I settled on doses that gave me strong results after dropping about 80 pounds, bringing cholesterol and blood pressure into normal range, and easing sleep apnea quite a bit though some remains from enlarged tonsils. The weekly amounts are 12 mg reta, 10 mg tirz, and 1 mg cagri that I cycle. Hunger is essentially gone, the weight stays off, and overall health markers improved. Has anyone else tried a comparable combination?

Seventy-two pounds lighter after thirty weeks on 18 mg reta plus 0.25 mg sema each week, split into two injections. Loss is still running around 2.4 pounds weekly with another twenty to thirty pounds to go. I keep cagri and another option ready but have not started either yet.

My schedule is close but with more tirz and less reta: 12 mg tirz every six days, 4 mg reta every six days, and 2 mg cagri every six days. The shift came after tinnitus and allodynia appeared when I tried raising tirz from 7.5 to 10 mg. I have since moved tirz up to 12 mg and have been on the new amounts for three weeks, so it is early to judge. Only seven pounds remain to reach my BMI target of just under 30, after starting from 40 and already losing 55 pounds in seven months on tirz alone.

Nothing really negative so far. I only log the whole number for heart rate. A week after the earlier note my weight moved from 210 to 206 pounds; this morning it read 206.8. Fifth DEXA scan is set for Monday afternoon.

I combine 15 mg tirz, 2 mg reta, and 0.3 mg cagri. That produces roughly a kilogram drop each week. If progress slows I plan to raise reta while easing tirz down a little. I am concerned about gastroparesis if reta keeps climbing without any reduction in tirz.

You are an inspiration. My current plan is 7.5 mg tirz on Fridays and 4 mg reta on Tuesdays, though I worry it may be too much. Eighty-nine pounds are gone with a few more needed to hit goal. I have cagri stored away in case a stall appears near the end.

My weekly amounts are similar: 15 mg tirz, 6 mg reta, 0.5 mg cagri, all split into every-other-day shots. I began after most of the weight was already lost, mainly to stop constant hunger and maintain the results. Earlier use of another GLP-1 produced strong nausea even at low doses, so I switched and later added low-dose reta without stopping the first agent. Cagri was added most recently. Raising either tirz or reta makes skin feelings more unpleasant, and extra cagri brings mild nausea. Another 14 kg have come off, moving from 78 kg down to 64 kg over the past year after beginning at 145 kg. I am near the point where hunger is controlled enough for stable weight with decent food choices, though I would like fewer side effects and might test one more option.

After moving slowly for a year and a half and then stalling for six months I began raising reta, which did little at first. Tirz was added mid-week. Some people respond less strongly. A study noted that stronger negative reactions often paired with greater loss, so I increased until I felt a bit more discomfort without it becoming unbearable. The last two and a half months on 20 mg reta and 6 mg tirz have worked well and loss is now faster than at the start, though for me that still means only a pound weekly. Adding cagri or another agent is under consideration but appetite control is already solid right now.

I am lowering tirz while raising reta and was concerned about going too high. Tirz was at 15 mg but just dropped to 12.5 mg with 2 mg reta added this week. The plan is to reach 12.5 mg tirz and 4 mg reta over the next few weeks. I made a chart to watch total exposure because I did not want to exceed roughly 27 mg of GLP-1s when on 15 mg tirz and preferred an average around 18-19 mg. Others here appear to go well past that. Are there any published studies on safe levels?

If you are still dropping about two pounds weekly there is little reason to add either of those. Losing faster is not desirable anyway. Solid progress though, keep going.

My target is specifically one percent body weight per week. At 206 pounds that points toward roughly 182 pounds. I also keep a broad stock of options I may never use.

Since several people asked, here is how I raised the doses. I followed standard ramp-up charts for each. I actually reduced the cagri later because it suppressed appetite too strongly and daily intake fell below 1000 calories, which is not sustainable. It felt as if the drive to eat had been switched off completely.

No published work exists yet on tirz above 15 mg, reta above 12 mg, or any combinations. One trial is testing higher tirz doses but results are not available. The closest data come from semaglutide studies at 7.2 mg and 16 mg, where extra loss occurred alongside more side effects, especially at the highest level. Scaling that suggests very large multiples of current maxima, yet no entirely new safety signals appeared. Still, gains diminish quickly while side effects rise, so higher amounts may not be worthwhile except possibly in severe obesity cases or true non-responders. More research on those groups would help.
