What tipped the scale for you in your decision to stack your GLP with another or add in an amylin?

43 replies22 peopleAug 4, 2026☆ Follow
Summary

Why stack another GLP-1 or amylin instead of just titrating up the current dose?

People added reta, cagri, tesa or survo mainly to manage returning food noise, add fat-burning effects, or avoid side effects from higher doses of their base GLP. Most kept the original dose lower and introduced the second agent gradually, reporting better appetite control or energy without full titration. A few noted unique issues like steroid use or persistent dry eye that prompted the change. Results varied by individual response and timing of the added compound.

Hello everyone. I'm reaching a decision point soon and have several solid choices. I'd like to know the reasons behind the paths others took. Specifically, for those who stayed on a non-max dose of their GLP-1 and added another GLP or amylin instead of titrating up, what led to that choice? Which considerations played a role? (I already searched the forum; sorry if this has been covered.)

PatientTimber12archiveAug 4

For me it started with tirz once a week. Around the five-day mark when the half-life was ending I got really hungry, so I split the dose into two shots a week to keep food noise down. Then I decided to try swapping one of those shots to reta instead of tirz to see if the glucagon effect would help as well. I like how tirz is working, which is why I haven't switched over completely.

AmberTimber37archiveAug 4

I added 2 mg per day of tesa to 4-5 mg per week of reta for belly reduction and it worked well. I've finished the tesa and will stop unless the current 6 mg weekly reta dose stops controlling the belly. My situation may be different because of high-dose prednisone and hydrocortisone for disease management; those corticosteroids are known to cause weight gain and a large belly similar to Addison's.

GreyLedger65archiveAug 4

I'm in maintenance on 7.5 mg tirz for six months. One persistent side effect is an extremely dry right eye. Dropping to 6 mg helped the eye but felt insufficient for maintenance, so I went back up. Recently I added cagri and lowered the tirz to see if that relieves the dry eye. It's only been two weeks and I'm adjusting both very slowly, so I don't know yet if it will work.

SteadySparrow75archiveAug 4

This weekend I added 2 mg of reta. I was on 10 mg tirz split 5/5 per week. I'm close to goal and wanted to try reta to see what it does. I expect less appetite suppression and the fat-burning third component. I'll see if I can shift dosing from tirz-heavy to reta-heavy. Plus I had some reta on hand and wanted to experiment.

How interesting! Any resting heart-rate increases or notable feelings from the 2 mg?

SlowAnchor19archiveAug 4
↳ replying to @GreyLedger65

How much cagri did you add, and any sides?

I feel like my weekly dose is like a security blanket at this point, so I'd be nervous to change that, but how are you finding the lower tirz dose with the added reta? How much reta did you add?

RustKettle57archiveAug 4

I had been on 7 mg reta for a while and was happy with weight loss, but appetite suppression and food noise were very strong. I added 2.5 mg tirz as a cheaper way to calm food noise and appetite, and it's been great. It hasn't stopped me from getting calories in, but the small addition has nearly eliminated food noise.

When I added reta I was doing two 5 mg shots of tirz. I swapped one to reta starting low at 1 mg, so one 5 mg tirz shot and one 1 mg reta shot. That wasn't enough, so after a few weeks I raised it to 2 mg. Now I'm at 7.5 mg tirz and 2 mg reta, which seems like a good spot so far.

SharpMarble35archiveAug 4

I experienced unusual reactions while using tirzepatide, so I decided to switch over hoping the positives would stay but the negatives would vanish. It's only been a short time yet things look good up to now, keeping my hopes up.

RustBramble70archiveAug 4

I've been on 2.5 mg reta twice a week for a few months. Food noise disappeared right away and I lost 40 lb quickly. It's been slowly returning lately, though mainly for protein-rich foods, and I'm worried hunger pangs could lead back to other cravings. Some tirz is on the way so I'll add it slowly to see how it affects cravings. I'm also ten weeks into 2 mg tesa daily hoping it would speed up visceral-fat loss, but results haven't been dramatic.

My food noise is coming back, not severe yet but I don't want it to get worse. Thanks for your input.

SteadySparrow75archiveAug 4
↳ replying to @SlowAnchor19

None so far. If anything, resting heart rate and blood pressure were a bit lower than normal. The only new feelings were a little more burping and a slight throat sensation almost like acid reflux, but without the actual reflux.

SlowAnchor19archiveAug 4

Yes, I know the feeling very well. Thanks for your input.

ClearWillow59archiveAug 4

I was happy on tirz at 2.25 mg but weight loss eventually slowed to nothing. I read about tirz fatigue and anhedonia at higher doses and saw some people stacking reta instead; they seemed happier, so I tried that rather than going up. I started very low at 0.5 mg and added more slowly. The only side effects were occasional skin crawling or sensitive spots that went away. It worked well for weight loss, and the tirz still keeps food noise away.

