To move or NOT to move?

Summary

Should I increase my Tirzepatide dose if responding well at 5 mg or stay until food noise returns?

The group consensus is to remain at the current dose while appetite control and scale movement continue. Increase only after a stall lasting several weeks or when hunger returns. Alternatives mentioned include splitting doses or shortening the interval between injections rather than raising the amount.

I began Tirz back on May 7th 2026. By June 29th, fifty-three days in, the scale shows exactly thirty-five pounds gone. Starting weight was 386, current is 351, and I am six foot two. Feeling thrilled with the complete absence of food noise. I have done the usual schedule of 2.5 mg for four weeks then 5 mg for four weeks. This coming Wednesday I am due to step to 7.5 mg. I am wondering whether that step is necessary given how well the lower amounts have worked. I would rather hold at 5 mg until hunger returns. I do not believe simply having more medication circulating will produce extra fat loss. Any thoughts or advice would be welcome.

Holding at 5 mg sounds reasonable to me. What was your starting weight and what are you aiming for?

Seven months on Tirz has taught me to remain on whatever amount is still working for as long as possible. For me that currently means 10 mg every fifth day instead of every seventh. I would keep the 5 mg going until hunger or food noise starts to show up again. At that point you could try a smaller jump than 7.5 mg, or test every fifth or sixth day. There are quite a few ways to adjust.

I only stepped up after appetite suppression faded or the scale stopped moving. Listening to your own body is the better guide for when to increase. Ignore the trial dosing schedules if you do not need to spend extra money.

Splitting the dose is another option that worked for me. It removed the hunger that used to appear on day six. I injected every three and a half days. That approach also slowed how quickly I needed to raise the amount. Results vary by person, but it helped in my case.

I used to follow the once-weekly schedule and the 2.5 mg jumps exactly as instructed. Now I am testing every fifth day to see how it feels. I might also try half doses every third day. It is still early, but weekly injections are not working well for me because I feel off for two days each week.

I am six foot two, began at 386 pounds, and sit at 351 pounds after fifty-three days on Tirz. My target is 220 pounds.

I went strictly by results. I stayed at each dose until it was clearly no longer enough. Some people reach their goal without ever using 10 mg. I waited thirteen weeks before trying 7.5 mg and finished at 12.5 mg.

The aim is steady, safe weight loss. I would keep the present dose until it fails to produce any change for a full month. Only then consider going higher. Using the smallest amount that still works for the longest time keeps both cost and side effects lower. Trial data showed little extra benefit from jumping to the highest dose right away versus moving up only when progress stops.

IronLedger65archiveJun 29

You are already seeing strong results on a low dose. Remain there until that changes. Return of food noise or several weeks with no movement on the scale are the signals to increase. There may come a time when even the top dose only maintains weight, so the longer you can delay reaching that point the better. These medications tend to produce their biggest drops early and then slow as the body adapts. Leaving room to move up later takes advantage of that pattern.

I have been on Tirz more than eight months and only increase when food noise returns and I notice myself snacking again. I split my doses and raise the amount by 1 mg per week when needed, which means 0.5 mg added to each half-dose. I usually stay on a given level for six to eight weeks. The hope is that this keeps my eventual maintenance amount lower. I still have ten to fifteen pounds left and time to adjust. Stacking another compound is something I would consider if a stall happens at higher levels.

CopperTimber97archiveJun 29

Many others have already said the same, but my own experience lines up. Remain on a dose that is still producing results. Only move higher when food noise returns or you need to break a plateau that has lasted four to eight weeks.

I can only agree with what has already been said. Keep the current dose while it continues to work. If the final days before the next injection become difficult, splitting the dose is one way to handle it.

I am still using the vial I opened on May 7th, so it is nearing the sixty-day mark. The liquid remains clear and I have had no side effects or return of food noise. I chose to take a modest step up to 6.25 mg to offset any slight drop in strength and to keep hunger away during the holiday weekend with lots of food around. That is 32 units instead of the 25 units I used for 5 mg. Once this vial is finished in the next couple of weeks I plan to return to 5 mg, expecting the fresh vial to feel stronger. Thanks for the input.

I find this chart really useful so thanks for putting it out there. It agrees with what I've noticed myself. How did your results look on the July fourth holiday?

I went all out with beer, plenty of whiskey, pizza, steak, and fries. I enjoyed the long weekend with family and did not hold back or lecture anyone. When I weighed myself afterward I was still exactly 351 pounds, the same as before the holiday.
