Update after week six at 2 mg once weekly. 5'9", 52-year-old male, started at 208 lb now at 188 lb, 20 lb down in six weeks. No need to increase yet. Food noise is completely gone even with ice cream, cashews and low-fat mozzarella around. Normally January to April/May I lock into clean eating and drop the same 20 lb then rebound. This time the urge for cheats or rewards is absent and food feels heavy or not worth it. Plan to drop to a micro dose after week eight once I near the 175 lb goal.

On day two after the first 2 mg dose I was at work and suddenly realized I had no need to eat. No thoughts about usual snack times either. Big shift in how food registers.

Third 4 mg dose on Friday hit appetite hard. Need to find something besides beer, which I enjoy, since I've already cut about 80 % of my usual intake. Went out for lunch and was full after two wings and three beers. Supposed to cook salmon and salad but no interest. Also drinking a lot of water which fills me up. Any thoughts on that?

Thanks. I'll probably drop to around 2 mg and do more intermittent water fasting, which is easier on reta, along with sticking to a meal plan.

True tolerance to these meds doesn't really occur according to studies. What feels like tolerance is usually just the result of weight loss itself raising hunger and lowering metabolism. Once weight stabilizes the drug is still working at that dose. Studies show weight drops for about a year then holds steady for years if the dose continues. Slow or fast titration won't change the final stable point.

At 2 mg still no effect so I added another 2 mg after three days. Came from 12.5 mg Mounjaro so that may explain it. Still nothing after the extra dose. Waiting until next week to reassess. Advice from others who switched from high-dose tirzepatide would help. Don't want to go to 6 or 8 mg in the first week.

Why hold back if you came off 12.5 mg Tirz? Multiple smaller shots can build the level without one big hit that brings sides you then have to ride out. I moved from a lower Tirz amount and used 7 mg total across six shots in week one.

Thanks, that helps. I was unsure because so many start at 0.5 mg, though that seems aimed at people without prior GLP-1 use. Going straight to 6-8 mg in week one felt like too much, but you are right that I had been at 12.5 mg Tirz.

When you say no effect, what exactly are you noticing? I agree tiny starting doses are not needed once you already have GLP exposure. The scales are different between the two drugs; my peak on Tirz was 5.6 mg versus 10.9 mg on Reta.

I mean I feel nothing different from before the shot, no nausea or appetite drop at all. My graph shows I am still below peak, which looks to be just over 3 mg. I stopped Tirz and am not stacking. Would like to hear from others who switched from 12.5 or 15 mg Tirz, what starting levels they used and how the first weeks felt.

I began low, adding 0.5 mg weekly while still on 5 mg Tirz, then adjusted up and down. Once Tirz was stopped I settled at 4 mg Reta weekly and dropped 23 lb in a month. I see no reason to go higher soon. At 180 lb with near-normal BMI I now use 1 mg each of Tirz and Reta to stay consistent.

For context I am at 240 lb now, down from 360 lb after losing 120 lb on Tirz up to 12.5 mg. I stopped a month ago and hunger returned strongly. Now on Reta I am trying 4 mg and plan to hold there a few weeks. It seems to take two or three days to kick in for me.

Since Reta does not suppress hunger as much, a small Sema add-on can help. Around 0.5 mg like your last dose worked for me. If hunger or food noise returns that is what I add.

I have stayed in maintenance for ten months at 2 mg weekly. Lowering to 1 mg or spacing to every two weeks did not work as well, so I returned to 2 mg each week and occasionally add Cag to quiet thoughts.

I began on 2 mg weekly from January 11 and have stayed consistent. A plateau happened during a holiday with extra food. Approaching 50 and always skinny-fat, this is my first GLP-1. So far it limits muscle loss well while weight drops. I will soon move to a lower maintenance amount for continued appetite control and body recomp.

It felt like a severe sunburn from scalp to feet plus aches. It took two weeks to fade after I raised the dose too quickly. The feeling has mostly gone now but was unpleasant. Knowing it was short-term and weight was coming off helped me get through it.

I came off 10 mg Tirz because of cost, waited a few weeks, then started Reta and reached 4 mg by week two. That brought strong sides including skin that felt like a second-degree sunburn head to toe plus joint soreness. I am mostly past those now and will move to 6 mg in two weeks, but the skin issue was rough. Slowing down would have been better. Even though both are GLP-1s they feel quite different.

I have been at that dose for about six months. I came from Tirz titrated to 15 mg weekly. Started at 215 lb, hit a high of 215, now at 152 lb. Currently stalled on 15 mg Reta split as 7.5 mg twice weekly and thinking about going higher.

Out of curiosity, why push past 6 mg? That is quicker than most here report, though it matches the trial pace. If lower doses are not producing loss then going higher makes sense.

I switched from 15 mg Tirz. Weight loss stalled on Reta but blood markers improved, slight muscle gain occurred with no fat gain. Started at 2 mg week one, 4 mg for the next five weeks, then 5 mg for three weeks so far. Next pin is the fourth at 5 mg. With a 30 mg vial I am considering 7.5 mg next since no sides appeared. Appetite suppression began in the third week at 5 mg. Based on how it feels, 7.5-8 mg seems comparable to 12.5-15 mg Tirz for suppression strength, though that is not a precise way to compare. I still need to lose 14-16 kg to reach goal.

Third shot was yesterday at 1.5 mg. First two were 1 mg then 1.25 mg, all on Sunday evenings. Only notable side was nausea on the first Monday.

I will stay at 4 mg for at least two more weeks. This morning after waking, using the bathroom, the scale showed a three-pound drop from last week. The earlier reading was higher because I had coffee, water, and clothes on. It looks like a steady three pounds per week. Total loss since January 6 is now 22 lb.

It seems to affect people differently. Mine stays around 72 bpm, which is normal for me. Sleeping readings range from 54 to 74 bpm.

Started November 30 2025 at 1 mg with no noticeable change, possibly from impatience. Moved to 2 mg for three weeks, then 2.5 mg for two weeks, 3 mg for five weeks, and 4 mg in February with no plan to go higher. The reason for the last step was hearing that full effects begin at 4 mg. I intend to remain here for a long time. Age 55, started at 275 lb, now 224 lb. Recent labs through the VA show all markers improved; previously cholesterol and A1C were off and I had refused medication. This has made a real difference.

Weeks 1-5 at 2 mg, weeks 6-7 at 1 mg to check hunger signals and loss rate (still steady but slower with more hunger), weeks 8-9 back at 2 mg. A couple pounds from goal so will drop to 1 mg for two weeks, then 0.5 mg for two, then 0.25 mg for two. After that I may test 0.1 mg once or twice weekly to finish the vial and ease any hunger. Reference: 17 lb loss in nine weeks with muscle sparing and even some gain.

After six months on Tirz up to 6 mg weekly I took a few weeks off for digestion to normalize. Then started Reta at 2 mg week one, 3 mg week two. Will probably hold at 3 mg another week or two before going to 4 mg for a month, then possibly 6 mg depending on tolerance.

In the second week of the journey I added a low dose of sema four days after the reta shot. Feeling very good at the moment and still have 15 kilos left to lose.

Thirty-seven days in and holding at 2 mg with a lot of weight lost. Eating and drinking feel a bit harder but I am managing. Reta really does change your life because the binge-eating disorder is gone.
