Sudden side effects after 3 months of Mounjaro?

20 replies7 peopleOct 18, 2026☆ Follow
Summary

Sudden upper abdominal pressure, burping and fullness feeling after three months on steady 5 mg Mounjaro dose, is this normal or should pancreas/gallbladder be checked?

Most replies treat the symptoms as common, fluctuating GI effects that can appear even after months on the same dose. Over-the-counter options like pantoprazole, simethicone or Pepto are suggested first, with advice to skip the next dose or see a doctor if pain increases or fever appears. Recent blood results were shared and one user listed specific pancreatic and cholestasis markers that could be requested along with ultrasound if needed.

Hey everyone, three months on Mounjaro at 5 mg and the weight loss has been good, down from 112 kg to 95 kg since mid-July. I’ve also ignored most of the usual warnings and lost too fast while eating and drinking too little. For the last few days I’ve had pressure in my upper abdomen, felt slightly nauseous yesterday, and now my stomach feels full even though I’m hungry. I’m burping a lot and quite strongly. Is this just a normal side effect or should I see a doctor to check the pancreas or gallbladder? My blood work from early September was fine but didn’t include those organs even though I asked.

Did you start a new vial recently? Is this from a doctor or elsewhere, and are you sure the dose was right? I ask because it sounds like you might have more medication in your system than usual, similar to what many people feel in week one.

If there’s actual pain then yes, see the doctor. What you describe sounds like ordinary GI issues that can appear out of nowhere. Try daily omeprazole for a week and see if it improves.

I’ve been on 5 mg for a few weeks already. It’s a real Mounjaro pen from my doctor.

Even on the same dose the medication can build up and cause GI sides that come and go. I get painfully bloated sometimes but it passes before the next meal. My usual remedies are a PPI for heartburn, Pepto for several stomach issues, Alka-Seltzer for indigestion, simethicone for bloating and pressure, or ondansetron if nausea is bad. I just skip the next dose until things settle.

I’m also on 5 mg. Some days I feel nothing, other days I’m full all day and burping constantly.

WarmQuill69archive9h

I still have plenty of pantoprazole left, so I’ll try that.

None of us can give medical advice, but if it’s bad enough it makes sense to see the doctor before worrying about tests. GI symptoms are common and often fade with time. If the discomfort is more than minor gas or comes with fever, the doctor is worth a visit.

WarmQuill69archive8h

It’s not really pain, more like discomfort, constant burping that sometimes brings up a little liquid, and a pressure feeling.

Is the feeling there the whole week or only right after the injection?

If the doctor is worried they might order an abdominal ultrasound.

WarmQuill69archive7h

It started only in the last few days. Injection day is Sunday but the symptoms were already present before today’s shot. I did drink alcohol on Friday and overate quite a bit for the first time in months, plus our child was sick and vomiting on Thursday, though the symptoms seemed to begin before that.

To be safe you could ask for pancreatic markers like lipase and amylase, cholestasis markers like GGT, ALP and direct bilirubin, plus an abdominal ultrasound.

WarmQuill69archive7h

My GGT was 26 (lab limit under 64), ALP 31 (limit under 50), direct bilirubin 0.52 and indirect 0.93. Bilirubin has always run high for me, even as a child, and the doctor wasn’t concerned.

WarmQuill69archive7h

I’m still on the real prescribed pens, but today was my first grey version. The sides started before that shot. It came from a well-known source that had testing done for purity and was filtered.

SharpCinder28archive7h

Even with testing, one vial from a large batch doesn’t guarantee every vial is the same strength. That could explain stronger sides at first. Differences in the active ingredient quality between the brand and other sources might also play a role early on.

WarmQuill69archive6h

The sides were already building for several days before today’s shot, so the timing was just coincidence.

When you see the doctor again, ask for a prescription for ondansetron so you have it on hand for nausea.

SharpCinder28archive6h

Splitting the weekly dose into two smaller injections can reduce sides for some people. I’ve noticed the sides return when I go back to a single weekly shot, so I’m planning to split again until I’m on a lower dose.

How large are those batches usually? Even if several vials are tested together, you still wouldn’t know the exact fill in each of your own vials, right? Thanks for the tips.

I don’t know the exact size, but pictures from factories suggest batches can be thousands of vials. Even then it’s still a small sample of the total, so variability can slip through. Other factors besides fill volume might also make some batches feel different.
