Adding Cagrilintide to Tirzepatide

40 replies12 peopleApr 6, 2026β˜† Follow
Summary

What experiences have people had adding cagrilintide to tirzepatide for better appetite control and weight loss?

Users report mixed results when stacking the two. Some see strong appetite reduction that helps break stalls or quiet food noise, while others find the effect fades after a few weeks or makes eating mechanically difficult without nausea. Dosing schedules vary widely, with many starting low and splitting injections across the week. Fatigue is a common early side effect that usually eases, and one user described a severe reaction after an accidental high dose.

I'm curious about experiences from others who combined Cagrilintide with their Tirzepatide use. What led them to try that approach? Which schedule did they settle on for the doses? And what kind of outcomes did they notice? At the moment I'm using 10mg weekly of Tirzepatide. It handles most things effectively, yet the constant thoughts about eating keep undermining my attempts to follow through on my food plan. Rather than raising the TZ amount any higher, I want to test whether adding Cagrilintide might deliver enough extra hunger control to reach my daily targets.

LevelTimber10archiveApr 6

Right now I'm taking 6 milligrams of Tirz, having begun back in July 2025 with 2.125 milligrams. I introduced Cagri about six weeks back, aiming to hold off on increasing beyond 6 milligrams for an extended period or using it as a two-day link until the next injection. This medication is extremely effective in my case. Plenty of others manage fine with just 1 or 2 milligrams. For roughly three days afterward, I find it really hard to consume food. It's not that I'm feeling sick, but the act of eating itself feels unappealing. The effects seem to fade quicker compared to Tirz, yet it easily gets me through until the Tirz injection day. I didn't encounter any sickness or added tiredness. My dose increases weren't the usual way because I was worried about the stories people shared, so I did 0.10, 0.25, 0.25, 0.39, 0.5, and last Sunday 0.67. I dropped four pounds in the previous week, though nothing the one before that. I'm unsure about the real benefit from adding Cag since I can't manage sufficient intake. Split doses might be something I consider. Weigh day is coming up tomorrow, so we'll find out.

One online source recommends using 0.6 mg of cagrilintide when adding it to an existing 10 mg tirzepatide schedule. I am thinking of starting with that dose. So far the only side effect I have had with tirzepatide alone is fatigue.

KeenPebble33archiveApr 6

Same approach here with the 10mg tirz plus adding the cagri. Started the cagri at a low amount and stepped up gradually from 0.25 through 0.5 up to 2.4. The initial results were impressive right after the shot but faded off pretty fast. Ended up going higher to the 2.4 level and plan to remain there for the rest of the combination. Won't be getting more after that.

BlueHarbour72archiveApr 6

I take 12.5 mg tirzepatide on Sundays and cagrilintide currently at 1.2 mg on Wednesdays. This will be week six. It works great but the fatigue in week one and two was palpable. I titrated cagrilintide 400 mcg, 750 mcg, 750 mcg, 1.2 mg, 1.2 mg. This week I will move to 1.7 mg.

Is it that the results fade away fast which keeps you from ordering more or do you have some other explanation?

KeenSignal54archiveApr 6
↳ replying to @LevelTimber10

Yes please keep me updated, I am really interested. I would also love to know whether you are in the group that uses only one type of water or the group that takes the extra step of using something else because of the pH.

KeenSignal54archiveApr 6

Has the fatigue eased by now? And can I also ask how you reconstitute it? Do you use just one type of water or do you also use something else?

LevelTimber10archiveApr 6
↳ replying to @KeenSignal54

I am using the standard water for lazy reasons and partly because I do not believe it causes issues that would do lasting damage. I am sure someone will tell me I am irresponsible and I might be, there is no way to know. My risk tolerance is really nuanced. Someone will probably come tell me I am wrong.

BlueHarbour72archiveApr 7
↳ replying to @KeenSignal54

Yes fatigue is much better. I reconstituted with only the standard hospital-grade water.

KeenPebble33archiveApr 7
↳ replying to @KeenSignal54

After continued use the effects appear to fade completely. Consuming the whole amount still leaves me with appetite the next day. Missing an intake creates no noticeable change at all. With the other option appetite clearly drops and missing one allows it to return slowly. For that reason I have no plans to get more since results do not hold up over extended periods when trying to drop weight. At first the experience seems different.

KeenPebble33archiveApr 7
↳ replying to @BlueHarbour72

Fatigue is mostly a short-term effect of cagrilintide and resolves itself after a few doses. I also only reconstituted with standard hospital-grade water and never had any gelling problems though I use it within two to three weeks max.

