Summary

how to titrate retatrutide while managing side effects and deciding when to escalate or hold doses

users report highly variable ramps from 0.25 mg every five days up to 4 mg or higher, with side effects including skin sensitivity, elevated heart rate, poor sleep and mild GI issues that often improve with time or split dosing. appetite suppression and weight loss occur at lower totals for some, while others stack with additional compounds or favor caution based on body composition or cardiac history. debates center on whether slow titration builds tolerance or simply delays results and whether to hold or increase doses during plateaus.

What this discussion establishesWhere people disagree

whether gradual ramps waste time or meaningfully improve tolerability and whether to hold steady or escalate when weight loss stalls

Still open

optimal starting doses, reliable sleep management approaches, and long-term maintenance protocols

Nothing here is advice.

I want to open a thread on starting doses for Reta and collect actual user experiences. My own approach was cautious at first: 0.5 mg weekly for two weeks, then 1 mg for the next two weeks. During the second week on 1 mg I split the dose and added another 1 mg. My current schedule is 1 mg Tuesday morning at 6 am and 1 mg Friday evening at 6 pm. Only at the full 2 mg per week do I feel the dose is appropriate. Earlier I noticed almost nothing beyond a mild effect right after the shot. I chose this route after reading accounts from others who went straight to 2 mg and ran into side effects they regretted. Now I’m questioning whether beginning below 2 mg actually helps with adaptation or just postpones noticeable results. My weight stayed flat until I hit 2 mg total weekly. I’m already fairly lean and not carrying a lot of extra fat, so that might affect how it works compared with heavier users. What has everyone else seen when starting?

LevelAnchor55archiveMay 13

You can’t predict tolerance ahead of time. I began at 1 mg and added 1 mg each week until reaching 4 mg three weeks ago. I held at 3 mg only when switching sources because I was unsure if the new one would feel the same.

LevelSignal46archiveMay 13

I began right at 2 mg weekly and got bad diarrhea that never eased, so I dropped back to 1 mg and things settled.

Thanks for sharing that. I keep wondering whether jumping straight to a higher dose would have triggered sides for me too.

CopperAlder25archiveMay 13

You handled it the right way. Slow increases seem wiser, especially if you’re not carrying much extra weight. At 1.5 mg I first noticed real appetite suppression and stronger glucagon activity. Sleep felt worse and resting heart rate climbed. At 2 mg, where exactly are you seeing the main benefits—appetite drop or actual scale movement?

PatientLantern92archiveMay 13
↳ replying to @CopperAlder25

Both effects showed up. The scale stayed still until then. I used to eat big portions and even had stronger sweet cravings early on. Only now have the constant food thoughts eased. I still eat five or six small meals daily to keep protein high, but the portions are much smaller and spaced out.

I messed up my very first dose and took 4 mg instead of the planned 2 mg. Felt awful and lost all appetite. Week two at 2 mg brought cramps plus alternating constipation and loose stools. From week three I switched to 1 mg Monday and 1 mg Thursday. That’s been ideal—no sides and steady fat loss, maybe even too fast. Appetite is mildly reduced but food noise is gone completely. I stick to my macros and hit protein every day. I’ve guided two friends onto it and told them both to start at 0.5 mg. One had sides the first week but they cleared by week two; next week he moves to 1 mg. Everyone should begin at 0.5 mg, let the body adjust, then climb slowly. Most people seem to settle around 2 mg without losing weight too quickly.

PatientLantern92archiveMay 13
↳ replying to @LevelBramble49

Thanks for the report. I’m still curious whether I would have run into sides if I had started at the dose we’re both using now. Right now this schedule feels comfortable.

WarmThistle43archiveMay 13

I started directly at 2 mg weekly and had diarrhea, felt cold all the time, and skin became sensitive. Those faded after about three weeks. When I moved to 4 mg I split the dose and got a milder version of the same sides for the first week only. Since then no major issues. Every time I titrate I seem to get a short run of sides; I just went to 6 mg and feel chilly again. Started at 197 lb, 5 ft 3 in, obese category, so plenty to lose. Been using since January 24th.

PlainBramble29archiveMay 13

I began research at 0.25 mg to check tolerance, then moved to 0.5 mg. Now at 1.25 mg heading to 1.5 mg. The subject still feels nothing and eats too much. Previously on another compound at 15 mg for nearly two years, now stepping down to 12.5 mg then 10 mg later this month. Has already lost 38 % of body weight and I’m unsure if that’s the end point. Need to run this compound for a few months to see.

AmberSparrow73archiveMay 13

Started at 1 mg weekly then 1.5 mg the following week. Using it for metabolic reasons rather than weight loss. Began dropping weight I didn’t want to lose (BMI already 19.3) so I returned to 1 mg weekly.

