My schedule so far has been half a milligram, then one, one point five, and now two. Appetite effects only showed up in the fourth week. My BMI sits in the low thirties and I weigh two hundred ten pounds at five foot seven. Because two milligrams is not doing much yet, should week five go to three or four? I know responses vary. I lift four or five times a week and my food choices are cleaner. Would it be better to hold at two for three more weeks? Also, what does the right dose actually feel like?

Go up more gradually. Plenty of people do not give the medication enough time before deciding it is not working.

What advantage does slower titration give beyond keeping sides down for larger jumps? With the other two compounds you titrate to keep appetite control steady, but here the main action is through glucagon receptors and that scales with dose.

I moved straight to four after four weeks at two. In hindsight I regret it. Resting heart rate was already a bit higher at two, but at four my sleeping rate hit eighty and eighty-two the last two nights. If you are not already logging resting and sleeping heart rate, collect a couple weeks of numbers before going to four.

I understand waiting is important. Still, when I see reports of ten or twenty pounds gone in a month it feels like I must be doing something wrong. I am already eating well and training, so I do not want to stall at a level that seems inactive.

I am going by the company trials. They ramp slowly to limit sides. I do not fully get the point of the half and one milligram steps, but I have not looked into it deeply. Every trial I saw started at two, though that information is from December.

Ten pounds a month works out to two and a half pounds a week. I am dropping one to two pounds weekly while on six milligrams. Results differ between people, so do what feels right for you.

You have only now reached the starting level used in the company study. No reason to wait more than two or three weeks before going up. Slow and low is not always the best approach; sometimes it underuses the medication.

My take is that if weight is still coming off at least half a pound a week, stay put. Slow progress adds up. Only move up half to one milligram after four weeks with no loss. The goal is the lowest dose that works and is tolerable. Responses vary widely; someone I know dropped eighty pounds in nine months on just two point five.

If I recall correctly the drug reaches steady effect around five weeks. Jumping to four means adding more while the earlier amount has not yet peaked. That is why most people hold each level for a full month. At only four weeks on two there is no need to move yet. Another week at two, then a few at two point five, then three. This is a long process. When the dose is right you will simply know.

It is striking how differently it hits people. I began at one and held it five weeks. The first week food was unappealing and I lived on protein shakes, dropping twenty pounds with little visible muscle loss. I am also on one hundred milligrams of HRT. The last two weeks I went to one point five. Because of the half life the amount in the body keeps rising even if the weekly shot stays the same. That is why the advice is to hold four weeks and only increase if sides are manageable. I am convinced the design allows hunger to return so you can refuel for the glucagon effect, so hunger alone is not a good gauge. Also drink plenty of water because water weight shifts the scale.

Whether to increase depends on side effects, rate of weight loss, and hunger. If hunger makes it hard to stick to the plan and sides are minimal, going up makes sense. If sides like nausea are already present at two, move slower or hold. If loss is acceptable and hunger is controlled, stay. Your current level will not reach its highest point for nearly four more weeks, so remember you have really only had one full week at two after the earlier steps. Starting BMI around thirty four means higher doses will probably be needed later, but moving after only one week at two raises the chance of sides. I would not go straight to four after one week at two. Three might be reasonable if hunger is strong and sides are light.

I know it is not the usual way, but week one I used one, week two two, week three three, and week four the vial ran out so I did one point five. When the new kit came I went back to three for week five. It feels like I may be getting used to it because the last dose produced even less effect than before. Maybe the new kit is weaker. Should I go to four this week?

A sensible rule is to keep the same amount for four weeks and move slowly. Just because the trials started at two and stepped by twos does not mean everyone must copy that.

I do not know the mass. The first vials were ten milligrams, the current ones are twenty.

Here is the schedule I used and decided to remain at five. Week one half, two one, three one point five, four two, five two again, six two point five, seven three, eight three again, nine three point five, ten four, eleven four again, twelve four point five, thirteen and fourteen four point five, fifteen and sixteen five.

I have lost thirteen pounds since starting. I take a prescription medication called Gabapentin for restless leg. The medication causes weight gain so that may explain why my loss is smaller than some others report.

It looks like no further effects have appeared yet though they might still show later on. Digestive troubles only began appearing for me after a full year at the lowest level. Dropping the amount felt fine once those started.
