Maintenance dose survey, who here is doing this, and what can you tell us about it?

Summary

What doses and strategies are people using for long-term maintenance on tirzepatide, retatrutide or similar after reaching goal weight?

Users report a wide range of approaches from keeping high doses that produced the loss to dropping substantially or switching compounds. Some maintain on the same dose once weight stabilizes while others titrate down gradually or add stacking agents. Experiences vary by individual response, with some noting food noise returning at lower doses and others successfully holding weight on micro-doses or extended intervals.

Hi, started this path in May 2025 and have lost roughly 24 percent so far. Looking at a mix of Tirz and Reta around 3 mg of each. Still figuring out my target weight but it is getting close. Thinking ahead about staying on these long term, whether that means one or both, what doses others settled on and why, plus how slowly to reduce for the best outcome. Would appreciate hearing what has worked for people.

I am an unusual case because I dropped over 90 percent of the weight before starting any GLP and now use them only to hold the result. Right now that is 15 mg tirzepatide plus 4.5 mg retatrutide, any higher and the skin sensations become bothersome, and I just added 0.25 mg cagrilintide. Goal is keeping a 54 percent total loss.

Congratulations on the loss, that is impressive work.

That amount looks high if the only goal is maintenance.

Lost 90 pounds from 275 down to 185 on 15 mg tirz over 13 months then stalled. Goal is 165. Added 5 mg reta mid-week on the tirz schedule and the scale started moving again, now at 174. Doctor suggested lowering the tirz, so I am considering phasing it out while increasing reta to a moderate level based on how it feels.

Reached goal and now using 0.8 mg survo plus 5 mg trz each week for maintenance. Might lower the trz slightly because I dropped a pound without trying this week, or maybe just have one big meal for Chinese New Year.

Studies show no reason maintenance doses need to differ from the amount that produced the loss, provided weight has stopped dropping. Graphs in the trials stay flat for long periods once that point is reached. Forum reports of reaching target without stalling are not the norm in trials, likely because of motivated participants and extra agents. Keeping large losses off without these drugs has historically succeeded for only a few percent of people. Long-term safety data is still limited because the drugs have not been available long enough. This matches my own history of reaching normal BMI then regaining after becoming obese at age seven.

What age, height and current weight? Mine are 60, 6 foot, 172 lb male. Now on 2 mg reta weekly for maintenance and I am okay with a bit more food noise than tirz since I am not trying to lose. It still controls cravings enough that meals stay mostly healthy. Finally adding muscle without fat. May lower further once body fat target is met.

64 years old, 6 foot, 172 lb. Plan is to stay within 175 plus or minus 5 lb for life. Starting to lower tirz from 15 mg to 12.5 mg weekly while watching the scale. If weight falls below 170 I will reduce faster; near 180 I will hold or go back up. Nice to manage this without doctors or insurance. Hoping habit changes will eventually allow an even lower dose while keeping the benefits.

Before this group I spent time in related subreddits that helped me locate compounding sources early on. Those places once had useful maintenance threads with some people dropping doses a lot and others keeping the same level, but the content has mostly disappeared. Hoping to gather current experiences here instead. Current weight is about 144 lb at 5 foot 6, aiming to recomp rather than lose more. Used to feel best around 130-135 but with low muscle, so now focused on strength and unsure what final weight makes sense or how much buffer to allow for possible gain.

On reta for maintenance since May 2025. Started with a cagrilintide stack but stopped the cagrilintide in November 2025 after dropping 15 lb past goal. Now stable on 3 mg once a week.

Should reach goal around March. Planning to stop the survo then and stay on 8 mg tirz. If weight becomes hard to hold I will raise the tirz a little but prefer not to stay on survo long term.

About to begin lowering to a maintenance level for a clean bulk after hitting goal weight. Current schedule is 7 mg weekly split as 2 mg every other day. Hope to reach around 2 mg weekly.

61, began GLP in August 2023 at 192 lb after highest of 250. Slow loss but that was fine. Hit goal of 155 lb last March or April. Micro-dosing did not work, but spacing doses 13 days apart has been successful.

Which peptide and what dose right now, if you are willing to say.

NorthQuill51archiveFeb 6

On a maintenance dose of 5 mg tirzepatide for over a year. Early on that amount produced about a pound a week loss, but now it holds weight steady near goal while I focus on recomp for the final 10 lb. Never made a deliberate switch to maintenance, the same dose simply became one once my body adapted. Food noise is starting to return so I may go up to 6 mg to keep things stable. Will probably stay with tirz since it has worked so far rather than change compounds.

Same situation here. Using 5 mg tirz every seven days hoping to maintain, but now considering keeping the dose and dosing every five or six days instead because food noise and cravings are becoming distracting again.
