Higher dose for higher weight

38 replies18 peopleApr 25, 2026β˜† Follow
Summary

Should I raise my Tirz dose while stacking Reta if hunger and food noise return after a couple weeks on 5 mg despite good diet and more weight still to lose?

The thread shows a user stalled with returning hunger on low-dose Tirz plus Reta at week 7, asking whether to titrate higher given substantial remaining weight loss. Most replies say yes, bump or split the dose because diet is already tight and hunger signals the current amount is insufficient; receptor burnout is dismissed as a myth at normal doses. Some note once-weekly may allow resensitization while others prefer more frequent dosing for steady suppression. Diet tweaks are suggested first but ruled out here.

Stuck for at least two or three weeks now and still dealing with solid hunger. I moved up faster than the usual schedule but I've seen that people with more to lose often need the higher amounts. Already dropped sixty pounds on my own and twenty on Tirz, with another seventy-five left. Stacking a little Reta on top at five milligrams Tirz and one milligram Reta. Would pushing the dose higher be a bad move? Only on week seven and the effect is nothing like the first month.

SharpTimber40archiveApr 25

Still have decent slowing of stomach emptying but the hunger is definitely back. Also got one vial of Cagri on the way to test that too.

LevelThistle44archiveApr 25

Keep it simple and just raise the Tirz. The main job of Tirz is cutting intake by lowering hunger and food noise, and you still have plenty of room to go up.

I get nervous about the idea of burning out receptors and losing the effect later, but I'm already not feeling much so my body seems to be saying go up. Still dropping a tiny bit from exercise and forcing a deficit, but it's a struggle. I'll increase and see if that helps.

GreenMeadow54archiveApr 25

The five-to-one Tirz to Reta ratio looks reasonable. A two-week stall is normal so I wouldn't change just for that. You're still on a low dose so going up makes sense, but first check if your diet is really tight. Getting enough protein at about a gram per pound and keeping the deficit around five to seven hundred calories? If hunger is there and those basics aren't locked in, fix the food side before the meds.

Nutrition is already locked down. Lower carb, whole foods, no liquid calories, right around seventeen hundred a day when maintenance is twenty-three hundred, and plenty of lean protein. Still losing a little but holding the deficit is tough. The Tirz seems to wear off after a couple days and then the food noise and hunger come roaring back.

Haven't seen any solid reports of people blowing out receptors even at twenty-plus milligrams a week or when stacking with Sema for extra GLP exposure. I started splitting my dose in week three on Tirz, carried that over when I switched to Reta, and the return of food noise on days four or five is exactly why I split.

QuietSignal61archiveApr 25

I hear you. Hunger is brutal. Remember you can always add more but you can't take less once it's in. Appetite suppression took two weeks to really kick in for me. Started at half a milligram Reta and kept going up to two milligrams a week, then switched to one milligram every three days. Now at two milligrams every three days and thinking about adding Cagri or Tirz. Don't dose just once a week. Split it and alternate the two meds. Give it time but don't hesitate to go higher if suppression isn't there. Sometimes I add a small touch-up dose in between.

GreenMeadow54archiveApr 25

Diet sounds solid so it might be time to raise the dose or split it. The case against splitting is that steady levels could speed tolerance compared with the peaks and valleys of weekly dosing. It's theoretical but plausible. I'd lean toward a small bump over splitting, though both are reasonable.

LevelThistle44archiveApr 25

In the trials weight loss scaled with dose but with diminishing returns at the top end, mostly limited by side effects when they appear. Receptors aren't a real concern if you're carrying a lot of extra weight.

I tried splitting but actually got better suppression spacing it every five or six days instead of every three. Hoping the added Reta will help more.

Big un is funny. Can't help it, I'm like a dog, totally food motivated.

Heard a doctor on a podcast say once-weekly is better because it lets the body clear some peptide and resensitize receptors, which could let you stay on lower doses longer. Can't recall which doctor but it sounded logical. Have you noticed any difference between weekly and every three or four days?

LevelThistle44archiveApr 25
↳ replying to @SharpTimber40

Food noise was my main issue, rarely true hunger but always ready to eat. My goal is to lose about a third of my starting weight on GLP or close to forty percent of my highest weight from 2018. Only twenty-four percent of people on twelve milligrams weekly Reta hit thirty-five percent or more loss, so I'm at sixteen milligrams a week to improve the odds.

I've heard the same advice. Not sure if it's true, doesn't sound right to me. Never gone more than five days without dosing because the hunger gets too strong. I love eating. The other day I took my two milligrams early to try to skip some brownies I made and still ended up sick and ate a huge one plus more later. Probably need Cagri.

