Summary

User experiences switching or stacking semaglutide, tirzepatide, and retatrutide for weight loss, appetite suppression, and side effects after stalling on sema

People describe tirzepatide as delivering strong appetite and food-noise control with steady loss and minimal sides for many, while retatrutide is noted for continued fat loss even when hunger returns and possibly better muscle sparing, though some find its suppression weaker. Semaglutide works initially for some but often stalls or brings more nausea. Stacking low doses of two agents is common when one alone is insufficient, and responses differ enough that users frequently try all three before settling.

What this discussion establishesWhere people disagree

Whether retatrutide spares muscle better than tirzepatide and whether its overall profile is superior once past the initial weeks

Long-term safety of retatrutide while still in trials and the best way to handle maintenance after reaching goal weight

Looking for real experiences here. Tried sema and dropped 46 pounds but now stalled. Trying to decide whether to add cagri, switch to tirz, or try reta. Heard reta might be strongest overall but tirz wins on appetite control, and that matters because I snack out of boredom even when full. How exactly does reta help more? Any suggestions on the next step?

NorthLantern99archiveFeb 5

Tirz took off 22 kg in twelve weeks with zero sides and wiped out food noise completely. Skipping reta for now because of the heart-rate increase shown in studies and because it is still in phase-three trials, so it does not feel safe enough yet.

SharpCinder28archiveFeb 5

You can stack them all, lol.

RustWillow66archiveFeb 5

Sema did not work nearly as well for me as tirz has.

SharpLantern71archiveFeb 5

Lots of different views out there. Plenty of people run low-dose cagri for the appetite piece and reta for the fat-burning side. Cagri did not sit well with me so I tried low-dose tirz plus reta instead. Even in maintenance I still want the suppression and I am looking forward to testing eloralintide at a low dose while staying on reta.

QuietAlder20archiveFeb 5

Reta is better at sparing muscle and fighting insulin resistance. Tirz is better at killing food noise. Sema is best at causing nausea.

WarmHarbour28archiveFeb 5

I cannot see how reta would do anything for me. I am already at 15 mg of tirz and it is not doing much anymore. Used to get punished for overeating but that effect faded. No evidence any of these are magic; they are supposed to train you but I guess I am too stubborn for that.

Do you mind sharing what maintenance doses you settled on and how you chose the amount and frequency?

LevelTimber64archiveFeb 5

Seen reports that sema scores highest on appetite suppression. RS did fine on it until it quit working, then added cagri because it works through amylin instead of GLP-1 and that made a noticeable difference. Switched RS to tirz at the start of the year and things are going well on that. Took me a while to figure out the best path forward.

GreyMarble12archiveFeb 6

Started at 336 late 2022, got down to 318 on my own, then lost 106 pounds on sema from mid-2023 to early 2025 before stalling around 211. Tried stacking 3 mg sema with cagri but it caused terrible constipation so I dropped the cagri. Lost over a third of my starting weight on sema alone, which is surgery-level results. Switched to tirz then reta at max doses separately but neither produced more loss. Went back to 1 mg sema for maintenance and slowly gained back to 224 by end of 2025. Now trying a sema-reta stack, currently at 218 aiming for 190. Running 1 mg sema on Mondays and 4 mg reta on Thursdays after four weeks at 2 mg. Need the hunger control that sema gives me because tirz and reta did not provide it on their own. Might add tesofensine later if this does not move things.

No, that is not accurate. One study showed retatrutide cuts fat mass in type-2 diabetes without big lean-mass loss. Still, other data indicate tirz produces a lower percentage of muscle loss than either reta or sema.

SharpLantern71archiveFeb 6
↳ replying to @GreyThistle26

Still figuring it out. Maxed at 12 mg reta and 5 mg tirz. Dropped tirz for a while and hunger came back. Tried cagri but got constant headaches. Went back to 4 mg tirz and now sit at 8 mg reta, stable for several months. Could probably lower reta but the weight is steady and suppression is good. Dosing once a week but not on the same day each time.

Cannot speak to reta or sema, but after only six weeks on tirz the change is huge. Fought weight my whole life and this is the first time in years I do not feel constant hunger and food noise. Results so far are unbelievable and I am already seeing clear physical changes.

QuietTimber26archiveFeb 6

Running reta at 4 mg every 84 hours. The final twelve hours or so hunger creeps back in. Suppression is definitely weaker than on tirz, but high-dose tirz was rough for me and left me wiped out for days. Hit a long plateau on tirz so switched to 8 mg reta over eight weeks. Scale has not moved much but I have lost noticeable fat around the middle, hips, and neck. Clothes fitting better matters more than the number anyway.

