With all the buzz around the latest Reta trial data, more folks are thinking about moving from Tirz over to Reta. I've seen plenty of notes on stacking or switching at lower or middle doses, but not much from people who were already at the top end of Tirz, 15 mg or more per week, then tapered off while climbing to higher Reta amounts. If you've done that, I'd like to hear how it went and what results you got.

I'm at a high dose, 8 mg Reta, which I consider high. Came off 11 mg Tirz because of sides and a stall. I don't combine with other GLP meds since they act on the same receptor and might raise pancreatitis risk. I did add Cagri while increasing, but at 8 mg I'm very suppressed and content.

When moving from 11 mg Tirz, did you begin right at 8 mg Reta or start lower? I've been at 15 mg Tirz for almost a year and hit a complete stall while maintaining. Still have 30 lb to lose. My tolerance is high, so I'm wondering if jumping to a high Reta dose would be a problem. Right now I'm doing 15 mg Tirz on Saturdays plus 2.5 mg Reta on Wednesdays and notice nothing from the small Reta add-on.

Without tracking blood levels over time you might miss how the compounds overlap. Because of long half-lives you're still stacking unless you wait a full 4-5 weeks. If sides track with peak levels, longer gaps help you spot problems before the next dose. Different half-lives and receptor responses mean non-linear effects. The safest route is waiting five weeks then following standard titration. What's your comfort level with risk?

I didn't start at 8 mg. I began Reta at 2 mg while lowering Tirz, then raised Reta by 1 mg each week since I had no sides. I wouldn't suggest doing it that fast because levels can build up. Be cautious. I kept reducing Tirz the whole time.

I'm also on 15 mg Tirz and want to do the same switch. I was thinking 10 mg Tirz plus 2 mg Reta for four weeks, then 7.5/4, then 5/6, then Reta only at 8 mg. Still not sure. Probably won't have the Reta for a couple weeks.

Looking at the plotter, a sharp taper seems necessary. At 12.5 mg Tirz I'd take that same dose the first week, add Reta four days later, drop to 7.5 mg Tirz for weeks two through four, then 2.5 mg for weeks five and six, and stop Tirz after week six. That keeps overlapping levels reasonable.

The plotter tool is useful. I ran numbers up to 12 mg Reta but might stay at 8 mg if things go well.

I was on 15 mg Tirz every five days. I moved straight to 10 mg Reta with no problems and dose it every six or seven days. I don't get the sharp drop I saw around day five on Tirz. Bodies differ, so stepping up is fine if it feels safer, but after more than a year on GLP-1 I don't think you shock the system or need to restart titration from scratch. I've stayed at 10 mg.

I'm impressed. Most people taper slowly or start at the trial beginner dose. I stacked for a while and reached an 8 mg Tirz plus 4-5 mg Reta split. Would like to hear from anyone else who went straight from high Tirz to high Reta.

I moved from 15 mg Tirz to Reta, beginning at 2 mg and increasing 2 mg every four weeks while lowering Tirz. I'm now at 12 mg Reta. I was still losing on 15 mg Tirz but wanted to try Reta since it's viewed as the strongest option with solid study data even though not approved yet.

It stayed high until around 6 mg, then started to help, and at 8 mg it really worked. The delay probably cost me two months of progress.

Just adding to the earlier reply. I tried something similar, 12.5 mg Tirz straight to 8 mg Reta. Same experience as you: week three in, only a little gas the day after the shot, nothing major. I had stacked earlier but never got Reta that high. Safest is still starting at 2 mg and titrating, but if high-dose Tirz was fine for you then beginning at 4 mg or more probably won't cause harm.

This thread got me to try Reta. I was tired of pinning multiple compounds every six days and still stalling. I was about to add Cagri or raise another dose but worried about new sides. Planning to go to 10 mg every six days. Started with a 5 mg vial after my last 15 mg dose three days earlier. If that goes well I'll try 10 mg four days later.

I'm also very interested. Currently on 12.5 mg Tirz, down 60 lb with another 100 lb to go, but stalled for a few months. Wondering if switching to Reta is worth it to break the plateau and whether I can handle the dosing change, since many say results on Reta are disappointing when coming from high Tirz.

I took my last Tirz on Sunday, first 5 mg Reta on Wednesday with no sides, and plan 10 mg on Sunday. Still deciding between six- or seven-day spacing.

I was on high Tirz and always cold, plus stalled, so I started very low Reta and slowly lowered Tirz while raising Reta. Went off Tirz completely but added a little back when arthritis came back. Now stack 2.5 mg Tirz with 8 or 10 mg Reta plus low-dose Cagri to finish the last stubborn pounds.

