Cagrilintide and retatrutide combination therapy enhances weight loss and metabolic outcomes in obese male rats

9 replies6 peopleSep 16, 2026☆ Follow
Summary

Does adding cagrilintide to retatrutide improve results over retatrutide alone in weight loss and metabolism, according to a rat study and user reports?

A rat study showed the cagri plus reta combo outperformed reta monotherapy and other pairings on weight loss and metabolic markers. Forum users report similar benefits in practice, with some stacking cagri during plateaus or maintenance while others prefer alternatives due to side effects. Experiences vary widely, from effective appetite control to fatigue that forces dose adjustments or discontinuation. Marketing considerations may limit official combined products even if the pairing appears potent.

A new paper in Nature reports that adding cagrilintide to retatrutide produced greater weight reduction and better metabolic markers in obese male rats than retatrutide by itself or similar combinations with semaglutide or tirzepatide. The outcome lines up with what people have already seen in practice, so it backs the idea that retatrutide users might gain from adding cagrilintide or eloralintide. It would be useful to see how the combo stacks up against a modestly higher retatrutide dose alone.

Lilly probably has little incentive to pair eloralintide with retatrutide. A 3 mg eloralintide plus 5 mg tirzepatide regimen already delivered 17 percent weight loss in 16 weeks, which beats retatrutide results so far. They risk cutting into sales of their existing drugs, so the more likely approach is marketing eloralintide with tirzepatide for plateaus and non-responders rather than launching a standalone combo that would rival retatrutide.

The domain combining those two names is still unregistered.

I asked an AI to project how retatrutide alone might perform at the 16-week mark against the eloralintide plus tirzepatide numbers. The projections are rough because no published retatrutide data exists at exactly 16 weeks, yet they still show the strength of the eloralintide plus tirzepatide approach. The amount of weight lost can feel excessive when it happens that quickly. My own use of eloralintide plus retatrutide is aimed at maintenance rather than further loss, so my targets differ from those chasing the largest possible drop.

When I reached goal weight and switched to maintenance I lowered tirzepatide from 15 mg to 10 mg within two weeks and added 0.125 mg cagrilintide. The next week I stopped tirzepatide entirely, started 1 mg retatrutide with 0.25 mg cagrilintide, and used the cagrilintide only if hunger became too noticeable. After a few weeks I raised retatrutide to 3 mg. I later removed cagrilintide because I lost another 15 lb beyond what I wanted. I have stayed on 3 mg retatrutide once weekly and weight has remained stable since. Cagrilintide can supply extra help when higher doses of the other drugs alone are not enough to finish reaching goal.

I combine cagrilintide with both tirzepatide and retatrutide and the results have been strong. I take 1 mg cagrilintide each week and have had no side effects. I followed the recommended slow titration schedule. It is worth testing if you are stuck or expect to stall.

Cagrilintide itself does not bother me and many people respond well to it. I tried it for two weeks and ran into a bad reaction, which is why I have been looking at eloralintide instead. That compound has not caused me any side effects.

I injected 0.25 mg cagrilintide on Monday and the fatigue has been overwhelming. I can barely function, cannot train, and just want to sleep all day. Cagrilintide is definitely potent.

Do you think the sleepiness came from the cagrilintide? I would not mind some extra drowsiness myself since I am currently on retatrutide only.

That level of tiredness sounds rough, and it makes you wonder what an accidental higher dose would feel like. Smaller vial sizes from manufacturers would help avoid mistakes. I tolerated 250 to 300 mcg while increasing retatrutide but have since dropped cagrilintide and now use 4 mg retatrutide as my upper limit as I move into maintenance.