PlainAlder11archiveMay 13

Followed the trial schedule at first then adjusted: 2 mg for four weeks, 4 mg for five weeks, 6 mg for six weeks, 7 mg for three weeks, now starting 8 mg. Average loss has been 2.2 lb per week over 18 weeks, totaling 40 lb or 18 kg.

NorthThistle70archiveMay 13

The phase-3 schedule was 2 mg weekly for four weeks, then 4 mg for four, 6 mg for four, 9 mg for four, and 12 mg for four. Reached 12 mg for two weeks and hit goal, then dropped back to 6 mg weekly.

LevelCompass97archiveMay 13

Started at 1 mg this morning. Will report back later if anyone wants updates.

Definitely noticed effects from Wednesday evening onward. I tend to respond strongly, which is why I began at 1 mg. Mild nausea yesterday but nothing serious. Appetite suppression is working well as of Friday afternoon even though I’d heard it might be weaker. Also noticed some extra anxiety, nothing extreme. Has anyone else felt that?

BrightMarble24archiveMay 16

Thanks for the thread; it answered most of the questions I had before starting my own run.

LevelCompass91archiveMay 16

I was concerned about resting heart rate after seeing it rise on another compound. Started at 1 mg for two weeks, then moved to 2 mg once it looked fine. Followed the trial titration after that. Sides only appeared at 6 mg (bad allodynia). Reached goal weight at 8 mg then titrated down. Now at 3 mg with no sides and weight holding steady.

BrightSparrow84archiveMay 16

First dose was 1 mg Wednesday night. Had trouble falling asleep despite magnesium and glycine. Stuck to protein shakes and vitamins during the day even after early training, then had a normal dinner Thursday and Friday. Felt the first real lunch craving today and acted on it. Thinking about another 1 mg tonight and then keeping a split schedule with the next dose next Wednesday.

PlainAlder11archiveMay 16

PlainAlder11archiveMay 17

I don’t track exact numbers. Just include it at breakfast, a shake at lunch, and meat plus fresh vegetables at dinner.

I started at 1 mg and felt what seemed like panic attacks but was probably just a faster heart rate. Chest felt tight and breathing was shallow. Had an episode at the gym and paramedics checked me over but found nothing concerning. Dropped to 500 mcg for a few weeks, then raised by 250 mcg every three or four weeks. Eventually reached 2 mg but have since settled at 1.5 mg. Coming up on 14 months total.

WarmCinder32archiveMay 17

Began last Monday with 1 mg and felt mild sides—crampy slow digestion and slight hunger reduction. Took the second dose Thursday so now at 2 mg weekly. Similar feelings after the second shot but food noise is lower. I can still eat but don’t feel starving; I eat because I need to. 58-year-old male, 6 ft 2 in, currently 230 lb (was 238 lb a week ago). Plan is to stay at 2 mg weekly and increase only when needed.

PlainAlder11archiveMay 17
↳ replying to @WarmCinder32

Eight pounds in a week is a lot. If accurate it’s probably water. Did you also start dieting at the same time?

AmberLantern34archiveMay 17

Starting dose probably depends partly on how overweight someone is. Splitting seems to reduce sides for many people, so I recommend beginning with 1 mg for the first injection if obese, watching closely for the next few days, then adding another 1 mg three-and-a-half days later if it feels manageable. Hold that level for at least two more weeks.

PatientLantern92archiveMay 17
↳ replying to @BrightSparrow84

I sorted out my subject’s sleep using DSIP.

PatientLantern92archiveMay 17
↳ replying to @AmberLantern34

That matches how my subject is running right now. Considering raising the dose by 0.5 mg by the end of next week. Appreciate the input.

ClearQuill72archiveMay 17
↳ replying to @PatientLantern92 (opening post)

I’m only on week four but took a similar path: 0.5 mg for two weeks then 0.75 mg for the next two. Even at those lower amounts appetite suppression and reduced food noise are already strong, so I’m not in a hurry to increase. Goal is the smallest effective dose. I’d rather climb slowly than overshoot, deal with sides, or speed up tolerance. It also lets me see exactly what each change does. People define “working” differently—scale movement, appetite control, body composition, inflammation, etc. I’m around 13 % body fat, eating roughly 2000 calories with 200 g protein daily, so scale movement is modest but body composition is visibly improving. Slower titration helps with adaptation and awareness even if peak effects arrive later.

ClearQuill72archiveMay 17
↳ replying to @PlainAlder11

It mainly means slower titration gives more time to watch how the body reacts before changing anything else. Hunger, energy, sleep, recovery, mood, cravings, GI effects, training performance and so on can all shift, and jumping doses makes it harder to know what caused what. By self-awareness I just mean noticing those patterns instead of chasing faster results right away.

