Voluntary restriction is hard to sustain and turns it back into a regular diet with poor long term results. The medications are meant to reduce appetite naturally so you eat less without constant effort. If that effect is missing even at decent doses then you may be among the small group that responds poorly.
I wonder if this effect is more common among women. Men tend to be the loudest in denying it is possible. It seems hard for some to accept experiences they have not had themselves.
