Retatrutide stagnating after 3 weeks?

45 replies19 peopleMay 15, 2026☆ Follow
Summary

Why does retatrutide appetite suppression weaken mid-week after a dose increase, especially when already lean at 12% body fat?

Users report hunger returning by day 3-4 after moving to 2 mg despite fast fullness on eating, with prior stronger early-week control at 1.5 mg. Half-life around 6 days explains the pattern for many, leading to suggestions of splitting doses or stacking tirzepatide or cagrilintide for steadier effects. Individual responses differ sharply even at low body fat, with some experiencing less hunger at low doses while others note increased food noise or glucagon-driven hunger at higher ones. Clinical data comes from higher-BF participants so applicability remains unclear, but weight loss still occurs and stacking is commonly explored for better satiety without raising reta alone.

What this discussion establishesWhere people disagree

Whether retatrutide reliably reduces food noise compared with tirzepatide, and whether higher doses help or worsen hunger via glucagon effects in already-lean users.

Still open

Long-term maintenance approach for naturally hungry lean individuals, optimal low-dose stacks, and whether metabolic adaptations at very low body fat differ from study populations.

Nothing here is advice.

Quick question on my retatrutide run. I bumped up to 2 mg recently after using 1.5 mg every five days starting from April 23. Now hunger returns noticeably by day three or four. I still fill up quickly when eating but get hungry again after three to four hours. Earlier on, the appetite control was stronger and steadier right after the shot. Since I'm dieting at about 12 percent body fat, perhaps my leanness is causing more resistance. Anyone else seeing this? Is it normal adaptation or am I messing up?

GreyLantern40archiveMay 15

Nothing unusual here. You're still on the starting dose used in the trials. I'd move straight to 4 mg per week and stay there. The TRIUMPH-4 trials begin at 2 mg weekly then step up every four weeks through 2, 4, 6, 9, and 12 mg.

WarmAnchor46archiveMay 15

Am I mistaken? I've seen multiple articles claiming Reta doesn't reduce food noise because its third receptor is glucagon and tricks the body into burning fat. I'll be splitting tirzep and Reta each week. Bill

You are mistaken. It doesn't quiet food noise as effectively as tirzepatide or semaglutide, so anyone coming from those might feel let down. For most people without prior GLP-1 use it still provides enough suppression.

WryAnchor38archiveMay 15

Retatrutide gives weaker hunger control than tirz or sema. For stronger suppression I stack it with tirzepatide, which is what I do at 2.5 mg tirz plus 1 mg reta. In general hunger tends to rise later in the week because retatrutide and GLP peptides have roughly a six-day half-life. Some split the weekly amount into smaller shots every few days to keep levels steadier. Pinning every three days is one option people try.

I gave myself the shot by pushing the syringe plunger.

RustSignal22archiveMay 15

I'm going to try that, but at this stage Tirz seems cheaper and more reliable if the main goal is hunger suppression.

Depends on your goals. Trials show greater average weight loss on retatrutide.

I have to look into stacking both properly because I read that Reta's glucagon effect only kicks in at higher doses starting around 6 mg and 1 mg is basically

That's correct. I overlooked that detail. I haven't gone through them, but I wonder if any low-body-fat people were included in the TRANSCEND-T2D-1 trials.

I understand the research focuses on heavier individuals, yet that doesn't seem connected to my situation here. The way it affects hunger signals and cravings ought to function identically regardless. Weight percentage only matters if progress had stalled, which isn't happening. Weight is dropping steadily and I'm consistent about hitting protein targets. The real problem involves those cravings and appetite returning sooner than anticipated. Away from any plan, it's simple for me to consume thousands of calories daily, since I'm someone with a big appetite by nature and I truly enjoy eating.

PatientFenwick39archiveMay 15

Okay, then diet fatigue isn't the issue. 6000 kcal? Sounds like you're not eating clean. Hitting 6k on junk food is easy, but keeping or going below 12% body fat requires changing the diet. I doubt you could reach 6k on just oatmeal, sweet potatoes, vegetables, and meat or fish. It's a tough truth, but very low body fat and loving food don't mix. Sacrifices are required.

If hunger suppression is the priority, Tirz is definitely stronger. I stack both with a 3.5-day offset at T 5 mg plus R 4 mg. Once my Cagri arrives I'll test the Reta-Cagri combination. Has anyone tried that?

RustSignal22archiveMay 15
↳ replying to @PatientFenwick39

I'm in a deficit now so I'm at 2000 calories and checking weekly whether to drop further or hold. I'm just saying I can handle large volumes of food in general. Clean food doesn't always satisfy that volume craving. Right now I'm eating only clean; my treats are whey with berries in low-fat Greek yogurt for the sweet tooth, though sometimes a stroopwaffle or whole bag is needed.

