Well.. i've seen the cautionary tales about dosage

23 replies13 peopleJun 27, 2026☆ Follow
Summary

What are real user experiences starting low-dose cagrilintide stacked with reta or tirz, including side effects and appetite impact?

Users report highly variable responses to cagrilintide at 100-500mcg doses when added to existing GLP-1 regimens. Some notice little extra appetite control and titrate up slowly, while others experience severe GI distress from even modest jumps in dose. A few report headaches or minimal added weight loss benefit, and several emphasize that individual tolerance differs widely with no universal sweet spot.

Whether cagrilintide provides meaningful extra benefit beyond reta or tirz for most users, or if it is often underwhelming or too punishing.

Starting out with cagrilintide at 0.1 milligrams, laughing about it. Getting hungrier on retatrutide lately, so curious to test what this adds. Planning to track blood sugar closely using my CGM because it tends to drop glucose levels.

I have some cagrilintide on hand for later use, and that starting amount lines up with how I picture beginning it myself.

AmberCinder18archiveJun 27

Same here, I have some ready but have not tried it. Will be curious to hear how your run goes.

PlainThistle36archiveJun 27

Yes. Good that you did the reading. If you are already on something like retatrutide you might want to ease back your current amount a little. A quarter to half milligram would be plenty. One hundred micrograms is a solid first dose for cagrilintide. You can always add more after a few days, but you cannot take back too much at once and will just have to wait it out.

BlueAlder28archiveJun 27

Maybe the cagrilintide I have is weak. I had already dropped about forty pounds on tirzepatide, going from three milligrams every six days up to ten over four months. Then I stalled. After four weeks I added five hundred micrograms of cagrilintide every six days on top of the tirz and one milligram of reta, all staggered. Eight weeks later I had only lost another three or four pounds. Raised cagrilintide to one milligram and reta to two, which gave seven pounds before another stall. Later raised everything again to tirz twelve, cagrilintide two, reta four. Finally seeing some movement again. Almost eight months in I am about ten pounds from my goal under thirty BMI, starting from exactly forty. It seems like a useful tool but not the game changer some describe.

After three days I have not noticed any extra appetite control. Hunger feels unchanged so I will probably increase the dose instead of waiting a full week.

I would increase very gradually. Plenty of peptides let you step from one hundred to two hundred fifty to five hundred to a thousand fairly fast, but cagrilintide is not one of them. It is harsh and does not forgive mistakes. I began at two hundred micrograms without issue, then foolishly went straight to five hundred and had the worst stomach trouble of my life, short of actual food poisoning from a bad fair corndog. Two full days of vomiting and diarrhea plus another day of bad nausea, though I did drop six pounds. Later I tried the five hundred microgram dose again after lowering my reta and got identical results.

Thanks, I will take it slow. Might just add another hundred micrograms now and move to two hundred next week.

CopperPebble51archiveJun 30

From my own run, point two five milligrams did nothing noticeable. Moving to point five made it impossible to eat much at all. Complete difference.

QuietBramble77archiveJun 30

I have some but have not used it. A couple years back I tried some from a questionable domestic source and got terrible constipation. Still unsure if it was even real cagrilintide. My current batch seems better but I am still hesitant to start it.

There are certain individuals who deserve two strikes right to the face.

I am seeing a broad spread of reactions, so it clearly hits people differently. A few even appear to be non-responders. I took another hundred micrograms yesterday and will watch how it goes. I have a reta split dose due today but am considering shifting it to make any cagrilintide effect clearer.

BrightCompass30archiveJul 8

Thought I would give an update even if it is not very helpful. I did the two times one hundred micrograms but then caught some bug that caused stomach issues anyway, so appetite was gone regardless. Probably not from the cagrilintide, but I will test again and find out if it was. Next time I will likely go straight to two hundred fifty micrograms.

I am hoping to begin my cagrilintide tomorrow. Planning to start at two hundred fifty micrograms. I have never had sides before and am a slow responder to semaglutide and tirzepatide, so I expect the same here. Fingers crossed.

BrightCompass30archiveAug 1

I tossed my first vial because it was old and opened a fresh one. Went straight to two hundred fifty micrograms and did not really feel it. Then I wondered if I should bother at all. I want less food noise but looking at my actual loss on reta alone at six milligrams, I am dropping about a pound a week, which seems like a solid steady rate. So I will probably just waste this vial too, since I am still losing without pushing further just to quiet the noise.

IronKettle95archiveAug 1

Someone pointed out recently that these medications lower food noise but some people keep chasing more peptides to wipe it out completely. A little noise around dinner time is normal. I looked into cagrilintide and a couple others just in case, but I am still losing without changing my tirzepatide or reta amounts lately.

BrightCompass30archiveAug 7

Took five hundred micrograms spread over a couple days this week and also had headaches on a couple of those days. I do not usually get them, so it stood out. Today one was bad enough to keep me in bed all day. Looked it up and yes, several people mention headaches with cagrilintide, so I think my experiments with it are finished.

SlowTimber22archiveAug 7

The GLP-1s will handle their part, but you still have to handle yours. They simply make it easier. Lots of people are hunting for the one injection that solves everything.

Wait, there is no magic pin? Aaaagggghhhh!

Very much this. I tell people the medication just gives me room to make better choices.

I read another forum full of new users. This idea is news to all of them and to the ones stuck in five-month stalls. Others jump in to say they are not eating enough or getting enough protein. Weighing food, tracking calories, or going for walks never occurred to some. These drugs are not a shortcut. They are tools for people willing to do the work to manage their obesity.

I have a friend who just started GLP-1s after watching what they did for me. Three weeks in she says she thinks it is working, yet she never weighed herself at the start, does not know her current weight, and is still eating out every night. It is like she is just waiting for the drug to melt everything off. Sigh.
