Taken seven shots total so far. The most recent was 7.5 on Tuesday. I realize the dose is climbing fast but the only problem is zero pounds lost and hunger feels out of control. How long until this starts working? Thinking about adding ret.

People respond at different rates. Moving the dose up faster does not always speed up loss and can just bring more side effects. Is your appetite down at all or are you feeling full sooner? I respond slowly myself and only reached 15 after taking my time, then the scale stalled from November onward. Effects on appetite and fullness showed up in week one but the numbers on the scale crept down very slowly while clothing sizes dropped two sizes. Have your clothes started to feel looser?

Have you used any GLP medications before or already lost weight prior to starting this one? What is your age, weight and height? If the answers are no to the first two questions then you may simply not be very sensitive to these drugs, since most people notice at least some appetite change or a little loss or nausea by 7.5 mg. I would stay with tirz alone for now so you can tell for certain whether it works for you. Adding or switching to reta partway through would leave you unsure which medication is responsible if things improve later. Weight management is a long process and tirz is either the best option available or second best, so knowing its true effect on you is worth another month or two. With no side effects and no diabetes or other major conditions you can raise the dose a little quicker, though there is still some added chance of side effects. I was on ozempic for about four years until the assistance program ended for Medicare patients. I had been off it a month before starting tirz. Starting weight on ozempic was 385 and ending weight was 230. In that month off it crept back up to 240 and now sits at 250. My sister passed away suddenly and I began stress eating. Appetite suppression was not very noticeable so overeating was easy. I began oz with an A1C of 72 and brought it down to 64 through discipline and the medication. I have been monitoring blood sugars closely with the faster dosing schedule and they remain stable, not low.

I would give it more time, at least four to eight weeks total or until you have done two to four shots at 7.5-10 mg, before deciding it is stalled. Track non-scale wins such as measurements, how clothes fit, energy levels, blood sugars which are already good, and any reduction in cravings. Keep calories and protein reasonable even when appetite is not helping. If you still want to try reta later, begin with a very small amount and track weekly. Since you are only at seven shots and reached 7.5 quickly with no issues, staying on tirz by itself a little longer and moving to 10 mg next if tolerated makes sense, because many people start seeing results at 7.5-10 mg after an initial period where nothing seemed to happen. Sorry about your sister, that is really tough and stress eating is a common reaction. You are already doing well on the blood sugar front. Hang in there.

All of that explains quite a bit. People with diabetes often see less weight loss from GLP medications even though they control blood sugar very well. The response also differs when you start at a much lower weight after already losing 130 pounds. Some tolerance to gastrointestinal effects is likely after stopping ozempic only a month earlier. Hunger tends to rise and metabolic rate drops after that much loss. Stress is a common trigger for regain, and my condolences again for your sister. With tirzepatide the first priority may be preventing the 130 pounds from returning, and since tirz is generally stronger for weight loss than ozempic that goal should be reachable and possibly a bit more. Long-term health benefits from avoiding full regain are huge even if additional loss is slower. Higher doses of tirz should still work

I know things have been difficult lately but congratulations on the earlier big loss. There is a good chance you will respond once you reach higher doses of tirz. My mom was in a similar spot after years on maximum victoza then maximum ozempic before starting mounjaro. She felt like nothing was happening until she reached 12.5 mg and then lost another 50 pounds between 12.5 and 15 mg.

Sorry about your sister, you have been through a lot and the weight gain is understandable during that kind of grief. The A1C improvement is still a major win as others have said. This might help: my sister was on Oz for two years then switched to Tirz because her A1C was not improving much. At 7.5 her A1C started getting better but the scale stayed flat. She was unhappy but stayed at 7.5 for three months per her doctor and by the third month the scale finally began moving. Four months later she is now at 12.5 and progress continues.

Thank you everyone. I have lost my patience I guess. It took four years to lose what I did before. But it was a tool and I did the work, learning how much food to eat and hitting my protein goals. I have learned a lot along the way. I will give it more time and see what happens. Thanks again for the condolences, it has been hard. She was my best friend.

Excellent update. Things are progressing well after all. Lack of patience got in my way earlier. Two pounds came off since the last check! Skipping the scale happened for more than a week following hospital release. Previous rounds should have reminded me of this pattern. Gratitude goes out to everyone who read my updates and shared their thoughts. Staying with the present dose level seems right while watching for ongoing shifts. Building courage to try the alternative source comes next. Costs should drop noticeably that way.
