Hi everyone, I began reta in February but the last two months my gallbladder and liver enzymes have all risen. Do I need to quit the medication to fix it or does anyone have ideas for bringing the levels down and easing possible bile blockage

This looks like the usual bile acid bottleneck. Plenty of hydration, more dietary healthy fats such as eggs olive oil and avocados, extra fiber, no more than two to three pounds lost per week, skip fried foods, and get some exercise. I changed greasy to fried because that seems like clearer guidance.

What exactly do the labs read and how much fat are you taking in. Low fat diets during weight loss can stress the gallbladder since it stops emptying properly. The type of fat does not matter much for getting the gallbladder to contract. I just had a prime NY strip and will call that medicine for my gallbladder.

Has a doctor checked this already. Reta and similar GLPs can rarely cause real liver toxicity though minor enzyme bumps are more common and sometimes settle if you keep going. An expert needs to decide on stopping or not. You did not say how high the numbers are or which enzymes so hard to tell. Slightly over is usually minor but three to five times normal is more serious. There were six toxicity cases in Victoria Australia recently thought to be from a contaminant in grey market reta though no details on what it was. You seem to be in the US so probably unrelated but worth noting. Liver patterns usually split into cholestatic with raised bilirubin which matches what you describe though gallbladder blockage is not the typical cause.

Yes this can happen with fast weight loss and reta might add to it. Gallstones are a known risk during quick loss so the drug itself may not be directly harming the liver. Which enzymes are up matters. AST ALT differ from alkaline phosphatase GGT and bilirubin which point more to bile trouble. Since it has lasted two months get it checked instead of trying to fix numbers alone. An abdominal ultrasound is an easy first step. You might not need to stop reta for good but loop in your doctor especially if loss has been very fast. Avoid crash dieting as that can worsen stones.

One more thing I forgot to ask. Any symptoms besides the lab numbers. Speaking with a doctor is sensible on its face. The rest drifts further from solid ground. Medical talk here does not equal practicing medicine. The original poster is from Canada while most others are in the US so First Amendment protections apply. This is no different from other medical threads the forum has hosted. Three other gallbladder threads already exist with similar info.

See your physician they can run a liver panel and order an ultrasound. Also worth checking other medications and hormones you might be using. Hope this helps.

I am not cutting weight or in a deficit right now. I use reta for chronic food noise and to stop binge eating after an aggressive cut in January. No symptoms except feeling light headed or dizzy if I stand too fast. Bilirubin total twenty umol per L, potassium five point four, GGT forty six which is just over the normal under forty four, ALT fifty seven over normal under thirty six, AST fifty three over normal under thirty. Amylase and lipase are high normal but not above. I eat high protein no fried foods though weekends have more sugar and I have always binged then. I am not overweight at fifteen percent body fat as a female.

Reta seems odd for food noise since I use sema or tirz for that and reta for cutting. In a peptide powered cut enzymes up to about three times the upper limit are not very diagnostic by themselves. Any symptoms besides the labs. Exercise can raise AST and ALT. I had a mildly high reading months ago blamed on training and other labs showed it was nothing. Fifteen percent body fat is quite low for a female. How was that measured.

Those numbers sit at the top of normal or just over so not an emergency. When my own enzymes were mildly up the doctor said ignore it and recheck in three or six months. Gallstones are unlikely here they usually cause pain if moving or stuck and only affect enzymes dramatically then. Age health starting weight current weight other meds and prior tests would help. Many drugs raise enzymes. If levels were already off before reta or loss then MASLD could be likely and ultrasound or fibroscan useful. Normal results beforehand would make reta more suspect. You can do these things. One check lipase especially if over one hundred eighty. Two get an ultrasound on the gallbladder. Three if stones show up you will probably get prescribed UDCA or ursodiol to dissolve them. Significant weight loss with or without GLP ones can cause this so check anyway.

Only if you do not drink alcohol. TUDCA or any bile salts with alcohol can harm the liver.
