380lbs -> 265lbs (without intervention from peptides)

Summary

Can you lose a large amount of weight through diet and exercise alone before starting peptides, and how do others view that approach versus relying on meds from the start?

One user lost from 416 lbs to 265 lbs in a year via calorie deficit and workouts, regained some weight, then began reta with strong appetite suppression. Others share parallel stories of natural loss followed by stalls or regain, praising the habit-building first and noting GLPs help with maintenance and food noise. Several emphasize that learning sustainable behaviors beforehand makes later progress more durable, while a few argue meds address root metabolic issues that willpower alone cannot fix long-term.

What this discussion establishesWhere people disagree

Whether weight loss without medication is sustainable for those with endocrine or metabolic issues, or if GLPs are required lifelong.

Still open

How much additional muscle-building weight might offset scale progress while on reta, and exact long-term outcomes after years on the drug.

Nothing here is advice.

Hello everyone. I wanted to post my before-and-after photos from losing weight without peptides. My heaviest was 416 pounds and I felt awful. I was so embarrassed that I never took any pictures at that point. Through eating in a deficit and training three or four days a week I reached 265 pounds after a year and felt really good. Then I put roughly forty pounds back on and now sit around 299. The after shots are from my lowest point. I recently started reta and it has basically removed my food thoughts and hunger. Still plenty left to do but I feel hopeful. Sharing this to stay motivated and keep moving forward.

Solid effort. It is a long race and reta helps, so keep posting updates on how it goes.

PlainAlder11archiveJun 4

That is great. Two quick questions: how tall are you and what dose of reta are you on right now?

Hell yeah. I enjoy these stories of people who built the habits through training and consistent choices instead of leaning only on medication. I went from 200 down to 160 over three years the same way. Your path was longer but I connect with it. You will be in a stronger spot because you figured out the habits first. Too many start the meds, drop weight, never change their routines, and then regain everything once they stop. You did it the other way around and that sets you up better.

Thanks. I am 5 foot 4 and began at 1 mg two weeks ago. No bad sides so far, which I was worried about. The drop in food thoughts is wild though. I realized today I had not eaten until 1 pm because I was busy with schoolwork and never felt hungry. That part might be tricky for me. I am also trying to add muscle so I need to keep protein intake high.

Thanks a lot. I still sometimes see my old 400-pound self when I look in the mirror. After being heavy my whole life and talking with a therapist I realized I had never really given myself a chance. If someone compliments me I immediately say no, or if asked what I am good at I answer nothing. I try to be kind to others but this hit me hard. I cannot hate myself into keeping the weight off for good. I am working on treating myself better too and accepting that I deserve it.

Nice work. Adding reta should make things even smoother. Getting diet and training dialed in is the hardest part and you already have that. I also lost a lot without meds, just through effort and workouts, though I was probably unpleasant to be around at times. Then I plateaued despite eating very little. I was insulin resistant and had other issues. Starting tirzepatide made the weight come off faster and without the misery. I am close to goal now and plan to move to maintenance. Health markers improved a lot. You should feel proud.

Keep in mind that adding muscle can slow the scale. Give each dose four weeks so your body adjusts. I am at 8 mg with almost no sides except feeling full. Keep going.

I am 68 and have lost nearly 200 pounds five or six times. Doing that damaged my heart and now I am dealing with the results. It is not about willpower or calories. Diets work until they stop working and you regain, which harms your body. That is why these GLP-1 meds are such a relief; they treat the underlying chronic problem. They are meant to be taken for life. If your metabolism is off there is no diet that will fix it forever. Glad you found them.

Great job. I went from 310 down to about 265 just by walking when a job change forced it.

Congratulations. I have been heavy most of my life, reached normal weight several times, then regained. Keeping it off long term is the real challenge you have already seen. Staying on something like reta can help with that so you only have to lose once. Studies so far show the loss holds for at least four years. I dropped from 145 kg to 75 kg without meds and held it for a year but it was difficult. Later the meds changed my hunger, fullness, and cravings in a big way.

Good progress. Respect for doing that much on your own. The meds can shorten the remaining road. Some people call it cheating. I tell them to give up their car and walk everywhere or eat without utensils. A few people I know went too far with not eating and some women lost hair in clumps. One thing worth checking is that your body can only pull so many calories from fat each day; run the numbers for yourself.

You can do this. Gather as much information as you can. I reached 318 and am now at 272. Not huge but I am aiming for steady progress and keeping negative thoughts out. I believe we end up where we are for a reason and there is something to learn. I am not an expert but I care about people and want the best for them. We are all trying our best. Stay motivated.

AmberCompass48archiveJun 7

Nice work. The peptides should make it much easier to get where you want to be.

SharpTimber40archiveJun 7

Reaching 265 naturally in a year is huge and you should feel really proud. You already put in the hard work so hopefully the rest will come easier now.

RustBeacon82archiveJun 7

Wow, that is impressive progress without any help. Excellent work, looking forward to seeing the next milestones. Keep going.

Stay positive. We are all on our own versions of this journey. You have taken control and that is great.

Yeah right. I have spent decades dieting, losing, and regaining. I even kept it off for four years once. After 45 years of that cycle I will take the drugs and not look back. These are a gift.

AmberAlder67archiveJun 7

Great job. Keep the effort and the positive attitude going and you will hit your goal. Make it your top priority and do not let anything stop you.
