I used tirz from the pharmacy starting a year ago and stopped about eight weeks back. I'll soon have more on the way since I had reached 15 mg doses earlier. My idea is to begin at 5 mg for the first two weeks to check tolerance, then move up to 7.5 mg and 10 mg monthly if needed. Does that sound reasonable or do I need to begin below 5 mg?

Using 2.5 mg twice a week as a restart dose is less likely to trigger bad side effects than 5 mg once a week. This also lets you make small increases more frequently, for example moving from 2.5 mg to 3.5 mg twice a week after a week or two, but any side effects mean you should pause or slow the increases until they settle. Once you reach the total weekly amount you want you can gradually shift one half of the doses upward while lowering the other half until you reach single weekly injections.

You might handle getting back on tirz fine, but start low at 2.5 mg for at least three weeks. As others have said, many people find tirz works noticeably less well the second time after a break. If it feels like it is doing nothing you should stop and try a different medication, which can sometimes restart progress. I helped a friend who paused for four months by using two months of low-dose reta first, then switching back to tirz; she lost another twelve pounds and stayed at a lower dose than before.