Yes. I was dosing 5 mg every two days, alternating with another compound after a cycle of that compound alone, and noticed that raising to 7 mg brought back sweating, which seemed positive.

I did it. Tirz for a year and a month at the top dose. Hunger was frustrating at first and made me wonder if the new med was working. I chose to stack instead. I like how Tirz kills noise and even enjoy its nausea sides, but after working through titration Reta feels stronger. GI sides on Reta are mainly hunger and constipation rather than nausea. I crave sweets and salt instead of total food repulsion. I can drink more water. No daily exhaustion like on Tirz, but sleep is harder. Still early, only three weeks in, and the scale hasn't moved yet.

I'm also a high-dose Tirz veteran. I tolerated it fine up to 7.5 mg; after that the first three days after each shot felt awful. Couldn't eat or drink without nausea. My wife does great on 15 mg. Before trying Reta I added CJC-1295/Ipa twice daily plus Tesamorelin in the evening to target visceral fat; those raised appetite but it wasn't a big issue. When I started Reta I pinned 2 mg four days after my Tirz shot. Just finished week four, titrating up: two weeks at 2 mg, two weeks at 4 mg, now moving to 6 mg. No negative effects from Reta and I'm much more comfortable on the lower Tirz dose. Came down from 12.5 mg.

Started on the first GLP-1 a few years ago, titrated to max, then switched to Tirz when it became available. Insurance issues forced a return to the first one, then back to Tirz. Stayed at 15 mg for two years with no further change and weight hovering. Switched to Reta late November, titrated up while lowering Tirz from 15 mg to 2.5 mg and Reta from 2 mg to 8 mg. Switched to 5-6 day spacing and weight finally started dropping with food noise gone and very low appetite. Down 20 lb so far, averaging 3 lb per week. Also found an adrenal tumor that had been raising cortisol for years.

I messed up transferring a single vial so ended up closer to 9 mg instead of 10 mg today. Still had strong suppression three days after the first 5 mg dose, but today less so. Was on a high combined weekly Tirz amount plus another compound and stalled, now switched cold turkey to high-dose Reta. First week without Tirz in over a year. The plotter suggests I should have pinned a day earlier, but I didn't account for the other compound's glucagon action.

Seven days after the first 5 mg Reta dose and three days after the 9-plus mg dose: no sides, strong suppression, but no real weight loss even though eating less. Will take 10 mg on Saturday, six days after the last dose.

I'll be raising Reta to 6 mg this week, my fifth dose. Took a full 12.5 mg Tirz five weeks ago when starting Reta, then dropped it. This week at about 3.25 mg Tirz and moving to 6 mg Reta. The lower Tirz amount is just because the vial concentration makes measuring tiny volumes tricky.

Moving from low-dose Tirz (2.5 mg twice a week) to Reta. Yesterday took 2 mg Tirz, today 2 mg Reta. Previous Tirz dose was six days earlier. Using split dosing on Reta based on any sides and prior experience, aiming for trough levels above 3 mg within the usual therapeutic range of 4-6 mg per week.

Haven't seen you posting in a while. Been in maintenance since May. I don't think my experience fits exactly because I didn't go to high-dose Reta, but I didn't wash out and went off Tirz cold turkey. Was on Tirz almost exactly one year, maxed at 15 mg for five months. Last 20 lb came at 15 mg. In the final weeks I dropped to 10 mg and added a small amount of Cagri, then stopped Tirz the next week and started 1 mg Reta with 0.25 mg Cagri. Still at goal on 3 mg Reta alone after dropping Cagri. Inadvertently lost another 15 lb during maintenance and stopped the stack to prevent further loss.

I was on Tirz close to two years before switching to Reta, at 15 mg for many months. Now on Reta for a few months and getting the same effects at only 2 mg per week. Things are going great.

Lost 110 lb on sema since mid 2023 then stalled even at 3 mg last summer. Now at 1.25 mg and still stalled. Started stacking 2 mg Reta a couple weeks ago. Had very few sides on sema except constipation. Debating how fast to increase Reta while still on sema without too many sides.

Update: took the second 9 mg Reta dose today, third Reta dose ever. Settled on 9 mg every six days. So far I love it. Can see and feel it doing something positive, whereas the previous high combined regimen wasn't doing much anymore. After the Tirz stall, both of the other two compounds only helped briefly. Will Reta keep working longer? Time will tell.

For the last few days my weight has stopped moving even though I'm still eating less than I burn and staying hydrated while balancing my salts. I think this might just be my system adjusting after dropping twenty two pounds across the previous month and a half.

The scale finally dropped suddenly today after seeming stuck, so I'm at a new record low since starting these meds. Both of the other two compounds did the same thing, only to rebound after three weeks.
