Summary

what happens to people after years on GLP-1 medications like tirzepatide or semaglutide

Long-term users report that the medication becomes a routine weekly habit with no major side effects after the first months. Most describe a lasting mental shift around food, reduced hunger, and lower interest in alcohol or overeating, though they still rely on the drug to maintain weight and other benefits. Many plan to stay on it indefinitely, viewing it like lifelong treatment for a chronic condition, while a few experiment with dose reductions or maintenance schedules.

What this discussion establishesWhere people disagree

whether the drug can eventually be stopped without weight regain or if lifetime use is required

I’ve been looking into lots of GLP-1 stories recently and most focus on the early months, the numbers lost, and any side effects. What interests me more are the long-term users. After a year or two, does it just feel ordinary? Do you still depend on it, or has your food relationship shifted for good? Experiences vary, but I’d appreciate input from those who’ve used it longer.

IronFenwick84archiveJul 27

That's an interesting query. I'll keep an eye on this discussion for insights from veteran participants. My time with this began back in March, and so far the outcomes please me.

Several days back I chatted with my mom regarding this topic. She began the medication at eighty years old, three and a half years earlier, dropping from one hundred ninety pounds to one hundred twenty five. Her dose stayed under two milligrams throughout, with changes happening gradually. These days she uses a upkeep amount of one milligram each week. She makes sure not to have any treats or processed snacks in the house since she would just dispose of them. At times like family gatherings she may feel the urge to consume extra but is unable to do so. The medication helps her stay at this lower weight and she worries about gaining back what she lost before. Everyone in the family is happy with the results.

I'd guess she's way more nimble these days.

Yeah I figure these meds will be a permanent part of my routine. That setup feels fine since the positives seem plentiful while negatives are almost nonexistent. Still the idea of building up resistance and having to keep raising amounts bothers me some. What used to be my reduction level has become the everyday amount and it sits fairly high now. I've hit the ceiling on one at 2.4 milligrams each Monday plus 4 milligrams of the other every Friday. Research on the first one goes up near seven milligrams these days. Curious how things will go during upcoming gaining and reducing phases.

I started this treatment aware it would last indefinitely, much like taking medication every day for a thyroid condition or to manage blood sugar. Half a year into the process, the routine already feels normal. The weekly injection blends into my schedule, and my phone planner simply flags the day it is due. The adjustments to how I eat are now second nature, and I notice the difference every single day. My outlook on meals has shifted in a big way. Not long ago at the grocery store, I spotted a small pack of three deli chicken strips in the cooler section. Before this treatment my first reaction would have been that the cost seemed high for a quick bite. These days the thought that pops up is how the same pack could stretch across multiple full meals instead. The change in how I relate to food feels permanent, and I have no desire to return to the old patterns.

QuietFenwick11archiveJul 27

The routine feels dull. After injecting I consume whatever I like but I get satisfied sooner. Junk food never posed a problem for me previously so skipping it now comes naturally. There are no adverse reactions even after extended use and that is positive. I am maintaining my weight yet I still shed a bit of body fat gradually with the unchanged amount. My intake of alcoholic beverages has decreased significantly which stands out. Previously I averaged around one and a half per week but now I can go long periods without any. I am not opposed it simply does not cross my mind. It is good that I remain able to enjoy one during social outings.

CopperLedger37archiveJul 27

Studies indicate these treatments stay potent across extended periods. It's unclear if the pattern holds once much more time has passed.

Life becomes like this.

NorthMeadow96archiveJul 27

It's been a full year since I began this medication. My weekly amounts never went above seven point five milligrams. I hit my target weight not long ago and began lowering the dose. Last week I reduced it to four milligrams. At first I thought about spreading out the time between shots but instead settled on cutting back the amount each week. The aim is to reach two point five milligrams weekly while avoiding the return of earlier problems such as sore joints, swelling, trouble with blood sugar control, high cholesterol, raised blood pressure, or losing the ease of skipping meals, smaller portions, and no urges for snacks. Long term use of this class of treatment feels acceptable to me. It seems like a better option than depending on cholesterol or blood pressure drugs for life.

I work as a doctor and have observed individuals taking these drugs. Quite a few have been on them longer than five years, particularly those with diabetes. The majority seem satisfied and manage fine. A number have lowered their HbA1C levels to 4.5. Patients using the tablet versions also keep their weight steady along with overall metabolic health. They are usually free to consume what they want yet gradually develop self-control.

