Retatrutide Breathing and Chest Tightness Side Effects

18 replies14 peopleSep 29, 2026☆ Follow
Summary

Did retatrutide cause breathing trouble and chest tightness after a first 1 mg dose, and is restarting at 0.5 mg safe?

The original poster reported racing heart rate for the first hour and chest tightness plus breathing difficulty that eased over five to six days after a 1 mg dose. Most replies attribute the heart-rate jump to nerves or the drug's known effect while disagreeing on whether breathing symptoms were anxiety, reflux, an allergic reaction, or something cardiac. Suggestions range from micro-dosing to dropping the drug entirely or getting medical checks; several users report no breathing issues at similar or higher doses.

New to this stuff, I tried 1 mg from a 10 mg vial using 10 units eleven days ago. The racing heartbeat in the first hour might have been nerves from finally going through with it, but the bigger problem was chest tightness that made breathing hard. Tightness eased after three or four days and breathing took another two days to settle. I still want to see results so I'm thinking about starting again at 0.5 mg for a few weeks. Does that sound okay, and has anyone else had similar sides?

A few things first: 10 units only tells volume, not mass, unless the vial was reconstituted with exactly 1 ml of BAC. The heart-rate jump right away sounds like nerves, since retatrutide's resting-heart-rate effect doesn't hit that fast. The four-day chest tightness is more worrying if other symptoms were present. Was there injection-site redness, a rash, or tongue swelling? First step is checking whether the vial was tested and what the COA showed, plus how much BAC was added total.

This sounds like anxiety to me. That said, the resting-heart-rate rise is well documented, and some people also see big jumps during exercise. Nothing says you have to use retatrutide.

I agree the heart-rate part was probably nerves. I've never heard of breathing issues from it though. Everything else in the previous reply makes sense too. Curious where the vial came from. Did you buy one or a kit of ten, and what kind of BAC water? Would like to see the COA if possible. Mixing the 10 mg vial with 2 ml BAC would make a 1 mg dose equal 20 units.

Heart rate does come back down after a while. Mine returned to a normal 72 BPM. Phase 2 data showed a dose-related resting-heart-rate increase that peaked around week 24 at higher doses and then declined toward baseline later. The glucagon action is the likely reason, and the rise is bigger than what's seen with tirzepatide or semaglutide.

Sounds like an allergic reaction to me. I'd keep epinephrine handy if trying again. Start at 0.25 mg or less, and if the same thing happens I'd drop retatrutide and maybe try tirzepatide instead.

Good advice already given so I won't repeat it. I did my first retatrutide shot on Saturday at what should have been a similar dose if you used 1 ml BAC, and I had none of those effects. Only side effect was a headache like the one I used to get when starting keto.

You'll probably get some sides but they usually fade. I was lucky and only had the elevated heart rate even up to 10 mg; my wife is on 12 mg with nothing. Mine has since dropped back to normal.

I expect sides as I go up but hope going low and slow will keep them minimal. I want to stay as low as possible while still making progress and I'm fine taking longer to reach my goal weight. Not afraid to increase if needed though.

It strikes me as anxiety too. Still, don't rely on forum posts to diagnose or decide what to do. Medication always carries risk, especially gray-market stuff without a doctor. Only you can judge your own risk tolerance and then decide next steps.

The heart-rate rise is pretty normal and usually improves after a while. As for the breathing, did it start the day after the shot? It could be acid reflux at night, which I get pretty badly sometimes.

How much BAC water went into the 10 mg vial? I once had mild breathing trouble for a few days where I got winded doing normal gym stuff, but it went away on its own.

I started at 1 mg a few weeks ago and only had the usual few-BPM heart-rate increase. No breathing problems, no palpitations, no chest tightness. Sounds a bit like anxiety. Are you afraid?

With retatrutide the only place to get information on what to do is the internet.

What volume of bac water got mixed into that container?

Most likely the heart-rate increase from the drug triggered a feedback loop that created anxiety-type symptoms. I've seen several people here describe the same thing, even a couple of panic attacks. An allergic response is possible but fits less well with the timing of the heart-rate change. Did you actually measure the heart rate? Age and general health matter too. The combination of fast heart rate, chest tightness, and breathlessness could point to something more serious like an arrhythmia or even a mild cardiac event, so getting an ECG might be worth considering if symptoms return.

Watch loose skin if you're losing fast, especially at older ages. I'm now on a maintenance dose after dropping 60 lb since January 9 in roughly 30-32 weeks at just under 2 lb a week. I have some loose skin on arms, chest, and belly. Started light weights and sit-ups last week.

Loose skin is a concern of mine too, which is why I'm going low and slow. Down 60 lb sounds great though, and getting back in the gym must feel good. Hate sit-ups as well. Keep it up.

Try micro-dosing back up and see what happens. Start at 0.25 mg for a few weeks or even twice a week, then move to 0.5 mg. Nothing says you have to jump straight to 1 mg. I began with 0.5 mg for a few weeks, then 0.5 mg twice a week, then 1 mg, then 2 mg, and stayed there before moving to 4 mg.
