I'm new to this so sorry if the question seems basic. Given Tirz half-life, should shots be spaced every five days or seven? Once it builds up, does the gap still matter much? What has everyone noticed in practice?

Five days works best for me. On seven days the hunger comes back too strongly, and splitting into two shots per week has not helped as much.

Starting with once a week is simplest because it is easy to keep track of. Most people do not notice hunger getting worse by the end of the week, but if that happens then more frequent dosing can help.

Same thing happening here. I only started Tirz last week after trying Sema which did not do much for me. I have been debating the five-day option because the food noise is strong. I am going to try five days.

Weekly works for me too. No problems and it is simpler to remember. I have lost 115 lbs so far pinning once a week the same way the trials did.

I would say start weekly and get used to any tougher days on six and seven. Those coping skills will be needed later anyway. I wish I had practiced handling harder days while doses were still low.

I have used seven days for over a year. I moved up only when food noise stayed around. Right now I am at 10 mg and have held there more than six months.

Since you are only in week two, I would wait until week four before raising the dose. It is better to let your body get used to it first. I felt nothing the first month on the lowest dose yet still lost 11 lbs, so it was working even without obvious changes.

Are these Tirz vials or pharmacy Mounjaro? The 2.5 mg start is meant to let your system adjust and check for side effects. After four weeks, if it is tolerated well and hunger control could be better, moving to 5 mg is an option.

Almost four months on Tirz at 10 mg, always weekly until now. I am about to test six days because days five through seven have been rough. I want to stay at 10 mg longer but need to see if shortening the interval helps.

Four weeks on Tirz so far. I began at 2.5 mg once a week every Thursday and added 1 mg Tesa five days a week. At week five I plan to go to 5 mg Tirz weekly while keeping Tesa the same for another four or five weeks, then reassess before going higher.

Yes, every five days starting at 2.5 mg. After four weeks with no loss I usually suggest moving to 5 mg. The right dose is the one that produces steady loss; more is not automatically better.

I stick to every seven days. Feeling some hunger on the last couple of days is not a problem. I prefer learning to manage hunger rather than trying to remove it completely, and even with those signals you still should not be able to overeat.

You are right that managing hunger matters. Overeating has not been an issue this week for my husband or me. We have both seen a real shift there, especially him since he has always had a big appetite.

I tried splitting after thinking about it and because of a short supply issue. At 7.5 mg weekly I switched to 5 mg twice a week for two weeks. I usually get hungry a day or two before the weekly shot, but the split actually gave weaker overall suppression so I went back to single weekly doses and progress picked up again. Everyone is different, but the lesson for me was not to change what is already working. Managing the natural ups and downs also seems useful for maintenance later.

So I raised it to 5 mg and now I feel it for about four days. That is a decent start. These are Tirz vials.

This is just my own experience. I do half the dose twice a week exactly 3.5 days apart, Tuesday morning and Friday evening. At the moment that is 3.5 mg per shot after a little over two months. The five-day half-life keeps levels steadier and side effects have mostly gone away. Twice-weekly use has not been studied as much, so unknown risks could exist.

When increasing the dose, if hunger returns mid-week or side effects appear at the level needed for steady 1-2 lb weekly loss, splitting can make sense. Weekly is mainly for convenience. I stay weekly but add Survo on a staggered schedule to keep food noise lower mid-week. While ramping I increased 20 percent each week dosing every three days so I could drop back if sides showed up. A blood level tracker can help visualize it.

All of that. If I were not on brand-name prescription I would have tried every five days for an extra month before going up in dose, then returned to weekly at the higher amount.