The idea of GLP-1s causing receptor desensitization is a misconception at normal doses. They work systemically and don't target one narrow set of receptors the way some peptides do, so titrate up slowly until it starts working again.

ClearThistle78archiveApr 25

Yeah, that sounds like Instagram influencers mixing up Reta cycles with testosterone cycles. Those people are already lean and chasing shredded looks. Most of us just want a healthy weight and the studies used longer continuous time frames, not cycling.

At that low Reta dose you might not be getting much glucagon effect. I'm at eight milligrams and thinking about adding some Tirz or Sema for hunger control. Reta alone doesn't curb my appetite well even though fat loss is still slow. Maybe swap the order of your Tirz and Reta stack.

LevelThistle44archiveApr 25
↳ replying to @ClearThistle78

Glucagon receptor activity looks dose dependent. My approach has been to push Reta up as fast as sides allow, otherwise it's basically just Tirz without the extra benefit. That assumes the goal is dropping a lot of weight.

ClearThistle78archiveApr 25
↳ replying to @LevelThistle44

Completely agree. I'm no doctor, just another poster, but it feels like the Reta at that dose is doing almost nothing.

Don't know enough about your situation to give advice, but seventeen hundred calories would be too high for me to lose much regardless of food quality. I've been aiming for twelve hundred with high protein and haven't stalled. Food noise is basically gone for me on five milligrams Tirz so some days I don't even hit twelve hundred. Added one milligram Reta mid-week to move toward Reta for maintenance and drop Tirz to a micro dose. Sixty-five pounds down, sixty-one-year-old female, on Tirz since December first.

SharpTimber40archiveApr 25
↳ replying to @WarmWillow50

I weigh two-sixty so twelve hundred would be way too low. Don't want to lose muscle. That would be over a thousand-calorie deficit for me. Congrats on the loss though.

Week seven is still early. GLP-1s are tools to support lifestyle changes, not magic. After moving to seven point five at week nine I saw better weight loss.

CopperSignal27archiveApr 26

With sixty-plus then another seventy-five pounds to lose you're almost certainly going to need the standard max doses eventually, fifteen milligrams Tirz or the matching amount in a stack, maybe higher. No need to rush. The main reason not to increase is side effects, and you haven't mentioned any. Hunger itself shows the current dose isn't causing much GI trouble so you can probably go up safely. Massive loss on low doses usually isn't realistic. Also, having already lost sixty pounds changes things. It lowers energy expenditure and raises hunger, so the dose that holds the new weight may not be enough to keep losing.

SlowSparrow81archiveApr 26

Yeah, yeah, I get it. πŸ™‚ What I believe in is solid research data πŸ™‚ I should look into this further. I understand that everyday intuition doesn't always match up with facts, yet this caught my attention and I wanted to see how it lines up with what others here have gone through.

I trust that least of all. Hard scientific evidence put me in the hospital for a week after the second Pfizer shot. No thanks. I'll stick with circumstantial anecdotes from overconfident forum users.

Sorry to hear you ended up in the hospital. Hope you're good now. The shot worked fine for me. I like science because it separates emotions and anecdotes from actual statistics. It's not perfect but it's the best we have, and it teaches me never to be overconfident.

New here, but what does food noise actually mean? I've seen it mentioned a few times.

IronWillow59archiveApr 27
↳ replying to @QuietKettle56

I think it means the background hunger or cravings that show up between meals when you don't want to be thinking about food.

GreenMeadow54archiveApr 27

To me it's the constant background thought of what can I eat or when can I eat again. Thinking about dinner while still eating lunch. It's not real hunger, just the urge to eat for the sake of eating. Not loud, just always there.

KeenHarbour58archiveApr 27
↳ replying to @SharpTimber40

I see that too. For me dosing every five days feels ideal. Though this one has a bit longer half-life so it might build up to a higher steady level. But that's nothing major. Studies begin participants at two milligrams. I dose it every five days together with my other one and figure I'll shift to every six days when it starts replacing the other more.

SlowHarbour21archiveApr 27
↳ replying to @SharpTimber40 (opening post)

In my case whenever progress stops and appetite signals return I tack on fifteen more minutes of movement while raising the dose. I change my meals too during those stretches. My regular eating habits are already decent yet I move over to fish with whole grain and greens at midday and evening throughout the week. It turns rather unappealing after a while though personally it succeeds.

SteadyQuill55archiveApr 27

This idea would look great as my ending phrase.

QuietKettle56archiveApr 28

Okay that makes sense. I had a lot of that in the first three or four months on Tirz but it's pretty much gone now.
