Stacking Tirz with Reta and Cagri Dosing Advice

19 replies6 peopleAug 13, 2026☆ Follow
Summary

How to safely add tirzepatide when already using retatrutide and cagrilintide?

The thread shows users discussing low-dose stacking of tirz on top of existing reta and cagri regimens. Participants advise starting very low with tirz if adding it at all, staggering injection days, and monitoring closely for overlapping effects on blood sugar. Several suggest increasing doses of current medications before introducing a third agent. Personal experiences with fatigue from same-day dosing are shared as cautionary notes.

Right now I'm using 5 mg reta each week plus 0.5 cag and I'm hoping to bring in tirz, but I'm unsure what amount would be safe. Any dosing ideas or thoughts on side effects? I've searched the forum already but haven't come across the details I'm after.

I'm actually going the other way and already on tirz while wanting to add reta. From what I've read, keeping both low seems preferable. The main warning I've seen is that the two act on similar pathways, which could lead to dangerously low blood sugar. I use a CGM with alarms so I get notified right away if levels drop too far.

Hopefully this thread brings some useful input for both of us. Are you type 1? I'm type 2. I've mentioned that here before along with other details and it didn't go over well. Do you remember where you saw the information about low blood sugar risk? I should probably get one of those monitors myself.

Just wondering why you're looking to add tirzepatide. Your current amounts of reta and cagri are still low, so you could probably raise either one before bringing in something else. Most people on the forum seem to add compounds only after reaching a plateau or max doses, which might explain why you're not finding much guidance. If you're set on trying it anyway, begin low and pay attention to how you feel.

I was hoping to try something different instead of just going higher on reta. I've come across people combining tirz and reta at moderate levels but not with cagri included. What starting amount would you suggest?

SlowAnchor19archiveAug 13

A few people here are already stacking these. You already have the two that pair well together. I'd probably raise the GLP first and see if that helps, pinning the GLP on day one and cagri on day four. If I were adding reta and cagri to tirz, I'd pin 10 mg tirz on day one then add 2 mg reta on day four along with the cagri. I'd hold there for a couple weeks and adjust by a milligram or two at a time if needed. I haven't tried this myself yet though. I'm still letting tirz work on its own before adding more.

That's helpful, thanks. Could I begin with a low amount of tirz for now? Maybe 2 mg weekly? Would pinning reta on Friday, tirz on Monday, and cagri on Wednesday work?

It's similar to adding salt while cooking. You can always add more later but you can't remove it once it's in. If you're set on adding tirz, and no one should judge that given the wild stacking going on, start low like 1 mg and check how you respond. The staggered approach mentioned is also worth considering. Pinning cagri the same day as a high tirz dose once was a bad move for me.

Thanks for the input and help. I want to add tirz after looking into it both here and elsewhere. 1 mg seems like a reasonable place to begin. What exactly happened with that same-day pin?

The nice thing about this space is you can test an approach and tweak it if it doesn't suit you. Good luck and keep us posted on how it goes.

Happy to explain. The next three nights I fell asleep on the couch at 8 pm, which isn't normal for me. It wasn't just regular tiredness though. A strong wave of mental fatigue hit and it felt like my mind was rebooting, then I'd be wide awake again fifteen minutes later. The effect was entirely mental and a bit unsettling. Staggering the doses reduced it somewhat but didn't eliminate the feeling entirely.

Thanks, I appreciate it. I'll check back in here after a week or two with an update.

That sounds pretty alarming. How has your sleep been since? The part about waking up wide awake after fifteen minutes is strange. I used to think a mind reboot would be helpful but now I'm not so sure. The first time I pinned reta I took my sleep meds afterward and ended up sleeping sixteen hours and missing class. Even then I didn't feel rested afterward. Did you feel rested at all during those episodes?

WarmMeadow92archiveAug 13
↳ replying to @QuietCinder90

I've always struggled with poor sleep, and now I'm seeking the main reason behind it. The elevated tirzepatide amounts could be the culprit. Earlier I raised it all the way up to 20mg, however I'm now lowering it back toward 15mg or below while boosting my elora intake. Since I'm nearing my target, experimenting with the doses makes sense. Adding some tesa might be something I consider. How did cagri affect you? Did it cause any tiredness?

QuietCinder90archiveAug 13
↳ replying to @WarmMeadow92

Have you considered melatonin? 20 mg is quite high. You're close to goal now and tesa could be interesting to try. How has the other compound been working? Cagri has been fine but I'm often tired anyway due to fibromyalgia and ocd, so I didn't notice any extra effect from it or much change in appetite.

WarmMeadow92archiveAug 13

I tried melatonin and magnesium glycinate but stopped both. The other compound has been good with none of the sides that cagri can cause. A lot of people don't feel cagri effects until higher doses. Looking forward to hearing how your tirz trial goes. Keep everyone updated.

I just began a cycle of sermorelin Monday night and either it's placebo or I'm already noticing less tiredness and brain fog. I'm only using the basic suggested amount and didn't expect to feel anything. Even though my sleep pattern seems unchanged, the fuzzy mental state I've had for years has lifted this week. I'm still generally tired and would love extra rest, but the frustrating can't-clear-it feeling is gone so far. I'll give it a full week before deciding if it's real.

Sorry for the delayed reply. I stepped away from being active here for a bit but good to see the promotion. Glad to hear the other compound is working well and I might look into it later. I've decided to raise my cagri to 1 mg before considering tirz because I've learned about some health issues that need checking first. If I try the stack later once things are stable I'll update the thread with the full details and side effect notes.

CopperHarbour89archiveAug 25

Just wondering, isn't cagrilintide already a lot when tirz and reta are already combined? From what I've read those two seem to cover what each other lacks, so adding cagri might push everything too far.