GreenAnchor65archiveApr 7

Roughly eight months back I began pairing cagrilintide alongside my tirzepatide schedule. I split the amounts and administer each on two separate weekdays, which means four shots total across both. Results have left me quite content so far. Yesterday I reached a full hundred pounds or forty five kilograms lighter since the start a year earlier, though the opening four months used tirzepatide by itself. Constant hunger used to bother me until the addition, whereas lately I sometimes skip meals without noticing. A type two diabetes label originally prompted the whole effort, yet my A1c readings have stayed normal long enough that I doubt the label still applies. That label is why I wear a continuous glucose monitor, and its low sugar alerts often serve as the nudge to finally eat something. So far nothing unwanted has shown up.

These outcomes line up with the progress I want for myself. Mind laying out the timing pattern you've stuck to across the previous four months? Appreciate it.

LevelTimber10archiveApr 7

I weighed 179 this morning so 3.6 lb lost, so the cagrilintide definitely helped the pounds along. But I think I will drop it and maybe start attempting to lower the tirzepatide because I have only got about 20-30 lb left to lose. I would buy it again if needed but not a whole kit.

KeenSignal54archiveApr 7

A lot of folks land somewhere in the middle on how much downside they're willing to accept. I'm looking into cagrilintide and want to move past the abstract parts of the acidity talk. I don't buy the story about permanent harm from those strands, yet the gradual breakdown still matters to me. What I want to know is whether anyone who's kept using it for a while has seen the results weaken after several weeks, or if we're mostly dealing with guesses based on lab data. The difference between holding up for a month and needing to stop at half that time matters when you're actually doing it. Should everyday reports lean toward the shorter window, that would shift my thinking. On top of that, tossing out a container after only a couple of doses feels wasteful given how I plan to use it.

SharpCinder28archiveApr 7

Yeah the loss in effectiveness does not really make sense with the studies including one of 68 weeks. One study with cagrilintide by itself also showed results over time.

KeenSignal54archiveApr 7
↳ replying to @BlueHarbour72

Really happy your tired feeling has improved. Curious how long you tend to use one container for and if it keeps working the same way the whole time.

People have every reason to feel wary around Cagri, but I can act foolish at times and the urge to test it beat out any worry about fibrils. Without AA nearby the urge grew too strong to ignore, so I carried on with bac anyway. My shifting comfort level with uncertainty clearly lacks logic. A few weeks back I was saying the opposite on this board about limiting myself to regulated products only.

BlueAlder12archiveApr 7
↳ replying to @SlowCompass69 (opening post)

I got stuck about ten pounds below my target even though I was already at the highest amount of tirz. Adding the other compound finally got things moving again once I reached the upper dose, though I kept the shots on separate days at first. Lately I have been trying to settle into something steadier for holding my weight, but the pattern keeps shifting. I usually just watch until the hunger signals return and then inject again, often after about a month. Dropping tirz down by itself did not hold things in check. A few weeks later I tried the full tirz amount together with the other med on one day and ended up feeling wiped out. The nausea hit hard the next day and I actually threw up early Sunday morning, bringing up what I had eaten Thursday. I skipped meals until Monday and even then barely touched anything. Roughly a month earlier I had dealt with a rough case of colitis that the doctors blamed on a virus, so maybe my stomach and intestines were still sensitive. Still, the combination did stop me from wanting food.

I see what you mean now. I had thought about adding this to what I'm already taking. It's unfortunate when the effects don't last very long.

KeenSignal54archiveApr 7
↳ replying to @GreenAnchor65

That's a fantastic achievement, congratulations on such a strong outcome. It's quite intriguing to see the effectiveness of pairing those together. Are you mixing solely with BAC? If you're comfortable mentioning it, what amounts do you use for each one? Also, when beginning cagri, what was the first dose and in what steps did you raise it?

KeenSignal54archiveApr 7
↳ replying to @LevelTimber10

It's amusing to see such a sudden change from insisting on full regulatory clearance to simply proceeding anyway. People clearly differ a lot in how much uncertainty they can handle, and those differences can reach total extremes.

KeenSignal54archiveApr 7
↳ replying to @SharpCinder28

That's pretty intriguing, I've encountered comparable information myself, which makes the contrasting reports unexpected. It seems results differ quite a bit depending on the individual. Hoping to end up among the fortunate few 😎

KeenSignal54archiveApr 7
↳ replying to @BlueAlder12

Wow that kind of response appears quite severe. It clearly demonstrates the potency of combining them together. While you worked on ending the plateau phase were you taking a single dose per week for each medication separately?

BlueAlder12archiveApr 7
↳ replying to @KeenSignal54

I guess the first one happened that Monday while the follow-up came on Thursday.

CopperPebble51archiveApr 28

My routine involves roughly eleven milligrams of tirz on a five-day cycle. Appetite started returning so I experimented with Cagri. The smaller amount had zero impact yet the larger one matched the initial strong effect from tirz. Food held no appeal at all. These days I reach for it only during periods when tension-related appetite increases. This makes it more of an occasional tool rather than regular. It's been a while since my last injection but reassuring to have available if appetite starts increasing again.