KeenFenwick47archiveFeb 6

Same here. Adding just 0.1 mg sema to 5 mg tirz gave perfect appetite control.

GreyBeacon92archiveFeb 6
↳ replying to @QuietAlder20

No data yet on whether reta spares muscle well or not.

QuietTimber26archiveFeb 6
↳ replying to @KeenFenwick47

Depending how the next month on reta goes I might add a little tirz at 2.5 mg or even 0.1-0.2 mg sema. Far fewer GI sides on reta, which has been a relief. On 12.5 mg tirz I needed daily famotidine and gas-x just to function.

BluePebble18archiveFeb 6

Was sick the entire time on sema. Tirz made me sick at first but that passed quickly.

SlowMeadow76archiveFeb 6
↳ replying to @QuietTimber26

That matches where I started too. Did not even own a scale when I began tirz because it was too depressing. Six weeks later I am wearing jeans I had packed away two years ago and a coat I could not zip before. Bought a scale for the first time in a decade because the progress feels real.

KeenPebble77archiveFeb 9

Been trying to lose for the past year. Weighed 429.5 in February 2025 at a doctor visit. Dieted and made small activity changes for twenty weeks and lost about twenty pounds. Not satisfied so looked into GLP-1 options and started reta in September at 2 mg, now up to 8 mg. Down to 357.7 yesterday, roughly fifty pounds in five months with little added exercise and loose calorie tracking. Focused on protein since appetite is low much of the time.

QuietCompass40archiveFeb 9

Used compounded sema before with some success but sides were bad enough to stop. Started reta recently and saw appetite drop right away at 0.5 mg, though it faded by day five or six. Titrated to 2 mg and suppression now lasts the full week, though some days it is hard to hit protein goals. Used to be a constant boredom and evening snacker; now the idea of snacking does not appeal. Stopped keeping those foods around to avoid temptation. Eating is now about hitting protein and fat targets rather than grazing. Might have seen similar results staying on sema but the sides were worse there.

LevelAlder78archiveFeb 9

Thought about food constantly and could not stop once I started something. On reta I have zero interest in food and struggle to even make dinner. Nothing sounds good. If something tasty is in front of me I might eat it but I do not keep those things around. Focused on protein. Also completely turned off alcohol, which I miss a little.

Never got the doctor lecture myself, just years of an RN family member pushing me. Tried on my own with zero progress. Saw a friend lose a lot on Ozempic but could not afford it without insurance. After the South Park special I started looking online and eventually found this path. Weight is way down and I am enjoying life again for the first time in years.

QuietAlder20archiveFeb 11

As someone three-plus years sober, tirz wiped out the leftover booze cravings that naltrexone never fully handled. Also helped drop a hundred pounds. Win all around.

WrySparrow63archiveFeb 11

Super slow responder in general. Took eighteen months of diet and training to go from 190-195 down to 150 and have been stuck there a year. Switched from sema after it stalled to tirz and finished the loss. Tried reta but got diarrhea once I hit 4 mg even splitting doses. Now on 8 mg tirz weekly for maintenance because my body likes being around 150. More interested in building muscle than the scale number anyway. Stay on tirz because I was borderline prediabetic.

IronThistle42archiveFeb 11

Same here. Congrats on three years. Almost at two myself. One day at a time.

For those who found tirz killed alcohol appetite, what dose worked? Has not happened for me as a wine drinker beyond the first month. Currently at 5 mg every six days.

WarmHarbour28archiveFeb 11
↳ replying to @CopperTimber28

From 10 mg upward, now at 15 mg. Oddly wine is the only drink that does not make me nauseous. Used to drink a lot of gin but never had strong cravings anyway. Knowing how sick it will make me is enough to avoid it.

CopperTimber28archiveFeb 11
↳ replying to @WarmHarbour28

Thanks. I do not do much hard alcohol, maybe a vodka martini every couple months. Comes down to awareness and moderation for me.

SlowBramble78archiveFeb 11
↳ replying to @KeenPebble77

The small daily choices like taking stairs or parking farther away are worth keeping up even while on the meds. Also cut out sugary drinks. Keep going, you will get there.

IronThistle42archiveFeb 11
↳ replying to @CopperTimber28

In my experience it is really about the overall craving, not just alcohol. Tirz curbs the urge for excess of almost anything I might put in my mouth. A martini once a month or even weekly is not the same as constant craving, so tirz probably will not affect that occasional drink. Your mileage may vary.

CopperTimber28archiveFeb 11
↳ replying to @IronThistle42

Appreciate the input. I do not have a problem with the occasional martini. The issue is being a nightly wine drinker and wanting to stop craving it every evening.

