Time to change up the rotation!

Summary

How should someone adjust a tirzepatide and retatrutide stack when facing strong food aversion, fatigue, and dizziness while still lifting weights?

Users report that combining the two drugs at moderate-to-high doses quickly produces excessive appetite suppression and postural dizziness. Most advise dropping tirzepatide entirely, lowering retatrutide if needed, and focusing on electrolytes plus adequate carbohydrates rather than protein alone. Several people who switched to retatrutide monotherapy describe a different hunger pattern: they feel hungry but become full after very small meals. Dizziness is widely attributed to blood-pressure drops from the medications plus rapid weight loss and is managed by increasing salt, carbs, and fluids.

Whether dizziness eventually resolves on retatrutide monotherapy or remains a persistent issue for some users.

Nothing here is advice.

Hey everyone, time to switch things around. I had been using tirzepatide at 10 mg and began moving over to retatrutide. Current schedule is retatrutide 8 mg on Mondays and tirzepatide 5 mg on Fridays. Food sounds completely unappealing and I have to push myself to eat the first two meals. I expected retatrutide would still leave some hunger so I kept a bit of tirzepatide, but that turned out unnecessary. Tuesdays and Wednesdays after the retatrutide shot I feel wiped out. Even with daily electrolytes and carbs like potatoes or rice I get lightheaded when I stand up after squatting, almost to the point of blacking out. Still lifting four days a week and aiming for 130-150 g protein at 183 lb body weight.

Did you jump straight to 8 mg retatrutide on your first try or work up to it? Also alternating 5 mg tirzepatide with that 8 mg retatrutide for a weekly total of 13 mg? If the goal isn't rapid weight loss that seems high. I always start low and creep up only as far as needed. Started retatrutide in early February at 1 mg split, now at 4 mg split and down 20 lb while still training. Plenty of room to stay here before backing off for maintenance.

On 3.5 mg tirzepatide now after starting at 2.5 mg seven months ago and peaking at 3.75 mg every five days. Planning to add retatrutide at 1.5 mg every five days for four weeks, then 2 mg while dropping tirzepatide to 2.5 mg every five days. Even that feels like it might be too much, so yeah, lowering both would be my call.

Stacking the two at once is too much. Taper off tirzepatide gradually and stay on retatrutide only. Raise the retatrutide dose later if appetite control slips, up to 12 mg if the trial data supports it.

Appreciate the feedback. I did work up to the 8 mg retatrutide. Tirzepatide was on a separate day. Stopping tirzepatide right away and will reassess retatrutide through the rest of the month. Might drop to 6 mg if the strong food suppression continues. Expect it will improve once tirzepatide clears.

The effect on appetite is noticeably different once tirzepatide is gone. Tirzepatide mutes hunger almost completely; retatrutide lets hunger return, sometimes strongly, but fullness hits after just a few bites. I crossed over from 7.5 mg tirzepatide by starting retatrutide at 2 mg while lowering the other. Now at 6 mg retatrutide split and off tirzepatide since early April. Still waiting for the last tirzepatide dose to fully clear, which takes more than a month. I get hungry once or twice a day but stop after seven or eight bites of dense food. Hit protein and fiber targets throughout the day anyway and add vitamins. After a month solo on retatrutide I plan to stay there until maintenance.

On 3 mg retatrutide plus 0.5 mg cagrilintide. During workouts lately I have to pause, bend over, feel lightheaded and close to passing out. Blood pressure has always run low so that's not the cause. Ordered electrolytes. Eating enough with decent macros and plenty of water. Read that retatrutide can cause dizziness as a side effect and hope it settles.

Nope, eat everything. Balanced macros with measured sweets, solid protein, and avocados included.

Sounds like postural hypotension: blood pressure fails to rise fast enough when you stand, cutting brain blood flow and causing dizziness or near-fainting. Weight loss itself can trigger it, and both drugs lower blood pressure further. Fluid loss, lower salt or electrolyte intake from eating less, reduced thirst, and blood shifting to muscles during lifts all add up. If you're not trying to lose more weight and already struggle to eat, lowering one or both doses is the logical next step.

Same explanation applies here except you aren't planning to drop doses. More details would help narrow it, but during exercise stay cautious; if you feel faint coming on, get your head below heart level right away. Electrolytes are worth trying and so is timing a meal before training.

Thanks for the detailed breakdown. Since posting I've looked into it more and fixed most of the dizziness with three changes: more healthy carbs like potatoes and rice, dropping tirzepatide completely and keeping only retatrutide, and daily electrolytes. Now only mild dizziness shows up around injection day. A lot of people seem to overlook that retatrutide users still need decent carbohydrates instead of focusing almost only on protein; skipping them can throw things off and increase dizziness.

Three years and it still happens? That's rough. I was hoping it would fade after a few weeks. Mine might be tied to the weight loss; I dropped about ten pounds in a month without realizing it. Electrolytes didn't help you at all? Is there anything else that eases it?