With this particular mix, what kind of effects are you noticing? Have there been any unwanted reactions? I'm curious in a serious way about how your digestion is holding up, because I'm concerned I might struggle to have regular movements when using similar amounts together.

Welcome to the low body-fat club. The only way to stay there is to treat food as fuel instead of something to enjoy. Your gastronomic life will suffer from now on. No more stroopwafel for you. If you don't have a specific goal like a competition or photoshoot I wouldn't even bother—it's not worth it. On a more helpful note, try tirzepatide or reta plus cagrilintide if you need hunger suppression. When you really want to get shredded you might as well consider stronger compounds.

6000 calories—that was literally me before reta because I hit the gym twice a day. I started reta four weeks ago and think I'm in the same situation. I don't know my exact body-fat percentage but the scale says 10.6 percent, though I'm not sure how accurate that is. I'm extremely lean and trying to get even leaner. I've been eating 1500 calories since starting reta, but my experience has been the opposite of yours. I'm on 1 mg every week, never hungry, always feel bloated on an empty stomach, and never crave anything. As soon as I start eating, within ten minutes or just a few bites I'm completely full, and after finishing a meal it feels like I ate a lot even though the calories are low. My suggestion is to eat very voluminous food. For me the problem is the reverse—I need to eat small volumes to hit calories because every day is a hassle and I have to force food down.

AmberCinder46archiveMay 15

I haven't run into any troublesome effects yet. Nothing resembling queasiness, loose stools, exhaustion or the like. Digestion does slow a bit, but it stays manageable when I keep up with produce and fluids. Letting that slide makes daily stuff tougher. You could say I picked up the lesson through rough experience.

My height comes to 1.82 meters, and my scale currently reads close to 74 kilograms while glycogen sits low. Reaching roughly 10 percent body fat should put me near 71.5 kilograms in that same depleted state. Replenishing the stores afterward would leave me around 76 kilograms at about 10.5 percent body fat, based on what I've seen before.

GreyPebble84archiveMay 15

Living with this thyroid condition makes getting through each day pretty tough. Lately I've been thinking about tossing another compound into my current routine just to check whether it might ease this new urge to munch all the time. Stopping that other prescription left me with this weird nonstop hunger I can't figure out.

I break up the amount and do the shots on two different days in the week which works out okay for most folks and when that falls short I just raise the total.

AmberTimber37archiveJun 11
↳ replying to @RustSignal22

I understand the pins and needles feeling relates to how glucagon works in Reta. This happens to me whenever I take four milligrams. That whole six milligram idea could just be something made up online. Regardless, I reached my target body mass quite quickly and experienced the tingling, quicker pulse along with impressive fat drop. Started at two hundred sixty two and ended at one hundred ninety.

QuietSignal61archiveJun 11
↳ replying to @AmberTimber37

Twice is the only time I've felt that prickly sensation on my skin, right after the initial injection of half a milligram, and then again shortly after reaching about one and a half milligrams altogether. These days I'm dosing two milligrams every three days, which adds up to roughly four point six six milligrams weekly. Everything else feels fine except my appetite is gone and food moves through me really slowly haha

PatientFenwick91archiveJun 11

Being lean myself, I raised the weekly amount to eight milligrams and saw appetite plus constant thoughts of eating increase right along with it. Not sure if one caused the other, yet the two clearly tracked together. Plenty of users claim this compound fails to quiet hunger the way similar ones do, but mine actually grew stronger instead. Food crossed my mind nonstop. I thought about adding a different option yet tried pairing with another agent. That pairing seems to make a difference. When stores of glycogen run low, the glucagon part probably won't ease appetite. The second agent hits separate pathways, so combining them felt logical instead of pushing the first dose higher. Right now I'm slowly lowering it toward one milligram while keeping the pair, before moving across to a final option.

BrightThistle70archiveJun 11

I've been wondering about those extremely lean individuals who sit near ten to thirteen percent body fat. Did they drop a large amount of weight through certain compounds and now look for ways to curb hunger while holding steady, or did they start out slim by nature, enjoy big meals, and simply want help managing their appetite? When someone stays that lean even after consuming huge daily amounts of food, it points to a body that already burns fuel fast. My reason for asking is to get a sense of what others aim for, which might help me picture how my own outlook could change once I hit my target and reach a similar level of leanness.