RustLantern55archiveAug 4

I tried adding 1 mg reta while dropping tirz from 10 mg to 7.5 mg. I was in an autoimmune flare and feeling really bad overall, and it didn't go well, so I'm back to tirz alone at 5 mg every four days or so. The main issue was severe fatigue, though I couldn't tell if it was from the tirz, the flare, or both. On the 5 mg doses I have normal energy and can eat again, which is a win. I'm still losing about 2-3 lb a month and nearly at goal.

IronKettle95archiveAug 5

One caution: I've read others say reta doesn't suppress food noise as well. If you need or want that suppression most, stick with tirz. I take 7.5 mg tirz on Friday and 4 mg reta on Tuesday as a mid-week booster. I added reta because I didn't want to titrate tirz higher and I like its fat-burning qualities. As I near goal it looks like I won't need more tirz and will probably stay steady on reta.

IronLantern24archiveAug 5
↳ replying to @SlowAnchor19

This is where I'm at right now. I'll give it a couple more weeks before deciding whether to add tirz. I split my reta every 3.5 days, so I'm still figuring out the timing for a second GLP shot. I might titrate up instead; I haven't decided.

SlowAnchor19archiveAug 5

I'm looking forward to learning about the choices made by others. Appreciate the contributions provided earlier. Taking a ten milligram dose of tirz works well for me. Hearing explanations for combining with reta or cag, along with their dosing schedules, seems worthwhile. Grateful for the replies received. Much more progress needed on weight loss, and the shared knowledge helps a lot.

GreyLedger65archiveAug 5
↳ replying to @SlowAnchor19

I did 0.25 mg the first week, then 0.375 mg the second week. No side effects at these low doses so far. I notice increased appetite suppression about 48 hours after dosing. I'm taking it three days after my tirz dose, so I know the peak isn't from the tirz.

PatientHarbour61archiveAug 5

I had been maintaining on 12 mg reta, but after starting a steroid cycle that became unworkable and I was eating well over target on many days. I raised reta to 24 mg, which balanced the steroids enough to maintain weight, but slow loss was still hard. After a couple months I dropped back to 12 mg and added an ancillary I thought would help. That has worked better so far and I've been able to cut weight since the change.

Can we actually see visceral fat? I thought it was hidden inside, hugging the organs but not visible, like a pouchy belly.

PatientPebble79archiveAug 5
↳ replying to @SlowAnchor19 (opening post)

Decisions feel driven by emotions over logic for me. Using a small amount of tirz starting in December. Reaching a standstill led me to introduce reta gradually rather than raising the tirz amount, for no clear reason. That made a difference. There have been weeks without any tirz. Earlier today the remaining contents gave 2mg tirz along with 2.5mg reta. Nearly three weeks had passed from the previous injection. Weight isn't coming off currently, perhaps from insufficient food intake. Years spent attempting smaller portions mean I can't consume anything without feeling hungry. This creates an issue : /

QuietCinder90archiveAug 5
↳ replying to @AmberTimber37

Did you have any side effects from combining those two?

QuietCinder90archiveAug 5
↳ replying to @PatientPebble79

No, we can't see it. It's deep inside hugging the organs and causing problems. Your subcutaneous fat is what you can grab and pinch.

IronKettle95archiveAug 5
↳ replying to @PatientPebble79

I have a scale that weighs daily and produces graphs I review. I'm about 30 lb from goal and still losing. The graphs show my visceral fat in the normal range; I still have some subcutaneous work to do. I attribute that to the 4 mg weekly reta shot, though I have no facts to back it up.

PlainAlder11archiveAug 5

The stuff is weird. I checked my heart rate and blood pressure this morning; heart rate was 73 but will likely rise when I get moving, blood pressure was 128/78.

SlowAnchor19archiveAug 5
↳ replying to @PlainAlder11

Still within normal limits. I don't want to use all my options too early, but I'm nervous about reaching max tirz dose too soon either.

RustBramble70archiveAug 5
↳ replying to @PatientPebble79

Yes, it can be quite visible depending on how much we're carrying. Subcutaneous fat is what you can pinch on the belly, love handles, etc. Visceral fat is the hard protruding belly some people have, though some look slim yet still have it. My subq fat went quickly, leaving sagging skin, but I still have the belly. My smart scale says I have an unacceptable amount of visceral fat, though I'm told not to rely on it for accuracy. All I have for evidence is the mirror reflection.

