Switching from Tirz to Reta led to sleep trouble for some users

15 replies11 peopleJun 17, 2026☆ Follow
Summary

Why does retatrutide disrupt sleep for some people while tirzepatide does not, and does combining them help?

Users report that retatrutide often causes insomnia with early morning waking and feeling wired, unlike tirzepatide which allowed good sleep even at high doses. Several note that sleep problems improve after the first couple of months on reta or with small bedtime carbs, while others see no improvement and return to tirz. Combining the two on staggered days is described as complementary for appetite control and fat loss by some, though heart rate elevation is mentioned as a possible factor in sleep issues.

What this discussion establishesWhere people disagree

Whether sleep issues stem mainly from sympathetic activation, elevated heart rate, or unrecognized nighttime hunger; also whether combining the two peptides provides meaningful added benefit over either alone.

Tirzepatide worked okay for me even at the top weekly dose and I dropped sixty pounds over roughly a year. After switching to retatrutide I could not sleep well. I started with 1 mg after ten days off tirz, added another 1 mg three days later, reached a total of 5 mg spread across two and a half weeks, then took 4 mg. Appetite stayed down and nausea was a bit stronger than with tirz, but I kept waking at two or three in the morning feeling wired even though I only drink two cups of coffee early in the day. I stopped reta and returned to 8 mg of tirz two weeks later; sleep has been much better since. Only a few pounds left to lose. Results differ from person to person.

That is unfortunate. Retatrutide works much better for me and I still add a little tirzepatide. It makes me noticeably more vascular, especially when lifting.

I use both at 10 mg each per week and after two months on retatrutide my sleep has actually gotten better.

I take both as well. Sleep was poor for the first couple of months on retatrutide but then improved even as the dose went up.

Retatrutide increases resting heart rate according to my doctor, and tirzepatide does the same but to a smaller degree. Maybe that is what is interfering with sleep.

A video suggested eating some carbs before bed to fix retatrutide-related sleep trouble. I now have a small snack with pumpkin seeds and peanut butter on bread plus a little magnesium when I wake up and it seems to help.

After running out of compounded semaglutide I started gray-market retatrutide. Hair that thinned during the prior two years on semaglutide and tirzepatide is now growing back. On the sleep side I normally go to bed around 2 AM and could sleep until noon, but now something wakes me between 6 and 8 AM and I cannot get back to sleep. My watch shows good quality sleep but only four to five hours total, and I do not feel tired even at bedtime.

The higher heart rate should not be what is disturbing sleep.

The glucagon action of retatrutide probably raises overall sympathetic nervous system activity, which would explain both the faster resting heart rate and the insomnia reports. The heart-rate change itself is not the direct cause; the extra adrenaline and noradrenaline activity is.

Retatrutide has wrecked my sleep and it took me too long to connect the dots. I also felt quite low and had skin issues. I skipped a dose for nearly twenty days, then tried a smaller amount two days ago while continuing daily GHK-cu and KPV. I like the other effects enough to want to keep using it if possible.

It may simply be hunger. The body wakes looking for carbs, yet we do not register it as hunger because the medication suppresses appetite and we do not want to eat at 2 AM, so we blame other causes.

Do you take both on the same day? Is there an advantage to using both? Tirzepatide is working for me but I have retatrutide on hand and am unsure whether to add it.

Not on the same day. There does seem to be a benefit. Official labs are exploring combinations and user reports plus a research paper on the differing GLP-1 receptor affinities convinced me to try it. The two appear complementary; my blood sugar control is tighter now and I have lost a noticeable amount of visceral fat.

Thanks for the details; that helps with my own decisions.

One more question: how many days apart do you space the two injections each week?

My resting heart rate stayed around 60-62 bpm with no real change on retatrutide, yet I still felt wired. Ten days after the last dose sleep is back to normal. I still have some left and might try again later, but I am enjoying good sleep too much right now and am close to maintenance anyway.