Would help to know how much overweight you still are, whether two hundred pounds or just another twenty. Age, how long you have carried the weight, and past attempts matter too. Medical checkup makes sense especially if older or with other conditions. Dose and sides from sema are relevant. If sema is still working and you are below 2.4 mg with no sides, just increase it. Otherwise switching to tirz or reta is reasonable; tirz usually has fewer sides while reta may be stronger if you have a lot left to lose. Cagri is best saved for when max doses of the others have stopped working, added low and slow because it tends to have more sides.

LevelTimber64archiveFeb 12

Moved from sema to tirz starting at the lowest dose. After six weeks now at 5 mg tirz with about a pound a week loss, which is fine since I am fifteen pounds from goal and twenty from stretch goal. Stacked 0.2 mg cagri with sema earlier but saw no extra movement. Always had gut issues so sides have not stopped me. From obese to healthy weight has been a long process and a big education. Where you start and where you are in the journey affects how fast you lose. Not everyone responds the same to each option. Medicine is not exact. Do your research, ask questions, and find what works for you.

Fifty pounds in five months on reta? That is impressive. Any sides? Thanks for sharing the story.

WarmMeadow92archiveFeb 17

That stack works great for me too, though my doses are higher than yours.

NorthThistle87archiveFeb 17
↳ replying to @LevelAlder78

Has cut my drinking quite a bit but I can still have a nighttime whiskey. Wish it worked better on the booze since none would be ideal. Maybe once I go above 3.5 mg it will kick in more.

WryBramble37archiveFeb 17

Will not underestimate retatrutide. Hit goal on tirz then switched and appetite rose but weight kept dropping on the maintenance dose. It burns the fat regardless of hunger for me.

KeenPebble77archiveFeb 18
↳ replying to @RustWillow39

Nothing beyond the usual slower digestion. Weight has been flat the last ten days but hunger and fullness signals have not changed so I am not worried. Wife started reta too after seeing my lack of sides; she is down from 215 to about 180 and never went above 6 mg. Discussing lowering her dose so she can focus on recomp if she wants, though she does not want to lose much more at her height.

LevelTimber64archiveFeb 18
↳ replying to @WryBramble37

That is great. What part of reta drives the fat burning, or are you just fitter now so muscle raises resting metabolism?

WryBramble37archiveFeb 18

Eating a bit better but I think the GIP and glucagon activity in retatrutide is doing the work. Exercise habits have not changed much. Only wanted to go from 202 down to 170.

IronThistle42archiveFeb 18
↳ replying to @WryBramble37

When you switched to reta what dose did you start at?

AmberWillow76archiveFeb 18
↳ replying to @RustWillow39 (opening post)

Moved from tirz to reta. Tirz has stronger appetite suppression but it came roaring back once I lowered the dose to start reta. Wanted reta for the more balanced profile and reported extra benefits like anti-inflammatory and heart-health effects. If it were me I would switch to reta and titrate slowly. Note that reta can take longer to build up if you have been on another GLP-1, sometimes four weeks to steady state. Everyone is different though.

WryBramble37archiveFeb 18
↳ replying to @IronThistle42

Stayed on 2.5 mg tirz weekly the whole three months while dropping from 202 to 170, reaching 13 percent body fat. Switched cold to 2 mg reta weekly, gained five pounds in two weeks, then raised to 4 mg after four weeks. Appetite did not change and loss resumed. Eating more after the switch but reta still works. Something at 4 mg told my body to burn more and energy is higher. Might drop back to 3 or 2 mg. Already on testosterone, CJC/Ipa, and GLOW the whole time and still using them. One full meal a day with protein, veggies, and some carbs plus a couple snacks. Play tennis and run/lift several times a week so very active. Reason for starting was being 60 and 200 pounds affecting joints and daily life.

IronThistle42archiveFeb 19
↳ replying to @WryBramble37

Thanks for the details, exactly what I needed. Similar situation here. Stacking survo on tirz for months, at goal for three months, wondering if one weekly reta shot could replace the two injections and whether energy might differ. Also use BPC, TB, KPV, and CJC/Ipa separately plus strength and cardio. Went from 240 to 185 since last June at age 62. Takes dedication but worth it. Congrats.

RustMarble78archiveFeb 19
↳ replying to @RustWillow39 (opening post)

Found reta really good at stopping food noise without completely shutting down the ability to eat, which may not be ideal anyway. Started at 2 mg weekly, moved to 4 mg, and now chatter is gone but I can still eat when needed. Cannot stuff myself or want junk food. Can eat clean food and hit macros to protect muscle. If you reach therapeutic dose, exercise, and eat clean it feels like the best option. After trying sema and tirz I am convinced reta is the healthiest choice, at least for me.