AmberTimber37archiveAug 5
↳ replying to @QuietCinder90

I didn't notice anything major, only some small effects. There was slight fluid buildup while I was using it. That cleared up once I finished the round. At the start, there was a noticeable sensation of it taking effect, sort of a pins and needles vibe. Right after the shot, within minutes, I got an unusual non-productive cough along with a peculiar sensation in my nasal passages. When I queried an AI about it, the response indicated that similar experiences had been mentioned by others. These symptoms didn't persist long. After that, it was all good.

ClearWillow59archiveAug 6

I underwent an exact measurement procedure and then contrasted the data from my digital scale with it to pinpoint its successes and shortcomings.

PatientHarbour61archiveAug 6
↳ replying to @ClearWillow59

I'm less optimistic about DEXA accuracy. I've seen results showing very low body-fat percentages after major weight loss that don't match visual presentation, and I'm one of them. A recent scan put me at 7.2 % body fat at 206 lb. I should look like a bodybuilder with loose skin, yet I look like someone who could still lose belly fat. My plan is to drop another 35 lb or until I look like a jacked shar-pei; if DEXA is struggling, most of the loss will register as lean mass.

WryCompass33archiveAug 6
↳ replying to @PatientHarbour61

Did you slowly work up from 12 mg to 24 mg of reta? I'm at 10 mg and will have to bump up soon. That's a long way from 24 mg, but I seem to need to max out on GLPs for them to have any beneficial effect.

PlainAlder11archiveAug 7
↳ replying to @SlowAnchor19

I checked my heart rate and blood pressure yesterday afternoon to compare. The reading showed 101 over 68 for pressure and 68 beats per minute. That seems fine to me. I'll check again in the evening. Injection day is today. This compound is quite fascinating. Reached my target with 10 milligrams several weeks back, then reduced to 6 milligrams over two weeks and 8 milligrams for one week. Weight has stayed consistent with changes of one to two pounds weekly. I feared continued loss. The idea is that staying at one amount might lead to a flat point. This explains increasing the amount after no progress at a level. Not sure really. Just an idea from my small mind.

WryCompass33archiveAug 8
↳ replying to @PlainAlder11

I seem to respond differently to GLPs. I've never worried about losing too much or too quickly. I'm not a strong responder and worry whether I'll lose at any dose. I don't have much left to lose, and some might question why I'm taking anything. Reta/tirz has at least helped me mostly kick a binge problem; I still slip occasionally but nothing like before.

WarmAnchor49archiveAug 8

I just completed my third year on this. Losing the 110 pounds took me two years. Maintenance has been going for a year. I'm still using 10 mg Tirz, 2 mg Sema, and 2 mg Cagri, injected once a week or sometimes after eight days. Appetite control is something I keep pursuing, and the combo does the job for me. Constipation used to bother me but has gone away on its own, with no changes made. Feeling cold was a problem before adding Cagri, but now it helps keep me warm.

PatientHarbour61archiveAug 14
↳ replying to @WryCompass33

I started with twelve during the first week then went to eighteen the week after that and twenty four after. Side effects have never shown up for me at all and staying on twelve for quite a while meant the step up didn't concern me much. Bigger amounts don't appear to add nearly as much as the smaller starting levels did.

LevelTimber64archiveAug 15

Last year around the start of a trip I felt awful for a whole day after my third tiny amount of Cagri. It could have been unrelated but I quit anyway. Later I added Survo which has worked out well and brought me to my target. With a fresh target in mind I'm now getting set to raise the Survo amount following this pattern of Tz ten every six days paired with Survo one point five every six days but shifted three days apart.

GreenMeadow27archiveAug 15
↳ replying to @LevelTimber64

Around a month back I started mixing survo in with my tirz. At first I kept the days lined up without any stagger, but lately I've been thinking about shifting to alternate days over the coming weeks. Together they land with real force for me, and I could use an extra lift later in the week.

LevelTimber64archiveAug 15
↳ replying to @GreenMeadow27

Exactly why I offset. Plus I have a daily pin and didn't want three in a single day. I might be a bit cautious; I sort of helicopter-parent myself on some things.

IronCompass33archiveAug 15
↳ replying to @SlowAnchor19 (opening post)

Back in February I began with 2.5 milligrams of Tirz. By May my progress had stopped. Even though that medication left me with barely any desire to eat, the numbers on the scale refused to drop. I avoided raising the dose further because I was nearing total dislike of food. Plenty of users here noted that Reta leaves hunger mostly untouched, so I tried adding a small one-milligram amount of it partway through the week. After doing so the weight began to drop once more. Early in August hunger returned, which made me raise the Reta amount to 1.5 milligrams.

GreyFenwick31archiveAug 15

Funny, I've noticed dry eyes too and never had that before; I didn't even think it could be from tirz.
