Hi everyone, I'm 5'11 and currently 285 pounds. I'm on 10 mg of reta but food noise is returning and I'm not getting full as fast as I used to, even after eating a full meal, unlike when I was on just 1 mg. I'm thinking about adding tirz to help with appetite and noise. Any suggestions or advice would be great. I've also been stuck between 290 and 285 for the past four weeks and could use ideas on breaking the plateau. Thanks for any help.

I dunno man. What works for one person might not work for another. I'd keep the reta going and try adding 2-4 mg of tirz, or maybe 0.25-0.5 mg of sema or cagril, then see how it affects the food noise. Good luck with it.

I use 8 mg tirz and 7 mg reta and that feels like my sweet spot. Mileage may vary though.

Cool response there. By the way could you share some background info on yourself since I still count anyone under sixty as youthful. How many months have you spent on this med. What weight did you begin at. Do you monitor your daily intake. What's the end goal. Dosing at that level comes across as pretty advanced.

Just sharing my own experience, not claiming to be an expert. I started on tirz and liked it, got to 7.5 mg, then added a second weekly pin for more appetite control. I believe in twice-weekly pinning. I added reta for the glucagon effect and now pin 7.5 mg tirz on Friday and 4 mg reta on Tuesday. Started my GLP journey on March 6th 2026 and haven't had a plateau longer than five days. Weight loss has stayed consistent and easier than other diets. I bought some cagrilintide for later but haven't used it yet. If I stall near goal I plan to add 1 mg on a third day to hit amylin receptors instead of pushing tirz or reta higher.

Nice write-up, but you're braver than me with the cagril. From what I've heard a first 1 mg dose can be pretty strong and maybe rough. I have some cagril and sema set aside for the same reason the OP mentioned, but I'd approach either one very cautiously.

If you're splitting two different GLP-1s you'd usually pin them 3-4 days apart.

I agree. I'd start at 0.25 for a few weeks and work up to 1 mg once I saw how I handled it. I've had zero sides from tirz or reta so far.

I'd increase reta first before adding anything else. I went up to 20 mg of reta then started adding 0.25 mg of sema. Hit my first goal weight this week and am now lowering reta, planning 6-7 mg on Monday since I split doses. I haven't had a sema bump in a week and am waiting to see if food noise returns. Chose sema because I hadn't used it yet and it gave the strongest sides, though still mild. I split 0.125 mg twice weekly and sometimes bumped to 0.1875 mg. My general approach is to escalate until sides become the limit. I'd add more reta at least to 12 mg per week, then try 1 mg of tirz and adjust from there.

My experience is a bit different since I started on tirz, but if I were in your spot I'd begin with 2.5 mg tirz on a day I don't pin reta. Check for sides and whether it helps, then increase gradually until it feels right. It took me about two and a half months to dial in the best balance.

I couldn't see what weight you started at, which might matter if you've already lost a lot versus just a little before stalling. Usually increased hunger and slower loss happen after significant weight reduction because the drug is now mainly preventing regain. If cost isn't an issue, early research suggests eloralintide could be a strong add-on, giving an extra 11 percent loss over 32 weeks when added to 15 mg tirz. That seems better than cagri based on current data. Adding some tirz or cagri are still reasonable, or simply going above 12 mg reta if sides stay low. Start cagri very low like 0.25 mg. Tirz might control food noise better than reta according to what people say online, though reta does...

I've heard folks claim that eloralintide offers improved tolerance over cagri. At present I'm solely using tirz, though my plan involves transitioning to reta first, followed by incorporating eloralintide should the hunger signals return.

I just took my fourth weekly dose of reta on top of my ongoing weekly tirz dose after six months. Started tirz at 2 mg, went up to 2.5 then 3 mg, and have been on 4 mg for the past few months. I've changed my diet, sleep, and work out four times a week for 1.5 hours mixing weights and cardio. Lost close to 35 pounds in a little over six months with tirz and lifestyle changes. Switched toward reta for the extra health benefits as I near goal. Began with 2 mg reta about three days after my 4 mg tirz shot. Original plan was a month at 2 mg reta then up to 3 mg while dropping tirz to 3 mg, and so on until fully on reta. But I'm really impressed with how I feel on the current 4 mg tirz plus 2 mg reta combo, so...
