Has anyone seen bilirubin rise on tirzepatide and is it tied to fast weight loss?

Most blood markers improved after two months on tirzepatide with a recent dose increase to 5 mg. Bilirubin rose while ALT and AST stayed normal, pointing to weight loss as the likely cause. Lipids may climb temporarily during active fat loss but should settle later. Follow-up testing in a few weeks is advised if the elevation continues.

Hello everyone, I got new blood tests yesterday after two months on tirze, with six weeks at 2.5 mg and two weeks at 5 mg. Most numbers got better, yet bilirubin rose further. ChatGPT pointed to fast weight loss as a possible reason. Has anyone else noticed this same rise?

Lab scientist here who runs these tests daily. The recent dose jump might have put some stress on the liver, so watching ALT and AST would help confirm. The body may settle in, and another check in two to four weeks would be useful. If levels keep climbing or symptoms worsen, going back to the lower dose could make sense. The platelet change looks like normal fluctuation and everything else came back solid.

The extra ALT and AST numbers just came in and they sit inside the normal range. That makes the weight-loss explanation for the bilirubin seem reasonable. Any thoughts on that?

What did your lipid panel show? My basic chemistry panel looked excellent, yet the lipids doubled and I cannot explain why.

That lines up with the weight-loss idea. I would still recheck the bilirubin in a couple of weeks to watch whether it keeps rising.

Those numbers should come back down once the weight loss stops. Triglycerides and LDL often rise while fat is being broken down.