BrightMeadow44archiveJul 27

This has gone on for two years in my case. I dropped around seventy pounds during the initial eight months or so, then it leveled off afterward. My typical weight has risen by roughly five to ten pounds from the lowest point ever, yet it remains in a favorable range. I consume whatever I desire, merely in smaller quantities. Throughout this period I've stayed on five milligrams of tirz, meaning if a concerning increase appears I still have significant room to adjust the amount.

PatientMeadow76archiveJul 27

A pair who makes videos online has used this medication longer than two years, maybe three. Their updates appear often. It looks like just another weekly habit for them, similar to taking any prescription. They rely on the mixed version and picking out certain details from their posts can offer some useful bits. For my part it's been close to ten months and now feels ordinary, though the loss stage still continues with roughly ten to fifteen pounds remaining before holding steady.

SteadyLedger49archiveJul 27

For more than two years now these GLP-1 medications have become a regular thing in my routine. Missing a shot leaves me extremely famished. Weight continues to drop at a gradual pace while my main aim has shifted toward gaining muscle mass. I began at four hundred forty five pounds and have reached two hundred through various habit adjustments. In seven days I plan to begin using testosterone to observe its impact on my objectives. At present I'm taking twelve milligrams of triz along with four of reta, transitioning fully to reta once the triz prescription runs out after a few months.

PlainBramble29archiveJul 27

I've been using this treatment for two years now and shed thirty-six percent of my body weight. After that it quit functioning even at the highest monthly amount. Starting in May I've been gradually moving to different options and aim to drop fifty more pounds. My overall well-being has gotten much better with issues like thyroid joint inflammation and organ function. I intend to continue this approach indefinitely since I've dealt with excess weight for forty-eight years. Looking forward to newer compounds that have fewer unwanted effects.

It's wild to look back on this. I've been taking the medication for around sixteen months total. Began at a low dose of two milligrams, raised it to nine, and then settled at six. You never notice the old eating volume until it changes, and that wasn't a positive thing. The difference shows up clearly between rest days and work ones, with just a few thousand steps compared to many more during shifts. Food intake rises on busier days, yet without nonstop excess. It started for nighttime breathing problems, though the added upsides confirm this will continue long term.

During the initial seven months of steady reduction I followed a very disciplined eating plan without any indulgences. I reached my target after seven months by dropping sixty-two pounds which equaled twenty-seven percent of my starting weight. That achievement happened more than twelve months back specifically during April two thousand twenty-five. For the past twenty-three months total on this medication my approach to meals has changed completely. I consume whatever appeals to me at any time yet portions are much smaller now. Food no longer dominates my thoughts the way it does for a pet that responds only to treats. Obsession has faded though I still appreciate meals. Unwanted effects have disappeared too such as tiredness and irregular bowel movements. Only after an especially heavy dinner do I notice some discomfort. My dose sits at fifteen milligrams and has remained there for forty-seven weeks. Body weight fluctuates just one pound either direction lately. I quit tracking daily intake numbers more than twelve months in the past.

That's intriguing. Reta, I've reached my target so now I'm planning to cut back to six milligrams after beginning at ten. Earlier I used nine milligrams across four weeks. What's your method for holding steady at fifteen milligrams? You're on Tirz after all. I've come across conflicting accounts on upkeep, some reduce while others stay put.

Some folks mentioned they kept dropping weight so they changed their amount. Hard to figure out why my results have stayed level. I dropped another four pounds while still on that middle amount but I also began the hormone therapy around then. My approach to the therapy stayed cautious aiming only high enough to ease the symptoms. I went past that target and reached a thousand on the reading when the other number hit 2.4. That change probably supported body recomposition and combined with lifting it may have added a couple pounds. I have dialed it back to 840 and 1.7 now. I stayed extremely disciplined during the drop phase but I have slipped since. I still avoid large portions yet I treat myself freely. From what I gathered staying on the top amount of the medication could bring extra perks for swelling and other upsides. I plan to hold at the highest level until the scale moves again and only shift if something changes. The handle is just a joke since I have barely seen any snow. Warm months out on the water feel nicer though because I do not get drenched in sweat from the southern sun anymore.

Appreciate the input. Got some lighter dumbbells here that I'll begin using. Planning to book a visit to the specialist for various concerns and mention TRT during that. My eldest son encourages starting that since he's using it and could provide some. I'll experiment with the doses on the scale. Still dealing with belly fat though my total weight is fine, but it adds up.