IronLedger65archiveApr 28

Curious how folks keep this stuff when not using it right away. Does everyone prepare a fresh container whenever needed? Anyone chilling the remainder in the freezer for later sessions?

SlowCompass69archiveApr 28
↳ replying to @CopperPebble51

This method seems promising. What was the delay before noticing any results once the amount reached half a milligram? Besides that, for occasional use, does the quantity remain unchanged at that level? Once the occasional portion gets taken, when does the result show up?

CopperPebble51archiveApr 28
↳ replying to @IronLedger65

Whenever I take some, the leftover container stays around for possible later needs. It goes into cold storage for two months before I get rid of it. Mixing always uses sterile water that's been stored properly. I steer clear of extra uncertainties in the peptide mix by avoiding unreliable liquid.

CopperPebble51archiveApr 28
↳ replying to @SlowCompass69

My dosage level has remained at half a milligram even though I take it infrequently. The material carries quite a kick. Hunger disappears entirely the next morning after I administer a dose. In the period when I took the weekly version of the main compound it bridged those final two days effectively. A few months back I moved to a five-day interval with that compound and ever since I have had almost no reason to reach for the add-on.

CopperSignal46archiveApr 29
↳ replying to @CopperPebble51

Thanks for replying. I'm taking 7.5 of Tz on a 5-6 day schedule to manage the appetite that returns near the close of each period. That works out to roughly 10.5 across a week. Since I increased the amount following the four-week guideline, I had concerns about exhausting my supply early. I've been considering cagri seriously. Tz handles most things well, yet sudden hunger sensations appear unexpectedly on certain days. The notion of keeping cagri ready for occasional use appeals to me. Your thoughts helped me make up my mind to get some.

I need to add my thoughts to the discussion. Yesterday my wife’s experimental subject got a dose of three point seven five milligrams of Cagri in zero point seven five milliliters of the bacteriostatic liquid instead of the regular seven point five milligrams of Tirz. Right now she feels extremely sick to her stomach and is vomiting so much that it has become dry heaving. I need to know what problems to look out for and ways to make her feel better. Since she takes care of everyone else in the household this situation falls outside what I usually handle. Thanks for any input. I have set out some water along with fluids that have electrolytes for her to take in small amounts. Up until this point she never had any reactions while building up to seven point five milligrams of Tirz. She often reacts strongly to mixtures of medications. She has not tried Cagri at any time prior to this incident. The person involved is a sixty five year old woman who is otherwise healthy. According to my figuring a five milligram bottle of Cagri combined with one milliliter of the bacteriostatic liquid given as a seventy five unit shot under the skin equals three point seven five milligrams. A calculation program confirmed the numbers. We are avoiding the weekly seven point five milligram Terz injection at this time.

That amount for the initial compound looks like it could be excessive from what I've come across. One diagram pointed to beginning much lower when pairing it with the other substance, but only after slowly raising the second one by itself across quite a few weeks. Checking every bodily sign and heading to a hospital for caution seemed reasonable to me, as staff there could supply whatever treatments might be required.

↳ replying to @PatientAnchor11

I'm no medical expert by any means but losing too much body water looks like the main risk from taking too much. Because of how old she is if holding down liquids turns impossible then heading to the physician for some vein hydration might help especially when the issue drags past a couple days. I'm thinking about starting Cagri with a quarter milligram along with the seven point five Tirz dose I already use.

I kept some medication ready just in case. Placed it beneath my tongue to dissolve. Possibly noticing a little progress. Taking tiny amounts of clear drinks, though they rarely stay down. What comes up is transparent yellow disgusting liquid. Appreciate it.

Kept medication ready beforehand. It melts beneath the tongue. Possibly detecting minor progress. Consuming small portions of plain beverages though most return upward. Expelled material shows as transparent yellow unpleasant liquid. Thanks

I figured taking her to the emergency room right away would let her feel better instead of dragging it out. The quick clinic might manage the medicine amounts or provide different ones. One anti-nausea pill works at frequent intervals or increased quantities. Another drug gets ordered for delayed gastric emptying and it eases queasiness, puking, plus indigestion. A serious thing to prevent is potassium levels falling dangerously.

SlowCompass69archiveMay 11

I watched closely for symptoms including chest discomfort, difficulty catching breath, intense belly ache, elevated temperature, or changes in awareness. Considering the person's years, I made sure to get emergency services involved right away if things got out of hand.

3.75 mg is an insane starting dose. I am 6 foot 3 and 350 lb and at 0.5 mg I can barely bring myself to eat anything. I do take tirzepatide as well.
