Started on these meds back in 2022 with semaglutide from a doctor and dropped 50 pounds over the first year and a half. Insurance dropped coverage so I moved to compounded versions and switched to tirzepatide hoping for better results on the last 10 pounds. It took another year to go from 135 down to 125 at 5'4". Hit the top dose of 15 mg but side effects were rough so I dropped to 10 mg and started gaining. By April the scale was back at 130 and kept climbing with life stress and more hunger until I hit 135 again. Switched to reta seven weeks ago beginning at 2 mg for two weeks, then 4 mg for three weeks, 6 mg for two weeks, and now 7 mg this week. No scale movement despite walking, weights, yoga and other cardio. Reta seems to increase hunger for me at times but I have my food intake under control.

At 135 your BMI sits around 23 which is already in a solid range. Bodies often defend a certain weight and while these meds can shift a high set point they cannot keep lowering it forever.

I agree and would add that most people who reach the top dose then take time off usually need high doses again when they restart.

The higher doses of reta worry me because my heart rate jumped right away at 2 mg and has stayed elevated since.

With the medication and exercise already in place the remaining factor is food intake. Are you logging everything and weighing portions? Are you sure you are holding a steady calorie deficit?

If tirzepatide worked better for you why not go back to it? Not everyone responds the same way to reta even if others share success stories. I tried switching out of curiosity and saw hunger return plus digestive issues so I went back and things normalized quickly. Tirzepatide seems stronger at quieting hunger but can still cause muscle loss without enough protein and lifting.

You are already at a healthy weight. To weigh less you simply have to burn more calories than you take in either by eating less or moving more. The meds are not the main answer here.

This is actually the first time I have tracked and weighed my food since starting these meds. Since switching to reta I average 1400-1700 calories depending on activity level. Before that I was probably eating less because the hunger was lower.

Try the Lose It app set to sedentary and do not eat back exercise calories. At my height weight and age my TDEE is only about 1600 so yours is likely similar or lower. To lose you probably need to stay below that number consistently.

I agree calories matter but my intake is now tracked. The bigger issue is that 15 mg tirzepatide kept hunger very low and now on reta the suppression is only moderate. I am mainly asking whether I should keep increasing since I am only at 7 mg or add another medication. After four years on these drugs my tolerance is probably high.

I am 36. My calculated TDEE at sedentary is 1595 but I am active so to lose I would need to drop to around 1295 which feels hard on only 7 mg reta because hunger returns. Raising the dose might help control that.

I set a flat 1500 calories and ignore exercise calories. Some days I am under and some over but it averages out over time.

More details would help such as exact height. At 5'2" a bit more loss might be reasonable but at 5'9" it could be pushing too far unless prepping for competition. Most people underestimate portions even when they think they are tracking. If calories are already low consider raising them a couple hundred for a few months while focusing on strength gains so metabolism improves. After four plus years in a deficit have hormones including thyroid been checked? It is surprising you are not hungrier on 7 mg reta after coming off 10-15 mg tirzepatide since the latter is usually stronger per milligram.

When starting from a higher weight the pounds come off faster with smaller changes. Closer to goal everything requires more precision and patience. After only seven weeks and fairly quick dose increases reta may not be fully working yet. Adding the other medication could help but time might be the better first step.

I do regular bloodwork including hormones and everything is in range though that may not be optimal for me. I have been tracking calories for the past nine weeks and eating more lately. From 125 in September I have slowly gained about 10 pounds with my cycle pushing it to 135 most of the month. At 5'4" that feels noticeable even if others see it as minor. What I really wanted advice on is whether to keep raising reta after coming from 15 mg tirzepatide or add the other medication because staying at the lower calorie level is tough on 7 mg. The meds are supposed to make the deficit easier to follow.

The difficulty stopping the regain is straightforward. You have already lost over 30 percent of your starting weight. Tirzepatide at 15 mg got you there and kept you stable without much struggle until side effects became the issue. Ten milligrams was not enough to prevent regain. At this point most of the medication's job is simply preventing the natural rebound rather than creating new loss. Finding a maintenance combination with fewer side effects than 15 mg tirzepatide is not simple.

This is really about consistent tracking and patience rather than the medication. You also moved from 2 mg to 7 mg in only seven weeks which is much faster than the trial schedule of increases every four weeks. That pace can raise side effects and limit future options. At your height and weight you are already light. Healthy eating and exercise do not automatically equal a deficit unless portions are weighed and logged every time. If you are not doing that you do not yet know whether calories are truly the issue.

I have not had side effects on reta except the higher heart rate which is expected. I came from 15 mg tirzepatide after four and a half years on these medications. The main problem now is weaker appetite control so I was looking for next-step suggestions. I also stopped drinking when I started the first medication in 2022.

When appetite suppression fades after years on these medications there is no well-proven fix. Trying the other medication at a cautious pace is one option but adding more may not be the answer. You are supposed to feel hunger at times. Noticing patterns like stress or cycle phase can help. The last few pounds are often the hardest.

The reduced hunger control is mainly because the current reta dose is lower relative to the previous 15 mg tirzepatide and because so much weight has already been lost. The medication is now mostly fighting the body's drive to regain rather than any true tolerance developing. Finding the right long-term maintenance doses with the fewest side effects can take experimentation.

I gained a pound after six weeks on reta. I am using 3 mg two or three times a week and plan to move to 4 mg soon. If that does not help I will switch back because reta does not seem as effective at quieting hunger for me. I have tried a few different sources.

I recall from the trial data that the resting heart rate increase peaked around 24 weeks and then started to come back down though it had not fully returned to baseline by the end of the study.

I am 63 and have tried many diets over the years. The final 10 percent is always the hardest to lose and easiest to regain. This is my first time using these medications and I expect the last stretch to be a real fight but I am determined.

At 5'3" and 135 pounds appearance varies a lot with muscle versus fat. Monthly photos and measurements give better feedback than the scale alone. You have already made great progress so keep going.

I like measurements too. Last summer at 125 my waist was 24 inches hips 35 and upper thighs 20. Now at 135 those are 25 36 and 21.5. I am leaning toward the right side of the comparison photos but preferred my numbers from last year. The thread had some useful suggestions so I will keep trying.
