Summary

How well does a tirzepatide plus cagrilintide combination work for substantial weight loss in a middle-aged man, and what issues come up afterward?

One user lost 71 pounds in nine months on weekly tirzepatide and cagrilintide following standard titration with no major side effects, then began tapering tirzepatide while planning a switch to retatrutide for maintenance. Others congratulated the result but raised concerns about muscle loss without exercise or adequate protein, and the original poster noted added glycine and feeling more confident. Discussion touched on mental adjustment to the new body, glycine's possible benefits, and reasons for stacking the two peptides.

What this discussion establishesWhere people disagree

Whether glycine meaningfully reduces muscle loss, inflammation, or improves sleep and blood sugar; some users report clear benefits while others see none.

Tirzepatide every Sunday and cagrilintide every Thursday. Started at 215 pounds in December 2024 and reached 144 pounds by September 2025. Stuck to the usual dose increases every four weeks up to the top recommended amount and had no real side effects. Checked weight at six months (165) and again at nine months. Planning to begin working out four times a week soon. Now slowly lowering the tirzepatide dose to switch over to retatrutide for keeping the weight off, with the idea of eventually stopping the retatrutide or cagrilintide.

Wow, congratulations! I have been on my own journey for two years and hope to reach my goal soon.

Congratulations on hitting your goal. I am close myself, just ten pounds left to go.

Losing that much without exercise can mean a big drop in lean muscle. I reached what I thought was my goal and got a DEXA scan that showed 33 percent body fat plus high visceral fat. My muscle mass ranked in the bottom few percent for my age even though I did not look overweight. The advice about exercising and eating enough protein turned out to be correct. Now I am working on fixing my body composition, which may be tough at my age.

Always good to see someone reach their goal while losing at a steady, healthy pace. Congratulations!

I feel more confident when I am out. Overall I am happy because I have not weighed this little since my late teens and early twenties.

I knew about the muscle-loss risk before I started, so I added a daily glycine supplement as well.

HGH plus exercise can bring the muscle back quickly. The same thing happened to me when I lost a lot of muscle without training. After using 3 IU five days a week for five or six weeks my muscles came back stronger than before. I am in my late fifties too but now in good shape. The scale went back up about fifteen pounds, yet I believe that is all muscle because my strength is the same or better than before the GLP-1.

What is glycine supposed to do for muscle loss? I have heard carnitine might help but have not tried it myself.

I followed the chart that was attached. 2.4 mg was the highest dose and that is still my weekly amount.

I searched for glycine supplement benefits and the overview listed enhanced muscle growth and recovery along with better sleep, less oxidative stress, better blood sugar control, reduced inflammation, and improved liver health. Other possible effects include support for healthy aging, cognitive function, wound healing, and lower risk of some cancers.

I have taken fairly high amounts of glycine for the past year or so but have not noticed any of those benefits. No change in inflammation despite having an autoimmune condition, and it did not help my sleep or blood sugar either. The other effects feel subjective. Still, I consider it a small net positive for health overall, though it seems a bit overrated.

I am not trying to lose a lot, just a little weight and relief from constant hunger. When I get near 5 mg per week, whether in one shot or split, I run into a bad side effect. What made you combine tirzepatide with cagrilintide?

SharpTimber13archiveNov 7, 2025

They act on four different receptors between them, so it seemed logical that weight would come off faster. The bigger reason was reading that tirzepatide starts to lose effect before the full seven days, so I wanted something else to cover that gap.

Did you just use bac water for the mix or add AA water too?

Three milliliters of BAC water only.