My worry was that my metabolic rate might slow down, prompting me to include some strength work while keeping protein intake solid. Since I'm older now, getting the approach right mattered to me. To steer clear of harm I steer away from heavy loads and rely on machines rather than free weights. My home setup stays pretty basic, and I generally stick with moderate resistance levels.

Maybe you're aware or maybe not, since I don't notice anything unusual. It slipped my mind that an elastic strap remains fastened around the handle on a sliding door, plus several extras handed over for recovery exercises at home following shoulder surgery. A couple of ten-pound weights have also been sitting unused for quite some time.

Wow, August marks three years since I began this medication and the time has passed faster than expected. Overall I've dropped roughly seventy pounds and stayed at my goal for over six months. Coverage ended about eighteen months back when I had already shed thirty pounds and permanently dropped nicotine too. A plateau had set in by then. Once access stopped I regained twenty pounds in short order. After that I switched to the other compound and kept raising the amount until reaching my target. That point required twenty milligrams every five days. For ongoing use I dropped to twelve milligrams weekly, later ten, and now eight milligrams each week. Side effects never appeared. I turned into a dedicated gym regular and genuinely enjoy the sessions. My metabolic rate picked up along with steady energy levels. My meals stay largely nutritious and plentiful. Nearly all the prior joint discomfort has vanished and I no longer need blood-pressure tablets or metformin. Injection day lands on Thursday and I still get excited for it. The treatment has improved my daily life so much that I treat the weekly dose like a small occasion. I plan to keep going indefinitely unless the ongoing benefits for stroke, heart-attack, and dementia prevention ever feel tiresome.

QuietKettle82archiveJul 28

Appreciate all the input from folks here. Hearing various stories has been interesting. Hoping to get insights from those who've been at it longer.

Right after spotting it, that upbeat tune about sunny days and sweets popped straight into my thoughts. Yeah, existence really turns out exactly like that! 😄

ClearWillow59archiveJul 28

Began back in May of 2025. Dropped 55 pounds which is 29 percent of my starting weight. I'm sort of holding steady now. Went from 188 down to 133. All the readings on my smart scale look fine including bone density. But I did lose quite a bit of muscle mass. So now the issue is adding muscle without putting on extra pounds. I'm taking 1.2mg of tirz every four days along with .8mg of Reta on a staggered schedule two days apart. I reduced the Reta dose after noticing a stronger response from a fresh vial. Probably that one was filled more than normal so I'm unsure of the exact dose I'm actually using. Not sure if the Reta is still useful since I'm not aiming to shed more weight anymore. Though I notice I get a boost in energy from it compared to the other one. Maybe I'll keep going this way indefinitely.

LevelWillow41archiveJul 28

After using it for nearly seven months, I still find myself depending on this stuff. Skipping even one leaves me feeling increasingly starved, and that mix of pleasure and fear is intense.

SteadyPebble24archiveJul 28

Began using this medication back in late 2023. Over the following eighteen months I dropped sixty-five pounds and my weight has stayed steady since then for over twelve months. During the weight loss phase I reached twelve milligrams but currently I'm at ten milligrams weekly. Skipping a dose or reducing it brings back intense hunger. The downsides for me include trouble sleeping, occasional faster heartbeat, and a dry mouth that hits hardest overnight. I saw information about a study on using lower doses or placebo for keeping the weight off where folks regained on the placebo. For me this stuff gives an edge by increasing calorie burn even when inactive and maintaining that internal heat. Prior to this no matter how much I restricted calories fasted tried low-carb or liquid diets the scale never moved down only up. The issue wasn't eating too much or bad decisions it stemmed from resistance to insulin along with my genes. I recall meals with thin pals where they had bread big helpings of carbs and sweets afterward while I stuck to a protein-topped salad and just water without any dessert yet I was still the heaviest there. This medication genuinely turned my life around. I'm wishing for an improved version of these drugs that has less unwanted effects when maintaining. I've built up a supply that would last eight years and continue to acquire more which makes sense because my food expenses are way down now. Earlier similar drugs gave me terrible reactions so I'm holding off on trying another one for the time being. I really long for restful nights again.

SteadyMeadow73archiveJul 28

I've decided to stick with this for half a decade before pausing to check if my body forgets the extra weight. Even though I'm aware that going off and back on carries some dangers, my current amount stays small. At four point five milligrams I run into a slight problem keeping intake below maintenance level, whereas five milligrams leaves me unable to eat enough food daily. Thirty three weeks out of the past thirty eight have been at five milligrams and I see no reason to increase it. I'm hopeful everything turns out fine, but if it doesn't I'll resume right away.
