My test subject is on 7.5 tirz with 4 reta right now, along with rotating NAD+, Mots-c or similar. Added TESA for the first time last night. Weight loss stopped this past week and the scale even crept up slightly, after steady 1-2 pound drops each week for months. The bigger issue is strong hunger returning along with heart rate no longer staying up. Only five months total on the two main meds, and it feels like a high dose already, yet results shifted with the same vial. Cycle is still two weeks away.

Every few weeks I suddenly crave foods I normally avoid during weight loss, like potstickers, enchiladas, buns, cheese and pie. I put on three or four pounds, notice it in the stomach, and worry the meds have quit working. Then my period arrives and everything reverses quickly, the extra weight drops off in a day or two and appetite returns to normal. You said cycle is not close yet, but bodies can be unpredictable, especially female ones, so a single week may not mean much.

Been on tirz at 12.5 for four months after trying 15 but the side effects left me wiped out. The current dose keeps food noise down and hunger manageable, yet the stall is frustrating with a trip coming up in March. Added reta at 1 mg last Friday and again today, with only mild nausea after meals so far. Long history with tirz and sema before that makes me comfortable adjusting reta, but hunger has crept back in this weekend despite the tirz. Post-menopause and on HRT so no cycle involved. Thinking about trying survo added to tirz instead.

Tesa and other ghrelin or secretagogue compounds can reduce GLP-1 signaling when doses are high, so the tirz effect may be blunted. Those same hormones also tend to add water weight, meaning appetite control and body recomposition may not happen together.

We started around the same time and use similar doses of tirz and reta. Never feel hungry though I could still eat if needed. Pin tirz on Friday mornings and reta on Monday mornings, plus regular 24- to 50-hour fasts, and the weight keeps coming off steadily.
