Hunger on injection day for Tirzepatide

18 replies14 peopleJun 23, 2026☆ Follow
Summary

Why do some people get hungrier right after their weekly Tirzepatide injection and what helps?

Several users report increased hunger specifically on injection day after the shot, even at steady high doses like 15 mg. Common suggestions include splitting the weekly dose every 3.5 days, shifting the injection to bedtime, or moving to a 5- or 6-day schedule with a lower amount per dose. Some find the pattern normal due to the drug's pharmacokinetics and simply eat more protein during those windows, while others add a small mid-week dose of another compound. Nighttime dosing helped the original poster substantially.

Over the past few weeks I've been getting hungrier than usual on the day I inject Tirz. This never happened before, and it's right after the shot. Has anyone else gone through this?

RustBeacon82archiveJun 23

How much are you taking and how long have you been at that amount? Going up should reduce that feeling.

The half-life is around five days so some users notice this pattern. One fix is to split the weekly amount and inject every three and a half days. Another option is to change the interval to every five or six days while lowering the dose each time. You can use an online plotter to see how the levels would look.

I don't think I need to go higher because the next day I'm fine and even the morning of the injection day feels normal. The hunger only shows up by afternoon after the shot. I'm already at 15 mg anyway.

If it's really bothering you then the split-dose idea from earlier makes sense. For me the hunger usually returns the day before my shot and I just go with it and try to get extra protein in during that window.

A lot of people I know switched to a five-day schedule and lowered the actual amount per shot. That removed the issue for them, especially now that most are just maintaining. A few also added a small amount of sema once a week.

For me it's pretty typical. The first day after pinning I don't notice much, maybe some burps overnight, but food noise drops the next day and even more on day three before leveling out. By day five or six it starts coming back. I've been splitting doses for the last few weeks and it feels better. If I change anything it would probably be a five- or six-day schedule. Good luck with your progress.

Blood concentrations reach their highest point about a day after each injection, meaning the lowest amounts appear right before the following one and then start climbing once more. Right after giving the shot the amounts sit low yet begin increasing. If fading effects caused the issue symptoms would intensify ahead of the next shot instead of afterward, whereas a side reaction would build to its worst a full day later. My suspicion though it is only a suspicion points to mild queasiness while concentrations climb since separating that feeling from appetite often proves difficult. For a full year on a smaller amount of the medication intended to curb nonstop appetite after dropping a large amount of weight without these agents I dealt with persistent queasiness mixed with hunger. At times I felt both sensations together or could not separate them and occasionally a small bite of sweet candy or preserved ginger eased the discomfort. Trying divided administrations to check whether that changes anything does not require much effort.

Splitting the dose worked really well for me. No more strong side effects and steady hunger control.

I saw someone with the same problem who switched their shot to nighttime right before bed. They slept through the window and woke up fine without the symptoms.

That actually lines up. I might give it a try since the hunger usually starts a couple hours after the injection.

I've always done mine in the morning because some people said it can affect sleep, though that might not fit with the timing of peak levels. I usually pin on weekends after fasted cardio, which happens to keep me less hungry for a bit afterward.

I had the same thing and moving the shot to night worked for me too. Hope it helps you as well.

That was the solution. It made a big difference. Thanks for the idea.

I've seen several people mention this. With a five-day half-life the drug doesn't suddenly drop off before the next dose. After a few weeks most reach fairly stable levels with only modest peaks and troughs. So the injection-day hunger probably isn't explained by pharmacokinetics alone. I don't know the exact reason but I don't see it as a problem. The goal isn't to feel zero hunger. A little appetite is normal and can make it easier to hit protein needs. I'd just enjoy the hunger and focus on getting good protein that day.

Adding a small mid-week dose of Reta fixed the end-of-week hunger for me. I'm doing 7.5 mg Tirz on Friday and 3 mg Reta on Tuesday. As someone experimenting I'd like to try lowering the Tirz amount eventually.

Yes, same here. I always inject on Mondays and the next five days hunger stays away, but it returns Sunday and stays through Monday. I have to push back against it on Mondays and then it goes away again.

I also inject Monday morning but I'm not hungry on Mondays. I do notice some hunger by Saturday or Sunday, which isn't necessarily a bad thing.