SlowLantern29archiveMay 17
↳ replying to @ClearQuill72

I like the careful details in your posts and the idea of working gently with the body instead of forcing it.

WarmCinder32archiveMay 18

I was already eating fairly clean, mostly keto-style but not high-fat. The compound helped with willpower so I simply ate less often. I had been stuck at 238 lb for weeks, so dropping to 230 was a pleasant surprise.

ClearCompass37archiveMay 18
↳ replying to @ClearQuill72

My thinking is similar. Started at 0.25 mg every seven days for four weeks, then 0.5 mg every six days for four weeks. Now at 1 mg every five days and will stay there at least four weeks before going up. Strong appetite suppression and lower food noise are already present. Even at these low doses I’ve had some sides: nausea and fatigue at 0.25 mg, higher resting heart rate at 0.5 mg, and irregular cycle since the start. Scale movement is minimal but I expect it later. I don’t want to lose more than 1–1.5 lb per week, so the plan is to titrate every four weeks only until that rate appears, then hold until it stops working.

AmberKettle85archiveMay 18
↳ replying to @PatientLantern92 (opening post)

I actually started with 1 mg last Wednesday. Besides more frequent bathroom trips I didn’t notice other sides or the effects I expected. Added another 1 mg on Saturday because I wasn’t sure the vial was even active. Food noise stayed about the same except maybe slightly smaller portions after meals. Also wondered if it was just because I’ve been eating the same meals daily for over two weeks. Might try a bit higher next week if effects still don’t appear.

How much weight have you lost across those 14 months?

SteadyMeadow94archiveMay 20
↳ replying to @PatientPebble33

I began mainly because I heard it could cut alcohol intake. Drinking is now about half what it was. Lost 30 lb on keto before starting this compound. Went from 178 lb down to 154 lb, which was too much. Consistent biking and swimming helped as well. Have been hovering around 170 lb for the last five months. Things feel steady at 1.5 mg. Not dieting or doing regular cardio right now.

I know you said not to ask, but I have to—how did you accidentally double the intended dose? Your story might help someone else.

LevelBramble49archiveMay 20
↳ replying to @WryCinder14

Haha, I used the wrong amount of bac water to reconstitute—1 ml instead of 2 ml for a 20 mg vial. So what I thought was a 2 mg dose turned out to be a much stronger 4 mg dose.

My situation is quite comparable. I tried an initial testing regimen that stepped up 2-4-8-12 over four-week intervals. I tweaked it somewhat by doing 2-4-6-8 instead, and right now I'm using 8mg. Should it be necessary, the following step would be 10. So far this has produced solid progress around one and a half to two pounds weekly.

SlowSparrow49archiveJun 4

Began with a low dose in the opening week and sensed almost no difference. Switched to every three days during week two and then noticed some effects, mainly skin sensitivity. It cut down how much I consumed at one time. Shed eight pounds early but that leveled off soon after, probably just fluid. Now in week five I've moved up to one point five milligrams every three days. Sides have stayed the same and I feel fine overall. I'll watch progress and adjust gradually when needed. Taking it easy with no rush. The scale has stayed level lately yet I look leaner, which is what counts for me. How outfits fit matters more than the number.

Taking Reta earlier in the day seems to help me avoid problems with getting to sleep. Pairing that with Tesamorelin later on, after going without food for at least two hours ahead of the shot, has really boosted the quality of my rest. I use Reta just once weekly in the morning hours, and Tesa on five consecutive days then two off.

LevelFenwick53archiveJun 4

I don't see much point in beginning with half a milligram. I'd go straight to a full one and just manage whatever reactions show up. From my experience with others, they usually tolerate one or two milligrams fine right away. With this medication the aim is to raise the amount fast until hitting at least four milligrams. Data from studies showed benefits climbing from around four milligrams and leveling near nine. Below that threshold the strong fat loss effects stay limited. When reactions feel strong, dividing the dose helps. Later stage studies started at two milligrams and added two more each month, with positive results overall. That approach seems solid to me, especially since I've never run into reactions from these medications even at twelve milligrams of one and four of this.

IronKettle76archiveJun 4

I've been going up in tiny steps only. Began at a quarter milligram dose, then added another quarter every five days. Right now I'm remaining at one milligram through three full weeks, meaning four injections so far, before moving up to one point five milligrams.

Because of my atrial fibrillation I realize this has the ability to start episodes which means I raise my dose very carefully and feel no need to go quicker.

Which drugs are they giving you right now? Mine started three and a half years earlier due to a serious lung infection that led to the issue. The facility gave me lots of treatments while I stayed there. My heart expert keeps prescribing a pair of them even today.
