Break through food noise?

7 replies7 peopleApr 8, 2026☆ Follow
Summary

Why does food noise return sometimes on long-term GLP-1 use and how should occasional lapses be handled during maintenance?

Users report that appetite suppression can weaken temporarily even after years on the medication, leading to rare extra meals or fast food without derailing overall weight stability. Most view these episodes as normal fluctuations that do not require action unless frequency increases or weight starts to rise. The group agrees that livability matters more than perfect adherence and that old habits can reappear independently of the drug's effectiveness.

I reached my target weight and have held it steady with a GLP-1 for two and a half years. On rare occasions I still have an off day and pick up fast food for an unplanned fourth meal or late snack, something that happened far more often before starting the shots. Has anyone else noticed this pattern? The good news is my weight has stayed stable anyway.

Nice work reaching your goal and keeping it there. If these moments stay infrequent and your progress holds, it seems fine to allow yourself the occasional treat.

It would help to name the specific medication rather than using a code word. Since your weight is staying off, does the occasional slip really matter? Long-term maintenance is tougher than the initial loss and ultimately depends on whether the eating pattern feels sustainable without constant struggle. An infrequent less-healthy meal or larger portion is usually harmless provided it stays occasional and does not escalate into uncontrolled eating or weight gain. If the episodes feel like a loss of control they might resemble a very mild form of binge eating, but only if they actually create problems. How often this occurred before weight loss could also influence how concerning it feels now.

I started GLP-1 treatment in 2021, hit my goal the next year at a BMI of 22, and have maintained since while currently using 2.5 mg Sema plus 1 mg Cagri. Appetite does rise at times and I sometimes eat more than planned, yet the overall weight holds steady. Early on the suppression felt so strong that food seemed unnecessary, but that intensity faded and hunger returned at much lower levels than before treatment. On those days I simply remind myself to reset. Even naturally thin people have higher-intake days, so isolated slips are not an issue unless they lead to regain. Watch whether the scale is moving up, whether the episodes are becoming more common, or whether they feel harder to manage; a small dose increase might help if needed. The medication handles most of the work, but appetite can still break through occasionally.

This happens to me as well. I try not to beat myself up, yet I still worry about sliding back into old patterns and losing control. That same concern is why I have avoided alcohol completely since beginning tirz, because the two used to feed each other in a nonstop cycle.

Some weeks the medication suddenly seems far weaker, almost as if I had injected plain water, even when using the brand version. A week later the suppression returns. The first time this occurred I panicked and assumed the drug had stopped working for good, but it corrected on its own. Now I expect these dips and simply do what I can to manage them. I have not seen any reports of people regaining everything while still taking the medication, which is reassuring.

That matches exactly how I feel after my third Tirz injection. The first two shots removed food noise and hunger right away, but today after the third shot I notice mild noise again, ate a little extra during the day, and now want something sweet. I even wondered whether I administered the shot properly. I am still on the starting 2.5 mg dose in week three.

Reading the original post, my first reaction was that it might point to mild BED or another eating disorder because it sounded psychological. Old habits can also resurface even while on tirzepatide. Fast food in moderation is acceptable, and the described bad day plus return to an old coping method is probably more about psychology than the medication failing.
