BPC-157 + TB-500 for Injuries or Surgery: What Doses and Schedules Are People Using?

10 replies7 peopleSep 23, 2026☆ Follow
Summary

What doses, frequencies, and cycle lengths are people actually using for BPC-157 and TB-500 when recovering from muscle or tendon injuries or after surgery?

Users report running BPC-157 at 500 mcg twice daily and TB-500 at 1-1.5 mg daily, often for 4-10 weeks, with some preferring separate vials so each peptide can be dosed independently. Several noted faster pain relief and better-than-expected healing after rotator cuff or knee issues, including one case with multiple shoulder repairs where the surgeon and PT were surprised by progress. Most inject BPC near the injury site and TB systemically, and a few combine both in one shot. No one reported side effects in the posts.

What this discussion establishesWhere people disagree

Some prefer a pre-mixed blend for convenience while others keep the peptides in separate vials to adjust each dose independently.

Whether results differ meaningfully between minor strains and major tendon repairs or between immediate post-op use and delayed start.

Nothing here is advice.

Hi everyone. I'm seeking real-world input on how much BPC-157 and TB-500 to use, how often, and for how long when dealing with muscle or tendon problems. The main situation is a man over 60 who tore his rotator cuff completely after a fall more than a month ago and is having surgery tomorrow. The shoulder damage is significant, so peptides alone won't fix it, but he wants to try them afterward to support healing. I store the two peptides in separate vials instead of a premixed version because that lets me change each one's dose or schedule on its own. I still draw both into one syringe. What amounts, timing, and cycle lengths have others tried? Did the protocol change between small muscle pulls and bigger tendon work? Any side effects or reasons to skip the right-after-surgery window?

I'm currently running a blend to ease pain in an arthritic knee. I pin 2 mg once a day but am thinking about switching to 1 mg twice a day, morning and early evening. The plan is to stay on it for a month or two. Only two weeks in so far, so I don't know if it will produce lasting change, but it seems like it should help after surgery or an injury. What I can say is it gives quicker relief than anything else I've used. Stairs usually hurt, yet fifteen minutes after the shot I can go up and down without pain.

I use a 50/50 blend and take 500 mcg of each in the morning. Then I add another 500 mcg of BPC only in the evening.

I keep them in separate vials on purpose. Five hundred micrograms of BPC twice a day, once when I wake up and once before bed. One milligram of TB once a day. Running an aggressive approach for a knee injury. Never had surgery.

Sounds like dosing BPC twice a day is the way to go. When you take the TB, depending on how much you reconstitute, do you combine it with the BPC in one shot to cut down on injections? Also, how long have you followed this schedule? Any noticeable changes after running it for a while?

Been on it six weeks. Getting an MRI tomorrow but I probably have a low-grade tear or some other knee problem. It's definitely improved. I'll post the results when I have them. No other aches or pains either. A couple pounds lighter even though diet and training stayed the same. Probably just lost some inflammation. I keep them separate and shoot the BPC above the knee then the TB into my abdomen.

I blended the substances by hand inside one cartridge. Getting the alternate version might have cost a little less yet that kind never appealed to me.

I had shoulder surgery back in March that included a patch on the rotator cuff plus three tears, a bicep tenodesis, and a labral tear. Six weeks in a sling, then started strength work at week seven. I ran low-dose copper peptide for ten weeks along with 2 mg BPC daily and 1.5 mg TB daily for the first two weeks, then dropped to 1 mg of each daily. Added a couple other compounds for a while. Stopped everything around week ten or eleven. Felt like it went well and both the surgeon and physical therapist were surprised at the outcome.

The real difference for me shows up once I reach 1.5 mg TB daily and 1 mg BPC morning and night, totaling 2 mg BPC. Lower amounts work but results keep getting better as the dose rises, within reason. I hold that level for four or five weeks, then drop to 1 mg TB daily and 500-750 mcg BPC twice a day for another four or five weeks. Typical cycle is eight to ten weeks, though twelve is possible before taking a break. First time users should start lower, like 1 mg TB every other day and 250 mcg BPC twice daily, then increase every five days or so.

If someone is already taking the copper peptide blend in the morning at 0.5 mg each, would you add more of both peptides at night from separate vials, and at what amounts?

The VA already posted my MRI results from this morning. Everything looks good. Seems the peptides did their thing.
