I began at 1 mg on October 11th to check my reaction to side effects. None appeared so last Saturday I moved to 2 mg, still under the usual starting amount. This week hunger seems to be coming back. After a tuna sandwich I still wanted more food, though I had not eaten much earlier and my total calories were only around 1300. Should I raise the dose next week or add another half milligram sooner?

Not questioning your choices but why skip straight past the normal 2.5 mg starting dose? Even that amount is not yet considered therapeutic. I would move to 2.5 mg and then go up to 5 mg after four weeks.

Thanks. Fear of side effects pushed me to begin lower than recommended. The past couple of days have been the hardest for hunger so far. I will take 2.5 mg this Saturday or possibly a bit earlier if the hunger stays strong.

The schedule from the maker calls for 2.5 mg each week for four weeks then an increase of 2.5 mg every four weeks after that. You have stayed below that plan the whole time. In the first month even at 2.5 mg it is normal to feel hunger return before the next shot because the weekly schedule and five-day half-life have not yet produced a steady level. That steady state takes about four weeks to develop. You are still on only your third dose and the amounts are small so levels are swinging a lot. Peak concentration after an injection can occur anywhere from eight to seventy-two hours later depending on the person, so adding a small amount right when hunger appears does not work that way. It is better not to try to improve on the established schedule with your own adjustments.

Some users notice no side effects until they reach higher amounts. With tirzepatide it is still best to follow the usual doses. If side effects show up at 5 mg you can drop back to 2.5 mg for a few more weeks and then try 5 mg again. If 5 mg feels fine then move to 7.5 mg after four weeks and continue that pattern. Some prefer to remain at a dose only until weight loss stops, but I am sticking with the maker's guidelines.

I always feel bad for anyone still dealing with hunger. A day or two after my first 1.5 mg dose I could already tell it was working. I have since switched to another medication and never questioned whether either one was effective. Are you getting the grey-market version? If so, is it from a source that provides COAs or third-party testing? When people report no response I usually wonder about the source first.

Agree, go up because you are still too low. You hear plenty about side effects, yet everyone who has none or only minor ones tends not to post about it.

Agree you should be at 2.5 mg at minimum, but after that do not increase unless hunger continues after four full weeks. The maker's main goal is profit, yet there is no need to go higher if a lower dose is already working. In fact you are more likely to build tolerance if you use more than necessary. Just give each dose four weeks because it takes at least that long for the medication to accumulate and reach full effect.

This chart is for glucose control. The maker's schedule for the weight-loss version looks similar, moving up to 5 mg then checking whether a higher dose is needed. The company set those levels for testing and approval. That does not mean you must follow them exactly with the grey-market version.