RustSignal22archiveJun 11
↳ replying to @BrightThistle70

It's impossible for me to consume that many calories daily without gaining fat. I don't believe claims like that hold up. My plan is to get my body fat percentage below ten percent first, then start a controlled bulk afterward. Since I have decent muscle mass already, I don't consider myself skinny. To me, skinny means either having minimal muscle at a low body fat level, appearing frail, or weighing little and looking slim in outfits while carrying excess fat, similar to the skinny-fat look.

BrightThistle70archiveJun 11
↳ replying to @RustSignal22

That remark wasn't aimed at any one person in particular. You're clearly not slim, which I accept. Body fat readings come across quite differently when comparing someone who's naturally lean against someone who's muscular, even at identical scale numbers. My question didn't come out right the first time. What I meant to ask those who began very heavy, dropped a massive amount, hit their target, and are now fine-tuning their shape is how they handle meals and what they picture ahead. Are they still using the substances to hold on to appetite control, or have they moved into an all-or-nothing frame of mind? Are they thinking about eventually dropping them for plain nutritional support, or do they expect to stay on them forever? I'm wondering about my own later years. Could I just keep going without end, or might I get fixated on chasing a look that stays out of reach by always wanting one more improvement? The broad health upsides from these substances are clear, yet some accounts leave me uneasy when people stack five, six, seven, or even more to manage, block, or boost different effects.

SlowThistle88archiveJun 12

I have been checking into pairing those two together since the second one aids hunger management along with promoting fullness.

CopperSignal27archiveJun 12

Little is known about how these medications work in lean individuals without diabetes. Effects might vary quite a bit between someone already at twelve percent body fat and those in the thirty to forty percent range where most studies have focused. A normal metabolic and hunger control system operates differently from the disrupted state seen in metabolic syndrome. When heavier people shed pounds their systems appear to cut energy use sharply in order to hold on to the remaining fat stores. What occurs when very lean or physique focused people push even lower remains unclear because that situation has not been examined medically. It is treated as a competition preparation matter rather than a clinical concern so the degree of resistance the body mounts against dropping those final kilos is unknown though adaptations that protect fat are certainly possible. Raising the amount taken would likely strengthen appetite control yet few comments here addressed unwanted reactions. Smaller amounts can still produce solid results on average though the process stretches out to roughly twelve months while quicker progress would probably call for larger amounts.

BlueKettle52archiveJun 12
↳ replying to @PatientFenwick91

For those who carry less body fat this compound seems to hit a bit differently. Early on I noticed a touch of appetite but that faded completely after several weeks while I stayed at the same weekly amount now reaching the ninth week. About seven days back I added the other one on top and the effect stands out sharply. Appetite stays gone yet the feeling of being satisfied has jumped way up leaving me full from morning to night. I see why combining them makes sense since they hit separate routes both slowing how fast the stomach empties and boosting that full sensation so they work nicely together.

PatientTimber45archiveJun 12

I aimed for a steady phase with reduced hunger signals by taking two point five milligrams weekly of one option through a single shot which worked well or else beginning at four milligrams of another in one injection while holding steady around twelve percent body fat where dropping more weight just gets tougher or happens more slowly the third choice wipes out appetite too strongly and I preferred to skip that so I looked for a comfortable balance where hitting two thousand one hundred calories each day feels mildly awkward without turning into an ordeal to get down.

SlowLedger31archiveJun 12

Despite cutting way back on portions from earlier habits, I still ended up raising my aerobic time by quite a lot. That now totals around two hours every day. The numbers on the scale have begun shifting once more. Not complaining or anything, heh.

PatientFenwick91archiveJun 12
↳ replying to @BlueKettle52

It seems the mix suits you well. From what I've seen, those pairings suit people who aren't carrying much extra weight, particularly when keeping amounts small. But with the first one alone, things went differently for me—I started feeling much more hungry as weeks passed. Eventually, food occupied my thoughts nonstop, day after day. My weight stayed exactly the same even though the appetite grew stronger. Maybe the extra hormone effect was overwhelming. So I'm planning to lower the first and raise the second to locate my ideal balance.

BlueKettle52archiveJun 12
↳ replying to @PatientFenwick91

This mix hits the perfect balance for my needs. These medications seem particularly effective on people who aren't carrying much extra weight, since smaller amounts mean the substance spreads more intensely relative to total body size. I had the same idea regarding increasing the amount of reta, as that boosts the glucagon part, leading to higher energy use and thus more appetite that fights against the benefits from the other two receptors. It feels odd and backwards because the point of using reta is to drop pounds without feeling hungry, yet too much of it activates glucagon excessively and hunger returns. That's the reason the one milligram level suits me so nicely, with no appetite or desires at all, and although glucagon is active it's not excessive so hunger stays away while fat keeps coming off steadily each week. It really is an ideal middle ground.
