Cagrilitide pH

Summary

How should cagrilintide be reconstituted regarding pH to prevent fibrils or degradation?

Users report mixed approaches: some add acetic acid to reach pH around 4-5 and test each time, while others reconstitute with plain bac water and see no problems after months of use. Several note that newer formulations appear buffered toward neutral pH, and clinical combinations with semaglutide suggest stability improvements. No one in the thread reports actual fibril-related issues, though theoretical risks are discussed at length.

Can anyone offer advice on this? I read that the pH needs to stay below 6, so how do people manage it? Maybe three quarters of a milliliter of bac water mixed with a quarter milliliter of acetic acid? I would appreciate any thoughts since I am new to peptides and want to try cagri without making mistakes.

I can only share what happened in my own case with cagri. I had not come across any pH warnings so I just used the bac water that came with it and mixed it up. I have done several injections and nothing bad occurred.

GreyHarbour55archiveFeb 26

There are quite a few older threads discussing different methods depending on individual tolerance. Early on I used acetic acid to hit a pH of 4 and checked every shot. After several months I switched to plain bac water and the pH usually sat around 5, which felt acceptable to me. Checking past discussions is worthwhile for more personal accounts.

ClearBeacon24archiveFeb 26

I believe they add buffering now to reach neutral pH, which may explain why they combine it with sema in current trials. I have simply used bac water and it appears fine.

I just received mine this week hoping it would help break a stall and drop the last ten pounds. Now I am hearing about this pH issue. Is it something to take seriously?

From what I understand, if the pH goes above 6 it can lead to fibrils forming which is undesirable and may cause breakdown. I am not certain about the details and would like input from anyone with more background on the topic.

I mixed mine earlier today and plan to inject later. I have some pH test strips meant for checking bladder infections. Is that the method people use for this? I have not explored it much yet.

From what I have seen most people test and adjust the bac water before mixing it in.

That is how we gain experience. I made mistakes when I first started too. I would not stress over it and it should turn out okay. Good luck.

Thanks. I am unsure whether I actually need the acetic acid or if I should just go ahead and inject. I have not had any issues so far in my research but I do not want to risk my health either. I will look into it more when I have time.

I think the emphasis on low pH comes from the original version. Newer batches seem to include buffers that bring it into a normal neutral range. They needed to do that for the sema combination trials. I would not worry. I have used it for over six months with only bac water and I can tell it is active each time.

That is good to hear. What dose did you begin with? I have been stuck on reta, sema or tirz for more than a year now.

ClearBeacon24archiveFeb 27

I started at 1 mg. It lasts a long time with a half life of seven to eight days.

Last I heard the brand sema/cagri version uses a special pen with separate chambers to keep the different pH liquids apart. Has that changed?

ClearBeacon24archiveFeb 27
↳ replying to @CopperQuill49

That could be accurate, but I have also read that newer versions are buffered for neutral pH. I did note that the brand name product might differ and may not be buffered the same way.

IronCinder46archiveFeb 28

What about the claimed risks of fibrils if the pH is not kept under 6? Does anyone have thoughts on that? I really want to mix my cagri and begin using it but I still have doubts.

ClearBeacon24archiveFeb 28

What would the worst realistic outcome be? The concern mentioned online is that mixing at a higher pH could create fibrils that might deposit in tissues or organs and lead to damage or inflammation. That sounds alarming but there is no actual evidence connecting small injections of slightly higher pH solutions to amyloid issues or lasting harm. The extreme risk remains theoretical. I have looked into this quite a bit.

LevelTimber10archiveFeb 28

I am doing exactly that, mixing with bac water, keeping it in the fridge, and finishing or discarding within thirty days. I have used it for two months with no issues during mixing and no noticeable loss of effect. It seems to be working fine.

The dedicated cag discussion area has active debate with useful data. I mix to pH 4 using acetic acid and bac water, check stability with every injection, and limit use to four weeks.

WrySparrow63archiveFeb 28

I just used bac water and am hoping for the best. I started with a low dose because I have seen reports of strong reactions and I do not want to feel sick or lose appetite completely for days.

LevelTimber10archiveFeb 28

I began at 0.10 mg for the same reason. It worked well at that level and I avoided the worst sides I had read about. I moved up to 0.20 mg next time with no trouble, though getting enough food in became a challenge.

WrySparrow63archiveFeb 28

Interesting. I went with roughly 0.125 mg. I could eat a lot right now so we will see how it goes. I am still watching my intake to avoid gaining but I hope this helps finish off the last ten pounds.

LevelTimber10archiveFeb 28

I hope it works well for you. I still have about thirty pounds left to reach my goal.

I respond very slowly. It took me eighteen months to lose forty pounds and nearly another year to drop five more. I have been focused on building muscle rather than just the scale number but I would like to weigh less while keeping muscle and drop a clothing size if possible.

Is this something you wrote yourself or copied? It reads like an AI generated reassurance without real data, and of course there are no long term outcomes yet because any potential issues would take time to show. If you wrote it, could you point me to the information about the original brand using pH 4.0 specifically for stability? I have some in the freezer and have not found that detail in my own searches.

ClearBeacon24archiveMar 1

It is not my writing. It is taken from a podcast by someone active in this area who collaborates with scientists. I find the source credible. Here is the full transcript for anyone interested.

I tried 0.125 mg yesterday assuming the vial concentration is correct and had zero side effects such as tiredness. I am still very hungry though. I have never gotten strong appetite control from sema, tirz or reta either. Maybe my response is unusual.

↳ replying to @WrySparrow63

I am curious why you chose such a small amount. I began at 1 mg and noticed good suppression once I reached 2 mg per week.

Right, lots of vague statements like I am pretty sure or it should be fine with no actual data provided. Seems thin. Good luck.

I kept reading that even 0.25 mg caused vomiting and total loss of appetite for some people. I feel nothing and might even be hungrier. Should I increase the dose? Maybe I need to adjust the pH with acetic acid. I am open to suggestions.

ClearBeacon24archiveMar 2
↳ replying to @WrySparrow63

The pH level does not change how strong it is. Only extremes would damage the molecules and then it simply would not work. Between 4 and 7 I would not be concerned. As I mentioned earlier I started at 1 mg and found 2 mg effective. Some people respond strongly to 1 mg while others like me have always needed higher amounts of any compound. Start low and monitor how you feel. Keep in mind it can take up to three days for the full effect after a subcutaneous shot.

WrySparrow63archiveMar 2
↳ replying to @ClearBeacon24

Got it. I will wait and see if anything changes. I injected Saturday morning.

LevelTimber10archiveMar 28

I increased from 0.25 mg to 0.5 mg. The description that food feels like a punishment is accurate. Getting past 800 calories a day has been difficult all week and even chewing felt unpleasant. A cookie tasted like cardboard and I had to force myself to finish it, which is completely new for me.

AmberMarble84archiveMar 28

I also started at that dose the first week because of everything I had read about nausea and fatigue.
