Resuming after a break from tirz: where to restart dosing

10 replies7 peopleJun 3, 2026☆ Follow
Summary

How to resume tirzepatide after running out and missing two weeks at a prior 10 mg dose?

The original poster lists doses dropping from 10 mg to zero over three weeks and wants to avoid both side effects and a full restart at 2.5 mg. Most replies say GI tolerance is already established, so 5 mg is a safe re-entry point today, followed by 7.5 mg or 10 mg the next week if it is tolerated. A couple of users suggest splitting the first 5 mg or titrating even more slowly by symptoms. One reply notes the spelling is Tirz, and another asks why no buffer stock was kept.

Whether to begin with a single 5 mg shot or split it into two 2.5 mg doses spaced 3–4 days apart

Nothing here is advice.

I ran out and could not buy more until recently. I am unsure what dose to take now. I want to avoid feeling ill yet do not want to begin again at the starting amount if that is unnecessary. Here are the dates and amounts I used: 10 mg on the 10th, 3 mg on the 17th, then nothing on the 24th or 31st. The new supply arrives today.

Not an expert, but in the same spot I would probably use the lowest amount for the first week to let it settle back in, then move to the next level a week or two after that instead of stretching the increase over a full month. If side effects stay manageable I would keep stepping up faster until I reached the amount I had been using before.

Missing two weeks after having reached 10 mg does not require returning to the lowest starting amount meant for people who have never used it. Tolerance to stomach effects is already built. Begin with 5 mg now and check how it feels. If it goes well move to 7.5 mg on the next scheduled day and back to 10 mg the week after. If the first shot feels strong, stay at 5 mg that next time and delay the increase. Keep meals small, drink plenty of water, and rest after the injection.

Current levels are below 1 mg so the choice depends on how you feel. Given the earlier 10 mg use you have experience with it. I would begin with 2 to 4 mg and add more every couple of days while watching for side effects until appetite control returns.

That matches what I had in mind, or possibly splitting a 5 mg total by taking half now and the other half after three and a half days if the first portion goes okay.

I will likely split a 5 mg amount, half now and the second half three and a half days later if I feel ready.

I realize this is nitpicking, but the substance is spelled Tirz.

No offense intended, but why not keep extra on hand so this situation does not happen?

The original post already explained the money was not available yet. Not everyone can set aside enough to stockpile for months or years. Plenty of people live paycheck to paycheck.

I overlooked that detail, thanks. I know others in similar situations and my own household went through tight times years ago. Sorry for missing it.

I agree, keep it simple. Five milligrams is fine because the body already knows the medication and will not overreact after only two missed doses. The same thing happened to me after a month off; I went straight to 7.5 mg, then 10 mg the next week and back to my earlier level the week after with no problems or side effects.
