How do you know when you have lost enough weight?

26 replies20 peopleJun 6, 2026☆ Follow
Summary

How to decide when to stop losing weight on GLP-1 medications and shift to maintenance dosing?

The thread shows people reaching low weights via semaglutide or tirzepatide then questioning whether BMI targets still make sense once muscle, skin, and visible definition enter the picture. Most advise moving away from scale numbers toward mirror checks, DEXA scans, waist-hip ratio, blood pressure, and functional fitness. Several report that bodies settle naturally after dose and calorie adjustments, while loose skin improves slowly over months. A few note incidental findings like hernias or cysts once fat is gone.

What this discussion establishesWhere people disagree

Some insist on continuing until a low BMI or ripped look is achieved while others argue self-image goals can become unhealthy and that health markers plus partner input should set the limit.

Still open

Whether another 20-plus pounds will be mostly fat or will cost muscle, and what final maintenance dose works best for each person.

Nothing here is advice.

Peak was 237, fell to 224 without medication and stayed near there for years. Began Wegovy March 2025, stayed on it nearly twelve months until coverage ended, reached 204. Started compounded Tirzepatide mid-March and hit 175.8 today. Partner thinks it is time to quit, yet BMI below 25 would require 164 and a preferred 23 would mean 151. Hoping to achieve visible muscle tone for the first time, lifting five days a week. Cannot picture where another 24 pounds would come off, just as the earlier 60 seemed impossible. Currently at 10 mg weekly. Should the dose drop to 5 mg now or stay until the target is met? Curious how others have handled lowering the amount for upkeep.

Given how near the calculated BMI goal already is, a DEXA scan could give clearer targets than BMI alone before choosing maintenance steps.

Will look into how and where to get one, thanks.

Can you already make out a six-pack on the stomach?

AmberMarble84archiveJun 6

BMI loses value once muscle has been added. Health and future weight control improve with that muscle, yet the number on the chart ends up higher than it would without it. Around age 50 the body feels different from ten years earlier even at similar weights. After losing a quarter of peak weight there is more muscle but also more looseness because skin has changed. BMI sits near 21 now.

Abs are starting to show, though some loose skin remains and I hope it tightens further.

Skin can keep improving for a year or more; age and other variables affect how much.

Same age range here. Everything feels different than in the twenties or thirties. BMI is convenient but not something to lean on heavily. The DEXA scan is next to see what the numbers actually show.

It's wild catching sight of those stomach muscles standing out and then running my hands over how solid they feel.

GreyAlder39archiveJun 6

The only target is how you see yourself, which may not be the healthiest approach; speaking with a professional could help. A partner already accepts the current look. Pushing further might stir unexpected reactions. Weight goals are better handled as a joint decision. BMI becomes less meaningful with added muscle and remains a population statistic rather than an individual one. Strong fitness levels are possible at BMI 25, as seen in many athletes. Better markers include being able to swim steadily for an hour or completing a run test without major discomfort.

SharpLantern71archiveJun 6

In the sixties, the target weight was adjusted downward four times. Once the final number was reached the dose was lowered and calories increased using a TDEE estimate plus light lifting. The body ultimately settled where it wanted despite the changes. Nearly half the starting weight came off and BMI is now 21, holding steady within three pounds for eight months. Abs became visible, yet a hernia on one side of the lower abdomen also appeared and will need surgery.

A cyst that had probably existed for years turned up as well and caused brief alarm, though checks confirm it is harmless. Similar experience of noticing things only after the weight came off. Just turned 60 and aiming to stay healthy long-term.

BMI is outdated; waist-to-hip ratio works better. Measure waist at the navel, divide by the widest hip measurement. Aim near 0.90 for men; check online for female values.

Military standards have relied on waist-to-hip measurements for requirements as long as remembered.

Arnold at 6'2" and 235 pounds had a BMI of 30.2 yet a waist under 34 inches and a body-roundness index of 2.6, rated very lean. Shows how poorly BMI fits muscular builds.

First learned about waist-to-hip ratio in this thread and will start using it as the better guide.

IronLantern24archiveJun 6

Still five pounds from the current goal and already unsatisfied; the number will probably shift again. Toning work needs to increase because the body at 170 now differs from twenty years ago. Planning to hold reta at 5 mg for a while and observe results.

NorthCinder83archiveJun 6

After two years on the medication and 100 pounds lost, skin stays somewhat loose. Avoiding further loss helps keep a ripped appearance possible. Tightening reached about 80 percent over time but the body still would not be shown at a pool. Overall feeling is excellent even though BMI reads 26, which matters less given bone structure and lack of athletic background.

Ignore BMI, check the mirror instead to judge whether the current shape feels right. So much weight has already come off that obsessing over the scale is unnecessary.

PatientBeacon15archiveJun 6

Mirror feedback is the way to go. Planning to hold weight once stomach muscles start to appear, not aiming for full definition but some visible shape. Lower body fat paired with higher muscle supports long-term health.

IronThistle53archiveJun 6

Blood pressure tracking adds another useful signal. Readings should settle near or below 120/80 at an optimal weight. If pressure stays high once a comfortable weight is reached, a doctor visit is worthwhile to rule out skinny-fat issues.

RustQuill94archiveJun 7

Blood-pressure meds have been used since the twenties and readings have improved, yet the aim remains around 150 at 5'8". Plenty of extra weight is still visible, so another 25 pounds will be tested to see how pressure responds. Having come this far, reaching what feels optimal makes sense.

Those military standards assume male fat patterns. For many women fat sits lower, so a good ratio can still exist even when overall fat is higher.

PatientPebble79archiveJun 7

Impressive sustained loss; these medications truly change outcomes.

CopperSignal27archiveJun 7

If abs are even partly visible then another 25 pounds of pure fat is unlikely. Losing that amount could remove equal parts muscle and fat, which offers little health gain. Objective fat-percentage data helps; waist-and-neck calculations or a DEXA scan provide it. Lower BMI achieved only by losing muscle is questionable for health.

ClearLedger33archiveJun 7

Will know the stopping point once the view in the shower shows definition. Until then the focus stays on how the body feels. Even starting from much higher weight, comfort and maximizing remaining years matter more than any chart or ratio.

PatientFenwick55archiveJun 7

Personal rule is to stop at a plateau. Roughly 20 pounds above high-school athletic weight remains, so a plateau is expected before year-end and will prompt more gym time for strength and slimming. Goal is functional fitness: decent mile pace, no breathlessness on stairs, ability to jump. Dose will stay at 7.5 rather than increase; daily walks of one to two miles are already part of the routine.
