Cagri titration suggestions

18 replies11 peopleJun 4, 2026☆ Follow
Summary

How should someone titrate cagri when adding it to tirzepatide or retatrutide without pushing side effects too far?

Users report starting low and increasing slowly while monitoring appetite and digestion. Several note that effects build over weeks due to the long half-life, so repeating a dose does not necessarily intensify sides as much as feared. Some combine low cagri doses with maintenance reta or tirz for added suppression and slower gastric emptying. A few have used it intermittently or stopped when nausea became constant.

What this discussion establishesWhere people disagree

Whether the body develops tolerance to cagri over months or a year, with some claiming the effect holds steady and others reporting stalls or need to stop due to nausea.

Still open

How long the complementary benefit lasts when cagri is stacked with reta or tirz at steady doses.

Nothing here is advice.

After many weeks at 250mcg and then fewer at 350mcg I went up to 500mcg. I am right at the limit of what feels okay. The results are good but any stronger and it would get rough. Should I stay at this dose for a second week so my body can adjust or drop it to keep accumulation from going past 500mcg? I want to hold exactly here. Tough to predict but curious what others think.

I began Cag last week. Why increase the dose? Is the lower amount no longer working or is the effect wearing off? I used 250mcg last week and 350mcg last night. Nothing noticeable yet. I planned to add it to a low maintenance dose of Reta.

Nothing at 250mcg, a bit at 350mcg but it still felt like it could improve. I am also on 12.5mg of Tirzepatide.

That matches where I am. This week I used 266mcg of cagri while on a low 1mg maintenance dose of reta for eight weeks. I am adding cagri as a complement. Appetite and hunger have been zero since starting reta, but with cagri digestion seems even slower. Food stays in the stomach longer, which keeps me full. When I eat I get full faster than on reta alone. It is pretty noticeable already. Stick with 500mcg for several weeks because it takes time for cagri levels to stabilize.

Took 5mg as one dose with no problems.

I began at 500mcg, which may not have been the best idea, but now that I am here I am continuing. This was my third 500mcg dose on Monday. The jump from 350mcg to 500mcg does not seem large. Because cagri has a longer half-life than tirz or reta the first 500mcg dose has not yet shown its full effect. That extra buildup is unlikely to reach any real danger level. Phase 3 studies used 100 percent jumps every four weeks, larger steps than yours, so you should be fine.

Danger zone was overstating it. I just meant any stronger and I would not be able to eat at all. Faster weight loss sounds good but it is not practical.

I understand now. That is a real possibility with cagri. It is tempting to skip eating but not sustainable. I have been having trouble hitting even a large calorie deficit and can only focus on getting enough protein.

How did you arrive at 266mcg? Drawing that amount consistently seems difficult.

Yeah it is an odd amount. I have a 10mg vial with 3ml bac water. To hit 250mcg I would need 7.5 units, which is hard to measure, so I draw to 8 units instead and get 266mcg.

Stay at the current dose until it stops working the same way, then increase.

PatientHarbour23archiveJun 8

Keep the dose as low as possible for as long as possible.

I was not asking about standard schedules, I know how those work. I wanted to know how much a second identical dose would add compared with the first. I did not want it to get much stronger because I was worried about not eating. I took it last night so we will see what happens.

I have used it over a year on and off. Started at 0.1mg for a few weeks to check for reactions, then raised by 0.1mg every three to four weeks. Now at 1mg weekly. It works well with tirz or reta when their effect fades. I had to pause a couple times because of constant nausea. These peptides take weeks to show full results so do not rush increases. Once you reach the highest dose the body adapts and progress stalls.

To answer my own question, repeating the same dose the second week did not make side effects worse. I am actually hungrier than I have been in a while, even before starting cagri. Not sure why, but I will keep going.

Have you stayed on the same dose for over a year? I have been on the same reta dose for nine weeks since I began, at 1mg per week. Last week I added 266mcg cagri. The reta now seems stronger than when I first started even though the dose is unchanged. I expect the same with cagri. I am using it to support reta and think the effect will hold rather than fade. No studies show tolerance but results probably differ by person.

ClearBeacon15archiveJun 12

I use 1.25mg along with my tirz dose and that works well.

CopperSignal46archiveJun 13

I take it only when needed rather than on a fixed schedule like my tirz and reta. So far 0.25mg has been enough and it lasts a little over a week. I do tirz or reta on Fridays and add cagri only if appetite suppression feels insufficient. Its only role is appetite control so I use it only when tirz is not enough. Ten days since the last dose now.

I have been raising cagri by about 0.1mg per month and have been at 1mg for three months. Reta is at 10mg. Lost 102 pounds in two years with five more to reach goal.
