a stall. Moving up too quickly mainly brings more side effects and shortens the time the medication stays useful. Taking it gradually works better in the long run.

Starting weight, current weight, goal weight and overall targets are also left out of most of these threads.

Good points, I added those details to my earlier post.

Slower steps are usually safer, though you could still try the bigger jump if you want. You might just notice the side effects more strongly. Also worth checking whether your food intake has changed lately, because that can make a difference too.

Exactly right. Only eight weeks in and the first week without a drop makes it easy to react emotionally. Stick with the longer view instead of changing course quickly. Going gradually gives better results over time.

Thanks everyone for the input. The post was written quickly between other things so I left out some details. The other peptide will actually be the lower amount on the five-on two-off pattern. Started at 105 kg, now 98 kg, goal 90 kg. Running an 800-calorie deficit, lifting three times and doing cardio twice a week. Away this weekend so meals will be off plan, which might actually help. I am going to do the middle dose for the next stretch as originally planned.
